The BIAL Foundation also assigns Scientific Research Scholarships for the study of neurophysiological and mental health in people, arousing the interest of researchers in the areas of Psychophysiology and Parapsychology.
To date the BIAL Foundation has supported 461 projects, more than 1000 researchers, with research groups in twenty-seven countries, resulting, until April 2013, in about 600 full papers, out of which 172 published in indexed international journals with an average impact factor of 3.6 and a substantial number of citations (1665).
Since 1996 the BIAL Foundation organizes the Symposia entitled "Behind and Beyond the Brain", a Forum that gathers well renowned neurosciences speakers and the BIAL Foundation Fellows which are spread around the world.
Classified as an institution of public utility, the BIAL Foundation includes among its patrons the Portuguese President, the Portuguese Universities Rectors' Council and the Portuguese Medical Association.

Abstract/Results: RESULTS:
Behavioural interventions including biofeedback represent an alternative or adjunctive therapeutic axis in the management of drug refractory epilepsy. Our previous study suggested that the modulation of the peripheral state of arousal as indexed by the Galvanic Skin Response (GSR) had considerable influence on cortical excitability, indicating the possible use of biofeedback in controlling seizure activity. In the present study we investigated the effect of the GSR biofeedback training on seizure frequency in patients with treatment resistant epilepsy. Eighteen patients with drug refractory epilepsy were randomly assigned either to an active GSR biofeedback group (n = 10) or to a sham control biofeedback group (n = 8). Patients received a total of 12 sessions over four weeks, representing either real GSR biofeedback training, where subjects were trained to control their GSR by receiving feedback of their own GSR change on the computer screen, or sham training, where the feedback was unrelated to the subjects’ GSR.
Biofeedback training significantly reduced patients’ seizure frequency in the active biofeedback group (p = 0.005), but not the control group (p>0.10). This was manifest as a significant between group differences in seizure reduction (p = 0.016). Furthermore there was a correlation between degree of patients’ improvement in biofeedback performance and reduction of seizure frequency (rho = 0.736, p = 0.001), confirming that the effect of biofeedback treatment was related to physiological change. Our findings demonstrate that a behavioural intervention, GSR biofeedback training, may be associated with a significant reduction in seizure frequency in drug refractory epilepsy, highlighting the potential therapeutic value of this method.

OBJECTIVES:
To examine the immediate and sustained effects of volitional sympathetic modulation, using galvanic skin response (GSR) biofeedback training on cortical excitability in patients with drug-resistant epilepsy.
METHODS:
Ten patients undertook 12 sessions of GSR biofeedback training over 1 month, during which they were trained to increase sympathetic arousal, using GSR biofeedback. Contingent negative variation (CNV) (a slow cortical potential reflecting cortical arousal and excitability) and the related post imperative negative variation (PINV) were quantified before and after biofeedback treatment.
RESULTS:
A significant reduction in CNV amplitude was observed in both the short-term (within the first session, after 10 minutes of GSR biofeedback) and long-term (sustained after 12 training sessions). Specifically, the change in baseline CNV amplitude after the 12 training sessions correlated with a percentage reduction in seizure frequency. Furthermore, changes in baseline amplitude of the PINV also correlated with seizure reduction.
CONCLUSIONS:
Our findings demonstrate that behavioral enhancement of peripheral sympathetic tone (GSR) is associated with modulation of indices of cortical excitability. Abstract/Results: ABSTRACT:
Behavioural interventions including biofeedback represent an alternative or adjunctive therapeutic axis in the management of drug refractory epilepsy. Our previous study suggested that the modulation of the peripheral state of arousal as indexed by the Galvanic Skin Response (GSR) had considerable influence on cortical excitability, indicating the possible use of biofeedback in controlling seizure activity.
We investigated the effect of the GSR biofeedback training on seizure frequency in patients with treatment resistant epilepsy. Eighteen patients with drug refractory epilepsy were randomly assigned either to an active GSR biofeedback group (n = 10) or to a sham control biofeedback group (n = 8). Patients received a total of 12 sessions over four weeks, representing either real GSR biofeedback training, where subjects were trained to decrease skin resistance using biofeedback, or sham training, where the feedback was unrelated to the subjects’ GSR. Biofeedback training significantly reduced patients’ seizure frequency in the active biofeedback group (p = 0.017), but not the control group (p>0.10). This was manifest as a significant between group difference in seizure reduction (p = 0.01). Our findings demonstrate that a behavioural intervention, GSR biofeedback training, may substantially reduce seizure frequency in drug refractory epilepsy, highlighting the potential therapeutic value of this method.

