Abstract/Results: ABSTRACT:
Psychophysiological insomnia (PI) is one of the most frequent sleep disorders. In this study we tested whether differences in terms of neural activation are present between a group of PI patients and a healthy-control group while they are exposed to idiosyncratic ruminations and worries, evoked visually by words, so as to explore their hypothetical link with default-mode network (DMN) dysfunction in PI. We recruited five PI patients diagnosed according to the International Classification of Sleep Disorders, version 2 (ICSD-2) of American Academy of Sleep Medicine (AASM) and five age- and sex-matched healthy controls. Patients were recruited at the outpatient Sleep Medicine Centre of the Coimbra University Hospital Centre. We used a functional magnetic resonance imaging (fMRI) block-design paradigm where the participants visualized lists of words related to past/present and future concerns and also emotionally neutral words. The results suggested that the PI patients showed a failure of the DMN to deactivate. Moreover, when these patients were exposed to words concerning both past/present ruminations and future worries, there was a pronounced and significant over-recruitment of brain areas related to DMN and self-referential processing when they were compared to healthy volunteers. The differences between the patient and control groups were also evident in self-report measures. In sum, despite the relatively small sample size, our study clearly suggests that in PI there is a dysfunction in brain regions pertaining to self-referential processing, which is corroborated by an overall pattern of hyperarousal in brain regions comprising the DMN. These data may be useful in the improvement of pathophysiological models, diagnostic and therapeutic interventions for insomnia.

Psychophysiological insomnia (PI) is a clinical condition characterized by sleep-related disturbing cognitive activity and biased self-related information processing. This hypothetical cognitive arousal has been hypothesized to be associated with overactivation within different brain areas and networks, especially when individuals are at rest, e.g., in the absence of any attention-demanding task. In this study, we carried out a resting-state fMRI experiment aimed at investigating activity of the different resting-state networks in PI. Our pool of participants was compound of 5 PI patients and 5 sex- and age-matched healthy controls recruited from the community. Participants from both groups also completed a set of self-report measures, including the sleep diary, Insomnia Severity Index (ISI), dysfunctional beliefs and attitudes about sleep (DBAS-30), and the World Health Organization Quality of Life Measure (WHOQOL-Bref). Our results showed that insomnia patients presented altered activation in the default-mode network (DMN), visual and auditory networks, and bilateral fronto-parietal networks. In the DMN, the patients presented a pattern of both decreased (right superior frontal gyrus, left medial frontal gyrus, and right middle temporal gyrus) and increased activation (left superior frontal gyrus, left anterior and posterior cingulate, right precuneus, left cingulate gyrus, and left middle temporal gyrus). Our findings on unbalanced resting-state networks in PI, with special emphasis on the DMN, may lay grounds to better understanding of the cognitive arousal experienced by PI patients and might help to further improve the clinical management of insomnia.

Action-related dynamic changes in inferior frontal cortex effective connectivity: A TMS/EEG coregistration study

Humans show exquisite abilities to perform versatile finger movements. The inferior frontal cortex (IFC) plays a pivotal role in the visual control of such movements through connections with other sensorimotor regions. Yet, the dynamics of IFC effective connectivity during action execution are still poorly understood. Using single-pulse TMS and simultaneous EEG recording (i.e., TMS-EEG coregistration), we stimulated the left posterior IFC at rest and during a visuomotor task. We recorded TMS-evoked potentials (TEPs) to assess action-related changes in IFC connectivity and localized their sources using sLORETA. We found two key time windows at ~60 and ~80 msec after IFC stimulation in which TEPs were modulated by task conditions in remote electrodes. In the first time window (~60 msec), action-related changes in TEP amplitudes were observed over frontal and temporo-parietal electrodes, reflecting increased IFC connectivity with fronto-parietal motor areas and decreased IFC connectivity with visual occipito-temporal areas. In the second time window (~80 msec), action-related TEP increases were observed in frontal, temporal and parietal regions partially overlapping with the default-mode network. No similar effects were observed when TMS was administered over a non-motor control area (the left posterior superior temporal sulcus, STS). These findings highlight dynamic changes in IFC connectivity with motor, sensory and default-mode networks. They suggest sequential stages of task-related changes in IFC connectivity possibly related to controlling and sensing actions and inhibiting default-mode brain activity during motor performance.

PURPOSE
In this study, our aim was to examine neural correlates of cognitive–behavioral therapy for insomnia (CBT-I).
METHODS
It was investigated to what extent successfully tailored CBT-I modified the neural activity pattern observed before the intervention, when two insomnia patients were exposed to idiosyncratic stimuli pertaining to past/present and future concerns in an fMRI experiment.
RESULTS
Our findings suggested that successful CBT-I seems to normalize brain overactivity. This finding is enhanced by the visible attenuation of activity in brain regions implicated in self-referential processing and by the significant activation within visual areas across all experimental conditions.
CONCLUSION
It is likely that psychological treatment changes the brain’s function in insomnia. However, studies with larger sample sizes are needed to establish solid evidence.

Purpose
Insomnia is a highly prevalent disorder with important implications for health and well-being. It is well known the “hyperarousal hypothesis” in insomnia which assumes that patients present high levels of activation, in particular, cortical arousal.
Methods
A preliminary resting-state fMRI experiment comprising two non-medicated insomnia patients was performed to evaluate whether cognitive-behavioral therapy for insomnia (CBT-I) changes the activation patterns of the most known resting-state brain networks, particularly, the default-mode network (DMN).
Results
As main results, we found modifications in the anterior component of the DMN, in the visual network, and in the auditory network after successful CBT-I. The most notable result was a decreased activity in the medial prefrontal cortex from pre- to post-intervention.
Conclusion
Findings from this preliminary study suggest that psychological treatment may translate modifications in neural activity of insomnia patients.

Obsessive-compulsive personality (OCPD) disorder is characterized by a pattern of excessive self-control, perfectionism and behavioral and cognitive rigidity. Despite the fact that OCPD is the most common personality disorder in the general population, published studies looking at the brain correlates of this disorder are practically nonexistent. The main goal of this study was to analyze the presence of brain alterations in OCPD when compared to healthy controls, specifically at the level of the Default Mode Network (DMN). The DMN is a well-established resting state network which was found to be associated with psychological processes that may play a key role in OCPD (e.g., self-awareness, episodic future thinking and mental simulation). Ten individuals diagnosed with OCPD and ten healthy controls underwent a clinical assessment interview and a resting-state functional magnetic resonance imaging (fMRI) acquisition. The results show that OCPD patients presented an increased functional connectivity in the precuneus (i.e., a posterior node of the DMN), known to be involved in the retrieval manipulation of past events in order to solve current problems and develop plans for the future. These results suggest that this key node of the DMN may play an important role in the pathophysiology of OCPD. (C) 2016 Elsevier Ireland Ltd. All rights reserved.

Insomnia disorder (ID) is a prevalent sleep disorder that significantly compromises the physical and mental health of individuals. This article reviews novel approaches in the study of brain networks and impaired function in ID through the application of modern neuroimaging techniques such as functional magnetic resonance imaging (fMRI).
The default-mode network (DMN) is presumed to be correlated with self-referential information processing, and it appears to be altered or unbalanced in insomnia.
A growing body of evidence suggests the lack of deactivation of brain regions comprising the DMN when insomnia patients are at rest. Moreover, core areas of the DMN demonstrate greater activation in insomnia patients when compared to healthy controls in self-referential related tasks. Despite the few studies on the topic, underpinning the correlation between abnormal DMN activity and ID deserves further attention in the future. Implications for therapeutics are briefly outlined.
