Among its activities the BIAL Foundation manages the BIAL Award, created in 1984, one of the most important awards in the Health field in Europe. The BIAL Award rewards both the basic and the clinical research distinguishing works of major impact in medical research.
The BIAL Foundation also assigns Scientific Research Scholarships for the study of neurophysiological and mental health in people, arousing the interest of researchers in the areas of Psychophysiology and Parapsychology.
To date the BIAL Foundation has supported 461 projects, more than 1000 researchers, with research groups in twenty-seven countries, resulting, until April 2013, in about 600 full papers, out of which 172 published in indexed international journals with an average impact factor of 3.6 and a substantial number of citations (1665).
Since 1996 the BIAL Foundation organizes the Symposia entitled "Behind and Beyond the Brain", a Forum that gathers well renowned neurosciences speakers and the BIAL Foundation Fellows which are spread around the world.

Abstract/Results: RESULTS:
The activity of the brain in pain modulation during chronic pain is changed. In general, the net balance of descending modulation during chronic pain consists on a decrease of descending inhibitory actions and an increase of facilitatory effects. Synchronous changes in neuronal responses at the spinal cord and pain control centres in the brain were detected. Variations in the expression of neurotransmitters and receptors in pain control centres of the brain may account for the effects of chronic pain. To target pain control centres of the brain, we elected a gene therapy approach based on the sustained and directed effects of the technique. The overexpression of pre-proenkephalin using a replication-defective vector derived from Herpes Simplex type 1 (HSV-1) in a pain control area of the brain (the caudal ventrolateral medulla), decreased behavioural responses in an inflammatory pain model, the formalin test, and inhibited nociceptive responses of spinal neurons. A reversal of behavioural pain responses was also induced in a traumatic neuropathic pain model using a HSV-1 vector in which a tissue-specific promoter (tyrosine hydroxylase; TH) controlled the expression oh the TH transgene inserted in an antisense sequence. Using this vector, the decrease in the release of noradrenaline in a pain facilitatory centre (the dorsal reticular nucleus) induced long-lasting antinociceptive effects, detected during 22 days. Gene therapy may be considered an excellent molecular tool to understand the effects of chronic pain installation in pain control circuits of the brain. Furthermore, based on detailed morphofunctional knowledge of pain control centres of the brain and by electing the vector type, determining the better promoter and the suitable transgene for the construct, the effects of chronic pain may be corrected using gene therapy approaches.

Abstract/Results: ABSTRACT:
Chronic pain induces functional alterations of the endogenous pain control system namely in the modulation of nociceptive transmission at the spinal cord. We used the c-fos expression as a tool to study correlated neuronal activation, induced by bending the inflamed paw of monoarthritic animals, between the spinal dorsal horn and medullary centers belonging to the endogenous pain control system, namely the lateralmost reticular formation of the ventrolateral medulla (VLMlat), the lateral reticular nucleus (LRt), the dorsal reticular nucleus (DRt), the nucleus tractus solitarius (Sol) and the rostroventromedial medulla (RVM). Awake monoarthritic rats were subjected to 4 min of paw bending followed by anaesthesia and perfusion either immediately or 2 h later. The numbers of Fos immunoreactive neurons in the spinal dorsal horn and in the medulla oblongata were significantly correlated mainly immediately after stimulation: lamina I correlated with the VLMlat, LRt, Sol and RVM; lamina II correlated with the VLMlat, LRt and Sol; and laminae IV-V correlated with the VLMlat and LRt. Between medullary pain control centers significant correlations occurred immediately and 2 h after bending at the VLMlat-Sol and LRt-Sol, at the VLMlat-LRt and VLMlat-RVM in animals perfused immediately, and at the VLMlat-DRt and LRt-RVM in animals perfused 2 h later. These data demonstrate that the mobilization of a chronically inflamed paw triggers intense correlated neuronal activity in several areas of the somatosensory system, indicating functional relevant links in pain control.

