The contingent negative variation (CNV) is a long-latency electroencephalography (EEG) surface negative potential with cognitive and motor components, observed during response anticipation. CNV is an index of cortical arousal during orienting and attention, yet its functional neuroanatomical basis is poorly understood. We used functional magnetic resonance imaging (fMRI) with simultaneous EEG and recording of galvanic skin response (GSR) to investigate CNV-related central neural activity and its relationship to peripheral autonomic arousal. In a group analysis, blood oxygenation level dependent (BOLD) activity during the period of CNV generation was enhanced in thalamus, somatomotor cortex, bilateral midcingulate, supplementary motor, and insular cortices. Enhancement of CNV-related activity in anterior and midcingulate, SMA, and insular cortices was associated with decreases in peripheral sympathetic arousal. In a subset of subjects in whom we acquired simultaneous EEG and fMRI data, we observed activity in bilateral thalamus, anterior cingulate, and supplementary motor cortex that was modulated by trial-by-trial amplitude of CNV. These findings provide a likely functional neuroanatomical substrate for the CNV and demonstrate modulation of components of this neural circuitry by peripheral autonomic arousal. Moreover, these data suggest a mechanistic model whereby thalamocortical interactions regulate CNV amplitude.

Negative amplitude shifts of cortical potential are related to seizure activity in epilepsy. Regulation of the cortical potential with biofeedback has been successfully used to reduce the frequency of some patients’ seizures. Although such behavioural treatments are increasingly popular as an alternative to pharmacotherapy, there has been no investigation of the mechanisms that might bridge the behavioural index of peripheral autonomic activity and the central regulation of arousal. Galvanic Skin Response (GSR) is a sensitive measurement of autonomic arousal and physiological state which reflects one’s behaviour. Thus we investigated the effect of peripheral autonomic modulation on cortical arousal with the future intention of using GSR biofeedback as a therapeutic treatment for epilepsy. The cortical negative potential was induced using the paradigm called Contingent Negative Variation (CNV) and measured in different physiological states. A high skin resistance state (reflecting a state of relaxation) and a low skin resistance state (reflecting a state of arousal), were engendered by two opposing procedures of GSR biofeedback. The CNV negative potential, acting as an index of cortical excitation, was significantly greater in amplitude at high levels of skin resistance (relaxed state) than at low levels of skin resistance (aroused state). Our results suggest an inverse relationship between a peripheral measure of autonomic arousal and an index of cortical arousal, the CNV. Moreover, we demonstrate modulation of this arousal-related potential by a behavioural intervention, indicating a potential therapeutic use of arousal biofeedback using GSR in the management of treatment-resistant epilepsy.

OBJECTIVES:
To examine the immediate and sustained effects of volitional sympathetic modulation, using galvanic skin response (GSR) biofeedback training on cortical excitability in patients with drug-resistant epilepsy.
METHODS:
Ten patients undertook 12 sessions of GSR biofeedback training over 1 month, during which they were trained to increase sympathetic arousal, using GSR biofeedback. Contingent negative variation (CNV) (a slow cortical potential reflecting cortical arousal and excitability) and the related post imperative negative variation (PINV) were quantified before and after biofeedback treatment.
RESULTS:
A significant reduction in CNV amplitude was observed in both the short-term (within the first session, after 10 minutes of GSR biofeedback) and long-term (sustained after 12 training sessions). Specifically, the change in baseline CNV amplitude after the 12 training sessions correlated with a percentage reduction in seizure frequency. Furthermore, changes in baseline amplitude of the PINV also correlated with seizure reduction.
CONCLUSIONS:
Our findings demonstrate that behavioral enhancement of peripheral sympathetic tone (GSR) is associated with modulation of indices of cortical excitability. Moreover, GSR biofeedback training over repeated sessions was associated with a chronic baseline reduction in slow cortical potentials and concurrent therapeutic improvement.

Abstract/Results: ABSTRACT:
Contingent negative variation (CNV) topography, hemispheric asymmetry and time-course were investigated in healthy subjects and non-medicated paranoid schizophrenic patients in two antisaccade paradigms with the short (800-1000 ms) and long (1200-1400 ms) durations of the fixation period. EEG and electrooculogram (EOG) were recorded. Saccade characteristics and mean amplitudes of slow cortical potentials time-locked to peripheral target were analyzed in 23 healthy volunteers and 19 schizophrenic patients. Compared to healthy control subjects, schizophrenic patients had significantly slower antisaccades and committed significantly more erroneous saccades in the both antisaccade tasks. The prolongation of the fixation period resulted in noticeable decrease of error percent in patients group. The analysis of CNV time-course has revealed two distinct stages in both groups. The early CNV stage was represented by a negative wave with the maximal amplitude over midline fronto-central area, and the late stage was characterized by increased CNV amplitude at the midline and left parietal electrode sites. In healthy subjects the simultaneous activation of frontal and parietal areas was observed in the paradigm with the shorter fixation interval; the increase of the fixation period produced consecutive activation of these areas. Schizophrenic patients' CNV amplitude was generally smaller than that of healthy subjects. The most pronounced between-group differences of the negative shift amplitude were revealed at frontal electrode sites during the early CNV stage in both modifications of the antisaccade task. The deficit of frontal activation revealed in patients at the early stage of antisaccade preparatory set in both antisaccadic paradigms may be related to pathogenesis of paranoid schizophrenia.