BACKGROUND:
Operant conditioning of one's slow cortical potential (SCP) or sensorimotor rhythm (SMR) can be used to control epilepsy or to manipulate external devices, as applied in BCI (Brain-Computer Interface). A commonly accepted view that both SCP and SMR are reflections of central arousal suggests a functional relationship between SCP and SMR networks.
METHOD:
The operant conditioning of SCP or SMR was tested with a single electroencephalographic (EEG) channel wireless biofeedback system. A series of trainings taught 19 participants to control SCP or SMR over vertex during 20 neurofeedback sessions. Each session consisted of 96 trials to decrease cortical arousal (SCP positivity/SMR enhancement) and 64 trials to increase cortical arousal (SCP negativity/SMR suppression). In each trial, participants were required to exceed an individual threshold level of the feedback parameter relative to a 500-msec prefeedback baseline and to hold this level for 2 sec (SCP) or 0.5 sec (SMR) to obtain reinforcement.
RESULTS:
Ten of the 19 participants achieved control over their EEG. In the SCP-trained group, 4 of 9 participants increased the differentiation between their SCP responses on positivity-required versus negativity-required trials. SMR suppression and enhancement was achieved by 3 and 4 of the 10 SMR-trained participants. The SMR-trained responders did not show differentiation in their SMR responses, but did show a differentiation in their SCP response—while trained on SMR.
CONCLUSIONS:
The results showed the proposed method was successful to teach control of SCP or SMR. Bidirectional control was very difficult to achieve with the present SMR training procedure. SCP positivity and SMR enhancement were easier to learn. The results suggest that SMR training modulates excitability thresholds in the striatal-thalamocortical motor loop, whereas changes in the loop's excitability thresholds by SCP training do not affect the thalamic bursting that underlies the SMR.

BACKGROUND:
Operant conditioning of one’s slow cortical potential (SCP) or sensorimotor rhythm (SMR) can be used to control epilepsy or to manipulate external devices, as applied in BCI (Brain-Computer Interface). To be practical, a BCI-system should use as less channels as possible. For this purpose, a wireless biofeedback system was developed that allows feedback of a single EEG-channel in discrete trials. The commonly accepted view that both the SCP and SMR are a reflection of central arousal suggests a functional
relationship between SCP and SMR networks.
METHODS:
A training was performed that aimed to teach 19 participants to control their SCP (n=9) or SMR (n=10) over vertex. Participants received 20 neurofeedback sessions, each comprising of 96 trials in which they had to decrease cortical arousal (SCP positivity/SMR enhancement) and 64 trials in which they had to increase cortical arousal (SCP negativity/SMR suppression). In a trial, participants were required to exceed an individual threshold level of the feedback parameter relative to a 500 ms pre-feedback baseline and hold this level for 2 seconds (SCP) or 0.5 seconds (SMR) in order to obtain reinforcement.
RESULTS:
Overall, 10 of the total of 19 participants achieved control over their EEG. In the SCP-trained group, 4 out of 9 participants were able to increase the differentiation between their SCP responses on positivityrequired vs. negativity-required trials over the course of the experiment. Improvements in control over the SMR in suppression-required and enhancement-required trials were acquired by respectively 3 and 4 of the 10 SMR-trained participants. These SMR-trained responders did not show differentiation between their
SMR responses in enhancement-required vs. suppression-required trials. Interestingly, the SMR responders did show a differentiation in their SCP response while trained on SMR.
CONCLUSIONS:
It can be concluded from this experiment that, with the proposed method, a number of the participants are able to acquire control over their SCP or SMR. For SMR, however, bidirectional control is very difficult to achieve with the present training procedure. Furthermore, SCP positivity and SMR enhancement are easier to learn compared to their counterparts. The observed SCP differentiation while training SMR and absence of equivalent SMR changes while training SCP suggest that SMR training modulates the central arousal system, whereas SCP training invokes local effects.

Abstract/Results: RESULTS:
Using the opportunity to record parahippocampal activity with foramen ovale electrodes in epilepsy patients we examined high-frequency activity and its relation to NREM sleep-slow oscillations and sleep spindles. Parahippocampal high-frequency activity was organized into bursts which were consistently associated with interictal epileptic spikes. Ripple density was higher during Non-REM than REM sleep (p<0.001). Ripple activity distinctly decreased time-locked to slow oscillation negative half-waves in the three patients without temporal structural alterations (p<0.001), whereas in the four patients with severe mesiotemporal structural alterations this coupling was obscure. Generally, in the patients ripple activity was increased before spindle peaks and distinctly decreased after the peak (p<0.001). Coordinated occurrence of hippocampal ripples, sleep spindles and slow oscillations have already been reported in animals yet the present results provide first evidence for such a temporal coupling in humans. Our findings are consistent with the notion of a hippocampo-to-neocortical information transfer during sleep that is linked to coordinate ripple and spindle activity, and that in the intact temporal lobe is synchronized to cortical slow oscillations.