The endogenous pain modulatory system is a complex network of brain areas that control nociceptive transmission at the spinal cord by inhibitory and facilitatory actions. The balance between these actions ensures effective modulation of acute pain, while during chronic pain the pronociceptive effects appear to prevail. The mechanisms underlying this imbalance were studied as to the role of two medullary components of the pain modulatory system: the dorsal reticular nucleus and the caudal ventrolateral medulla, which function primarily as pronociceptive and antinociceptive centres, respectively. Both areas are connected with the spinal dorsal horn by closed reciprocal loops. In the spino-dorsal reticular nucleus loop, the ascending branch is strongly inhibited by spinal GABAergic neurons, which may act as a buffering system of the dorsal reticular nucleus-centred amplifying effect. In the spino-caudal ventrolateral medulla loop, the ascending branch is under potent excitation of substance P (SP) released from primary afferents, which is likely to trigger the intense descending inhibition detected in acute pain. During chronic pain, the activity in the lateral reticular formation of the caudal ventrolateral medulla changes, so that the action of the caudal ventrolateral medulla upon SP-responsive spinal neurons shifts from inhibitory to excitatory. The mechanisms of this modulatory shift are unknown but probably relate to the decresed expression of µ-opioid, d-opioid and GABAB receptors. Normalizing receptor expression in the caudal ventrolateral medulla or controlling noci-evoked activity at the dorsal reticular nucleus or caudal ventrolateral medulla by interfering with neurotransmitter release is now possible by the use of gene therapy, an approach that stands out as a unique tool to manipulate the supraspinal endogenous pain control system.

Chronic inflammation affects the activity of the supraspinal endogenous pain modulatory system, with an enhancement of facilitatory actions. A pronociceptive action, conveyed by a direct bulbospinal pathway, was ascribed to the dorsal reticular nucleus (DRt) in acute and sustained pain models, but the role of the DRt during chronic inflammation has never evaluated. Based on the expression of opioid receptors in spinally-projecting DRt neurons, we used the monoarthritic rat to evaluate the behavioural effects of local administration of a Herpes Simplex-type 1 vector encoding proenkephalin (HSV-1-PPE) in comparison with the opioid receptor agonists [D-Ala2, Glu4]deltorphin (DELT) and [D-Ala2, NMePhe4Gly-ol5]enkephalin (DAMGO). Male Wistar rats received an intraarticular injection of 50µl of saline (control) or complete Freund's adjuvant (monoarthritic) and 7 days later a guide cannula was implanted in the animals used in the pharmacological study. At the 14th day of monoarthritis, the animals used in the gene therapy study were injected in the DRt with 2µl of HSV-1-PPE or HSV-1 vector with lacZ. Paw withdrawal latencies (PWL) were determined 2, 4, 7, 10 and 14 days after viral injection. In the animals used in the pharmacological study, 0.6 µl of saline, DELT (1.2, 12,120 and 1200 ng) or DAMGO (0.1, 1, 25 and 50 ng) were injected in the DRt and PWL were determined at 15, 30, 45 and 60 minutes post-injections. A one-way ANOVA followed by the SNK post-hoc test was used for each time point studied. After HSV1-PPE injection, nociceptive thresholds were higher in controls at 2 days post-injection and lower in monoarthritic rats at 2 and 4 days. In control animals, PWL decreased after DELT or DAMGO injections, with maximal effects at longer times. In monoarthritic rats, the effects of DELT were similar whereas 50ng of DAMGO increased the PWL. The results suggest that the effects of HSV-1 are due both to actions upon DRt neurons and other brain afferents. The different effects of opioid modulation in control and monoarthritic animals support previous proposals of the adaptative mechanisms during chronic pain.