Abstract/Results: ABSTRACT:
The hypothesis under investigation in this study in that early onset alcohol abuse is a factor underlying impulsive violence particularly amongst personality disordered offenders. The study has recorded forensic and substance abuse histories, psychometric information, Contingent Negative Variation (CNV) and performance on the Iowa Gambling Task (IGT) in Personality Disordered patients in Broadmoor. Most prevalent diagnostic category is Anti-social personality disorder. CNV was first described by Grey Walter in 1964. Howard et al (1982), studying special hospital patients reported that, using a symmetrical avoidance paradigm, CNV differentiation was inversely related to psychometric Impulsivity. Previous study indicates that early onset alcohol and drug use is highly prevalent amongst high secure psychiatric patients in the England and Wales jurisdiction (Lumsden et al 2004). This potential risk factor may lead to reduced frontal lobe function. Multi-channel electrophysiological recording of the CNV were collected as a measure of frontal lobe (possibly anterior cingulate) function (Stephan et al 1995). Results are presented from the study relating electrophysiological function (CNV), neuropsychological function (IGT), psychometric/personality profiles (UPPS and IPDE) with behavioural measures (offending and substance use histories and ongoing behaviour in the hospital). Age of onset of alcohol abuse in combination with personality variables is a factor relating to offending and IGT performance.

The time course of temporal discrimination: An ERP study

Abstract/Results: ABSTRACT:
OBJECTIVE - The question of how temporal information is processed by the brain is still a matter of debate. This study aimed to elucidate the brain electrical activity associated with a visual temporal discrimination task.
METHODS - For this purpose, 44 participants were required to compare pairs of sequentially presented time intervals: a fixed standard interval (1000ms), and an equal-to-standard, longer (1200ms) or shorter (800ms) comparison interval. Behavioural data and event-related potentials (ERPs) were analyzed.
RESULTS - Long intervals were more rapidly identified than short intervals. The amplitude of the contingent negative variation (CNV) found at frontocentral sites before the end of the comparison interval was significantly affected by the difference between its duration and the standard one. The amplitude and the scalp distribution of ERPs registered after the offset of the comparison interval were linearly modulated by its absolute duration.
CONCLUSIONS - ERP components associated with the offset of the comparison intervals clarified the involvement of working memory processes and different brain structures in temporal discrimination.
SIGNIFICANCE - This study further improves our understanding of the cognitive processes and neural substrates underlying temporal discrimination in healthy subjects and lays the ground for the investigation of clinical samples with time processing deficits.

Among the BIAL Foundation fellows is worth highlighting the presence of scientists from prestigious universities from the United States, United Kingdom, Australia, Russia, Germany, Japan, France, Canada, and many others.
The BIAL grants are promoted biannually.

BACKGROUND
The processing of emotions from facial expression is an automatic process every day. However, how different emotional expressions affect our performance and social behaviour remains poorly understood.
AIM
The aim of the study is to investigate how the process of different emotional expressions modulate the neural activity supporting attention dependent behavioural responses. Specifically, by defining the mechanisms through which emotional processes interfere with basic cognitive and behavioural functions, we aim to provide a novel insight to the brain basis for Social anxiety.
METHOD
In the current study, we examined the impact of emotional face stimuli (presented in the interval of the forewarned reaction time task) on the Contingent Negative Variation (CNV) and related brain activity. CNV refers to anticipation related slow cortical potential that is a signature of attention related cortical excitation. 15 healthy participants and 5 patients with social anxiety disorder took part in a combined neuroimaging (fMRI) simultaneous EEG study.
RESULTS
Contrary to our prediction, CNV amplitude increased significantly with presentation of both face and non-face stimuli during the delay period of the CNV task. Furthermore, there was a significant modulation of CNV amplitude by emotion type: CNV amplitude was greater for happy and angry compared to fear and disgust face stimuli. Correspondingly, neural activity within thalamus, supplemental motor area (SMA), dorsal anterior cingulate and orbitofrontal cortices was greater for happy and anger compared to fear and disgust CNV conditions. Activity within pons was attenuated for happy and anger conditions relative to fear and disgust. Negative face expressions (fear, disgust and anger) activated the amygdala providing a route for expression of social anxiety on preparatory attention.
CONCLUSION
Specific emotional expressions differentially affect CNV amplitude: specifically a difference is noted between the effects of approach emotions (anger, happiness) compared to withdrawal emotions (fear, disgust).

Antisaccade task performance and mean amplitudes of slow cortical potentials (contingent negative variation—CNV) were investigated in 19 healthy volunteers, 16 schizophrenic patients (SP), and 12 patients with stereotyped form of paraphilia (PP). Compared with healthy subjects, schizophrenic and paraphilic patients committed significantly more erroneous saccades. The clear between-group CNV differences were observed during the early CNV stage that is associated with cognitive aspects of preparatory set. In SP, as compared to controls, the significant decline of CNV amplitude was found at frontal-central area. PP have demonstrated the lack of CNV over central and parietal regions, but their CNV amplitudes in frontal area did not differ from values of control group. Thus, two distinct types of CNV abnormalities have been found. The SP results have been interpreted as support for frontal dysfunction in schizophrenia. The disconnection between prefrontal cortex, sensorimotor cortex, and related subcortical structures is hypothesized in paraphilia group.
