BACKGROUND
Primary headache has a high prevalence in children. Worldwide, 57-82% of children and adolescents suffer from recurrent headaches by the age of 15 years. Tension type headache and migraine are the most important primary headache syndromes in children and adolescents. In the Netherlands half of all children between 4 and 18 years of age suffer from headache once or more per month. This can have significant impact on children’s quality of life and emotional state, often continuing into adulthood. It can result in missed school days, disturbed peer and family relationships and emotional stress. Because the exact pathogenesis of tension type headache and migraine is unknown, several different therapies have been described. Some symptomatic and prophylactic drugs have proven to be effective in children with primary headaches. However these medications are often associated with several unwanted side effects . In recent reviews, inconclusive findings have been reported with respect to the efficacy of relaxation and cognitive behavioural therapy in reducing the severity and frequency of chronic headache in children. Therefore, there is an urgent need for further research to evaluate non-pharmacological treatment options with minimal or no adverse effects. Because stress is an important factor in the pathophysiology of headaches, therapies that are capable of stress reduction may be a promising treatment option for this group of patients.
AIM
The aim of the current study is to determine the effectiveness of two relaxation techniques, e.g. Transcendental Medication or hypnotherapy added to the standard medical treatment of paediatric primary headaches in comparison to relaxation exercises as part of standard medical treatment.
METHODS
This pragmatic, randomized controlled trial with three parallel groups was executed between March 2012 and April 2017 in six hospitals in de Netherlands. All participating children received the usual treatment for headaches and were also randomized to either Transcendental Meditation or hypnotherapy or regular relaxation exercises (as part of the 'standard treatment'). Measurements were taken before the start of the intervention and directly after the intervention (after three months) and a long-term follow up (after nine months). Primary outcome variables were mean frequency of primary headache attacks, as well as the clinically relevant reduction (=50%) in headache frequency. Secondary outcome variables were subjective improvement in symptoms (adequate relief), coping with pain (PCQ), anxiety and depressive symptoms (RCADS-25), somatization (CSI), sense of coherence (SOC-K) and satisfaction with treatment. Furthermore, information was gathered on the implementation of hypnotherapy and meditation in the everyday paediatric clinical practice.
RESULTS
A total of 131 children were randomized into the study, 45 children in the HT group, 42 in the TM group and 44 children in the usual care plus exercise group. The mean age of the total group was 13.3 years and 77% was female. The headache frequency was significantly reduced after 3 months for all groups from 19 days to 13 days at three months and 11 days at nine months with headache per 4 weeks (P < 0.001). However, no significant differences were found between the intervention groups (p = 0,85 three months, p = 0,57 nine months). The percentage of children with clinically relevant reduction in mean frequency of headache attacks was 41% at three months and 47% at nine months and did not significantly differ between the three groups. Besides headache-related effects, other positive experiences were reported by parents in all three intervention groups: children were more relaxed, slept better and were more aware of their own influence on their headache. No adverse events were reported. Concerning feasibility and parental satisfaction with the intervention in a clinical setting the following things were indicated as important: The intervention should fit a child’s personality, the patient-therapist relation is important and the travel distance to a therapist should not be too far.
CONCLUSION
Relaxation techniques effectively reduce primary headache in children and are safe. This study shows that it does not matter whether that applies to hypnotherapy, meditation or other relaxation techniques. It is recommended that, when choosing a specific form of relaxation technique, to choose the form that best suits the child or the form that is best organized in the hospital (availability, travel distance, costs and waiting lists) to achieve the best result.