Abstract/Results: ABSTRACT:
Chronic pain induces plastic changes in neuronal activity in spinal cord and supraspinal regions. We recently detected a rapid Fos induction in brainstem neurons upon brief mobilization of the inflamed joint in monoarthritic animals. Here we evaluate if spinal cord neurons present a similar ability for a rapid c-fos induction by studying Fos expression after 4 minutes of noxious or innocuous stimulation of the inflamed paw.
In accordance with international ethical guidelines, male Wistar rats received an intraarticular injection of saline (control animals)
or complete Freund´s adjuvant (monoarthritic rats-MA). After 14 days, MA animals were perfused without further manipulation or
subjected to 4 minutes of bending or pinching the inflamed ankle. These noxious stimulated animals were perfused immediately or 2 hours after the application of stimuli. The numbers of Fosimmunoreactive cells were counted at the dorsal horn of L4-L5 spinal segments and compared by the ANOVA test.
Control animals and MA rats without stimulation presented similar numbers of Fos positive neurons. According to the applied stimulus, a differential activation of c-fos in laminae I, II, III and IV-V was detected. Pinching an inflamed paw induced a 3-4 folds increase in the numbers of Fos-positive neurons in all laminae in relation to normal animals perfused 2h after stimulation. In MA animals subjected to innocuous stimulation and perfused immediately an increase in Fos expression was detected in laminae I, II and IV-V while in MA animals perfused 2h after the onset of the stimulus the increase occurred only in laminae I and IV-V.
An adequate time after the onset of noxious cutaneous stimulation is necessary for an increased noxious-evoked Fos expression in the spinal cord of MA animals. By contrast, innocuous articular stimulation appears to induce a rapid increase of c-fos. These data suggest that, during chronic pain, the pain control system undergoes several adaptative mechanisms in order to process different types of noxious stimulus.

During chronic pain, the supraspinal pain modulatory system undergoes plastic changes with enhancement of facilitation transmission at the spinal cord. The changes induced by chronic pain at descending modulation often affect opioidergic modulation, and were never described for a key facilitatory component of the system, the dorsal reticular nucleus (DRt). Neurochemical characterization of the DRt-spinal pathway showed that d-opioid receptors are positioned as to indirectly modulate the activity of non-projecting DRt neurons, whereas neurons expressing µ-opioid receptors project to the spinal dorsal horn or act as interneurons, the latter of which co-expressing GABAB receptors. In monoarthritic rats, the expression of µ-opioid receptors decreased in the DRt whereas the levels of endogenous enkephalin remained unaltered. To increase the opioidergic inhibition of the DRt, we locally injected selective agonists of d- and µ-opioid receptors or a viral vector containing the human preproenkephalin transgene. Injection of the Herpes Simplex viral vector encoding preproenkephalin induced thermal hypoalgesia in non-inflamed animals and hyperalgesia in monoarthritic rats. The opioid agonists [D-Ala2, Glu4]-deltorphin (DELT) and [D-Ala2, NMePhe4Gly-ol5]-enkephalin (DAMGO) induced thermal hyperalgesia in both non-inflamed and monoarthritic rats, but with lower doses in the latter group. The present study shows that opioidergic neurons at the DRt are modulated by GABAergic cells herein controlling the descending facilitation of pain transmission. The DRt exhibits plastic changes during chronic inflammatory pain, with decrease opioid receptor expression which may account for increased descending facilitation during chronic pain.

The orbitofrontal cortex (OFC) is fundamental for the update of reward value in decision making tasks in humans and animals. Previous works in rats have shown that lesions of the OFC, as well as chronic pain lead to impairment in the Rodent Gambling Task akin to the Iowa Gambling Task (Bechara et al., 1994). Here we used multielectrode recordings to study orbitofrontal cortex activity in rats performing this task during control and chronic pain conditions. As expected, control animals developed preference for low risk choices while monoarthritic rats developed preference for high risk choices. Neuronal activity showed that control animals present a higher proportion of cells that differentially fire for high and low risk choices during the whole task, while monoarthritic animals only present this differential activity in the beginning of the task. Comparison of neuronal responses to rewards showed that control animals presented a higher proportion of cells with different firing rates for high and low risk rewards, but that the pain group presented an increased number of cells that differentially fired for rewarded and non rewarded trials in the low risk lever. These results show for the first time altered orbitofrontal cortex processing during decision-making deficits in chronic pain conditions and support the notion that chronic pain alters the reward/aversion system.