Ripples are high-frequency oscillation bursts in the mammalian hippocampus mainly present during Non-REM sleep. In rodents they occur in association with sharp waves and are grouped by the cortical slow oscillation such that, in parallel with sleep spindles, ripple activity is suppressed during the hyperpolarized down-state and enhanced during the depolarized up-state. The temporal coupling between slow oscillations, spindles and ripples has been suggested to serve a hippocampo-neocortical dialogue underlying memory consolidation during sleep. Here, we examined whether a similar coupling exists between these oscillatory phenomena in humans. In sleep recordings from seven epileptic patients, scalp-recorded slow oscillations and spindles as well as parahippocampal ripples recorded from foramen ovale electrodes were identified by automatic algorithms. Additionally, ripple and spindle root mean square activity was determined for relevant frequency bands. Ripple density was higher during Non-REM than REM sleep (P < 0.001). Ripple activity distinctly decreased time-locked to slow oscillation negative half-waves in the three patients without temporal structural alterations (P < 0.001), whereas in the four patients with severe mesiotemporal structural alterations this coupling was obscure. Generally, in the patients ripple activity was increased before spindle peaks and distinctly decreased after the peak (P < 0.001). Ripples were consistently associated with interictal spikes suggesting that spike-ripple complexes represent an epileptic transformation of sharp wave-ripple complexes in the epileptic hippocampus. Our findings are consistent with the notion of a hippocampo-to-neocortical information transfer during sleep that is linked to coordinate ripple and spindle activity, and that in the intact temporal lobe is synchronized to cortical slow oscillations.

In 1994 the BIAL Foundation launched a programme of science research grants with the aim of encouraging the research into Man’s physical and mental processes, namely in fields still largely unexplored but which warrant further scientific analysis, as Psychophysiology and Parapsychology.
Since its launch, applications to the BIAL grants have been increasing. Up to now 461 projects have been supported, involving more than 1000 researchers from 27 countries.
The approved applications have benefited from grants in amounts comprised between €5,000 and €50, 000. The amount to be granted is fixed by the Scientific board according to the needs of each project.
The supported projects have originated, until April 2013, in about 600 full papers, 172 out of which were published in indexed international journals with an average impact factor of 3.6 and a substantial number of citations (1665).
Among the BIAL Foundation fellows is worth highlighting the presence of scientists from prestigious universities from the United States, United Kingdom, Australia, Russia, Germany, Japan, France, Canada, and many others.
The BIAL grants are promoted biannually.

Increased parahippocampal delta activity characterises virtual navigation and REM sleep in humans

Abstract/Results: ABSTRACT:
Hippocampal theta or rhythmic slow activity (RSA) occurring during exploratory behaviours and rapid-eye-movement (REM) sleep is a characteristic and salient oscillatory rhythm in animals. By contrast, much controversy exists regarding the existence and the characteristics of this activity in humans. Some studies argued that human hippocampal theta activity appears in short and phasic bursts. On the contrary, earlier we showed that REM-dependent RSA recorded from the parahippocampal region is continuous like in animals but instead of the theta it falls in the delta frequency range. Here, we examined parahippocampal activity in 10 epilepsy patients implanted with foramen ovale electrodes while they were engaged in a virtual navigation task. Navigation was tested according to three conditions: an acquisition, a recall and a non-learning route-following condition. Recordings were also sampled from resting and REM sleep. For analyses, spectral power densities (SPD) were calculated for each 1 Hz wide frequency bin up to 10 Hz. During acquisition, SPD significantly increased in the 1 Hz, 2 Hz and the 3 Hz frequency bins when compared with resting. A similar but more robust increase was present in the 1 Hz, 2 Hz, 3 Hz, 4 Hz and 5 Hz frequency bins during REM sleep. Thus, increase was present in similar frequencies during the two conditions and these frequency ranges were below the traditional theta band. At the same time, other features of this activity, such as state-dependency and tonicity made this activity comparable to that seen in animals and suggest this activity to be the human analogue hippocampal theta in animals.

Ripples are high-frequency oscillation bursts in the mammalian hippocampus mainly present during NREM sleep. In rodents they occur in association with sharp waves and are known to be grouped by the cortical slow oscillation such that their occurrence is maximal during the surface positive, up states. Ripples were also shown to be temporally linked to sleep spindles. Importantly, coordinated spindleripple events have been suggested to provide a framework for information transfer between the hippocampus and the neocortex underlying memory consolidation. Our aim was to examine whether ripples recorded via parahippocampal electrocorticography are also temporally linked with slow oscillations and sleep spindles in epileptic
humans. Whole-night recordings from seven epilepsy patients implanted with foramen ovale (FO) electrodes were analysed. Negative half waves of the slow oscillation and sleep spindles recorded on the scalp as well as ripples in FO recordings were detected by an automatic algorithm. Ripple activity was also measured as root mean square (RMS) of the 80–140 Hz activity. Compared to baseline both ripple measures showed a significant increase time-locked to sleep spindles. In the three patients without temporal structural alterations ripple activity decreased time-locked to slow oscillation negative half waves. In the four other patients exhibiting hippocampal sclerosis, modulation of ripple activity by the negative half waves was much less clear. Ripples significantly increased during NREM sleep and were consistently associated with interictal epileptic spikes. This suggests that ripple/spike complexes represent an epileptic exaggeration of ripple/sharp wave complexes in the human epileptic hippocampus. Whether ripples are epileptically transformed or not, the present data provide first evidence for a temporal coupling between hippocampal ripples and neocortical spindles in humans.

The analysis of synchronization, particularly phase locking, is being increasingly used in neuroscience to explore coordinated brain activity. The application of this methodology to magnetoencephalographic (MEG) and electroencephalographic (EEG) recordings would seem promising because these two recording techniques are not invasive and have great temporal resolution. However, current methods of synchronization analysis applied directly to raw MEG/EEG data may produce distorted results. In this work we introduce a model of brain activity based on random current dipoles that reproduces the main characteristics observed in measurements of real data synchronization, even when no synchronized activity is taking place among the sources. In particular we describe the effect of field superposition alone when secondary currents are excluded. It is also shown that the enhanced local synchronization, previously described in some studies of epileptic seizures, may result from the activity of only a few unsynchronized sources. We suggest three lines of research for further development and improvement to obtain more precise physiological interpretations of the synchronization analysis.

Fluctuations in phase synchrony have been characteristically found in recordings from both normal physiological brains as well as pathological ones. These fluctuations, characteristically termed as noise, are
logistically difficult to separate from true neuronal activity largely due to current recording and analysis methods. We propose that phase synchrony fluctuations serve to maintain brain activity in an optimal state for cognitive processing, thereby not allowing it to fall into long-term periodic behavior. We compared fluctuations from controls to those found in patients with epilepsy or traumatic brain injuries (TBI). Phase synchronization analysis using the Hilbert Transform was performed on magnetoencephalography (MEG) and electroencephalography (EEG) recordings from controls and patients. In the MEG study, 4 epileptic patients and 4 controls were tested. A total of 14 head-injured children and 2 controls were analyzed using scalp EEG, after performing a Laplacian Derivation. Results - Phase-locking patterns between brain areas were found to display fluctuations at different scales. Both epileptiform activity and TBI had relatively low levels of fluctuations in phase synchrony. This may contribute to the generation of synchronized states seen in patients with these brain disorders. Conclusions and Implications. Determination of phase-locking patterns and their variability in brain recordings reveals characteristics of the pathologies with which they are associated. While there is no clear distinction between noise and highdimensional dynamics, we propose that the examination of neuronal fluctuations at different levels will provide important insights into our understanding of the relationship between brain and behavior.

Abstract/Results: RESULTS:
In this Project, we have investigated memory mechanisms in the human visual system, by employing a combination of psychophysics and functional magnetic resonance imaging (fMRI) methodologies to identify the neural correlates of different types of learning processes. For this purpose, we implemented and optimized a number of fMRI visual mapping paradigms in order to identify the visual brain areas of interest.
We first observed the adaptive functional reorganization of visual memory in a population of temporal lobe epilepsy patients with right unilateral hippocampal sclerosis. We used fMRI to measure brain activity changes during the episodic encoding of abstract line drawings and found that the patients engaged the left hippocampus more than controls and that the level of engagement was correlated with their memory recognition scores. These finds suggested functional reorganization of visual memory with an adaptive role, as a consequence of prolonged disease. We also found that familiar items activated visual cortical areas more than novel items, suggesting a reinstatement of visual information as a consequence of learning.
In a second study, we implemented a visual learning paradigm whereby subjects performed a face discrimination task on faces presented in multiple orientations, in order to investigate the hypothesis that the human visual system contains specialized processing mechanisms that are more engaged by upright faces than by inverted faces or non-face objects, as a result of extensive practice with upright faces. Our psychophysics experiments showed that participants gave more errors and were slower in their responses as faces were rotated away from 0º. In addition, performance was improved when participants underwent periods of training, both for trained and untrained orientations. Consistently with the expertise hypothesis, we also found a parametric modulation of fMRI activity in specialized visual brain areas, according to the quadratic behavioural effect of face orientation.