Acting efficiently in the world depends on the activity of motor and somatosensory systems, the integration of which is necessary for the proper functioning of the sensorimotor loop (SL). Profound alterations of SL functioning follow spinal cord injury (SCI), a condition that brings about a disconnection of the body from the brain. Such disconnection creates a substantial deprivation of somatosensorial inputs and motor outputs. Consequent somatic deficits and motor paralysis affect the body below the lesion level. A complete restoration of normal functions of the SL cannot be expected until basic neuroscience has found a way to re-establish the interrupted neural connectivity. Meanwhile, studies should focus on the development of technical solutions for dealing with the disruption of the sensorimotor loop. This review discusses the structural and functional adaptive reorganization of the brain after SCI, and the maladaptive mechanisms that impact on the processing of body related information, which alter motor imagery strategies and EEG signals. Studies that show how residual functions (e.g. face tactile sensitivity) may help people to restore a normal body image are also reviewed. Finally, data on how brain and residual body signals may be used to improve brain computer interface systems is discussed in relation to the issue of how such systems may help SCI people to re-enter the world and interact with objects and other individuals.

Abstract/Results: ABSTRACT:
Hypnosis is well documented in the literature in the management of acute and chronic pain. Virtual reality (VR) is currently gaining credibility in the same fields as hypnosis for medical applications. Lately, the combination of hypnosis and VR was considered. The aim of this scoping review is to understand the current studied contexts and effects of virtual reality hypnosis (VRH) for the management of pain. We searched on PubMed, Taylor & Francis Online, and ProQuest databases with the following terms: “virtual reality,” “3D,” “hypnosis,” and “pain”. We included 8 studies that combined hypnosis and VR. All articles are in English. Two included healthy volunteers and six are clinical studies. Short-term results indicated significant decreases in pain intensity, pain unpleasantness, time spent thinking about pain, anxiety, and levels of opioids. However, results are not consistent for all patients all the days. VR alone seems to reduce pain independently of the hypnotizability level. One study claimed that VR and hypnosis could alter each other’s effects and another argued that VR did not inhibit the hypnotic process and may even facilitate it by employing visual imagery. We cannot affirm that VR added value to hypnosis when they are combined. These trials and case series gave us indications about the possible applications of VRH in different contexts. Additional randomized clinical trials on VRH in the future will have to test this technique in clinical practice and help define guidelines for VRH utilization in pain management.

In 1994 the BIAL Foundation launched a programme of science research grants with the aim of encouraging the research into Man’s physical and mental processes, namely in fields still largely unexplored but which warrant further scientific analysis, as Psychophysiology and Parapsychology.
Since its launch, applications to the BIAL grants have been increasing. Up to now 461 projects have been supported, involving more than 1000 researchers from 27 countries.
The approved applications have benefited from grants in amounts comprised between €5,000 and €50, 000. The amount to be granted is fixed by the Scientific board according to the needs of each project.

Maladaptive brain plasticity has been reported in chronic pain (CP) conditions, though it remains unclear if there are common alterations across pathologies. Therefore, we systematically synthesized literature comparing resting-state functional magnetic resonance imaging (rs-fMRI) in CP patients and healthy controls (HC), and meta-analyzed data whenever applicable. Separate meta-analyses were performed for each method - (fractional) amplitude of low-frequency fluctuations (fALFF, ALFF), regional homogeneity (ReHo), seed-based connectivity (according to the seed) and independent component analysis (according to the network). In qualitative synthesis, sensory-discriminative pain processing - thalamus, insula, temporal and sensory cortices - and cognitive and emotional processing - cingulate, prefrontal and parietal cortices and precuneus - regions concentrated CP/HC differences. Meta-analyses revealed decreased ALFF and increased ReHo in the precuneus, increased fALFF in the left posterior insula and disrupted within- and cross-network connectivity of default mode network (DMN) nodes, as well as altered connectivity in top-down pain modulation pathways. Specifically, it showed decreased anterior and increased posterior components' representation within DMN, enhanced connectivity between the medial prefrontal cortex (mPFC, part of the DMN) and anterior insula (part of the salience network), and decreased mPFC connectivity with the periaqueductal gray matter (PAG). Collectively, results suggest that CP disrupts the natural functional organization of the brain, particularly impacting DMN nodes (mPFC and precuneus), insula and top-town pain modulation circuits.