Many children suffer from headaches. Since stress may trigger headaches, effective techniques to cope with stress are needed. We investigated the effectiveness of two mind-body techniques, transcendental meditation (TM) or hypnotherapy (HT), and compared them with progressive muscle relaxation (PMR) exercises (active control group). Children (9-18 years) suffering from primary headaches more than two times per month received either TM (N?=?42), HT (N?=?45) or PMR (N?=?44) for 3 months. Primary outcomes were frequency of headaches and =?50% reduction in headaches at 3 and 9 months. Secondary outcomes were adequate relief, pain coping, anxiety and depressive symptoms, somatisation and safety of treatment. Groups were comparable at baseline. Headache frequency was significantly reduced in all groups from 18.9 days per month to 12.5 and 10.5 at respectively 3 and 9 months (p?<?0.001), with no significant differences between the groups. Clinically relevant headache reduction (=?50%) was observed in 41% and 47% of children at 3 and 9 months respectively, with no significant differences between the groups. No differences were observed in secondary outcome measures between the intervention groups. No adverse events were reported.Conclusion: All three techniques reduced primary headache in children and appeared to be safe.Trial registration: NTR 2955, 28 June 2011 ( www.trialregister.nl ).
What is Known: • Stress may be an important trigger for both tension type headache and migraine in children. • Good data are lacking on the effect of transcendental meditation, hypnotherapy or progressive muscle relaxation as possible stress-reducing therapies in children with primary headaches.
What is New: • Three non-pharmacological techniques, i.e., transcendental meditation, hypnotherapy and progressive muscle relaxation exercises, all result in a clinically significant reduction of headaches and use of pain medication. • No large differences between the three techniques were found, suggesting that children can choose either one of the three techniques based on personal preferences.

QUESTIONS:
To investigate the effectiveness of hypnotherapy or transcendental meditation (TM) versus regular relaxation exercises added to usual medical treatment of paediatric primary headaches.
METHODS:
A pragmatic, randomized controlled trial was carried out in six hospitals in de Netherlands. Children (age 9-18 years) with primary headache and headache attack frequency of =2 times per month were randomized to receive either hypnotherapy or TM or regular relaxation exercises (control group), in addition to usual medical treatment. Primary outcome were mean frequency of primary headache attacks and clinically relevant (>50%) reduction in headache frequency, after three months of intervention. Secondary outcome was subjective improvement in symptoms (adequate relief).
RESULTS:
112 children were included: hypnotherapy (N?=?39), TM group (N?=?36) and control group (N?=?37). Mean age was 13.3 years and 77% was female. Preliminary analysis were performed and showed that headache frequency was significantly reduced after 3 months for all groups from 18 to 13 days with headache per month (p?=?0.0001). No significant differences were found between the groups (p?=?0.95). The percentage of children with clinically relevant reduction in headache attacks was 38% and did not significantly differ between the three groups. Subjective improvement after intervention (adequate relief) was reported by 30% of children, with no significant difference between groups, although there was a positive trend for hypnotherapy (38,5% versus 25% in TM and control group, p?=?0.34). No adverse events were reported.
CONCLUSIONS:
Final analysis and conclusions with respect to the effects of hypnotherapy or meditation versus regular relaxation techniques on primary headaches in children will be completed before the start of the conference and presented there.

History:
In 1994 the BIAL Foundation launched a programme of science research grants with the aim of encouraging the research into Man’s physical and mental processes, namely in fields still largely unexplored but which warrant further scientific analysis, as Psychophysiology and Parapsychology.
Since its launch, applications to the BIAL grants have been increasing. Up to now 461 projects have been supported, involving more than 1000 researchers from 27 countries.
The approved applications have benefited from grants in amounts comprised between €5,000 and €50, 000. The amount to be granted is fixed by the Scientific board according to the needs of each project.
The supported projects have originated, until April 2013, in about 600 full papers, 172 out of which were published in indexed international journals with an average impact factor of 3.6 and a substantial number of citations (1665).
Among the BIAL Foundation fellows is worth highlighting the presence of scientists from prestigious universities from the United States, United Kingdom, Australia, Russia, Germany, Japan, France, Canada, and many others.
The BIAL grants are promoted biannually.

Title:
Predictors of cognitive dysfunction one-year post COVID-19

Objectives:
(a) To characterize the frequency of objective cognitive deficits and self-perceived cognitive difficulties and (b) to explore demographic and clinical predictors of cognitive dysfunction and cognitive complaints.
Method:
One hundred and ten adults diagnosed with COVID-19 between March and November 2020, aged = 74 years underwent a brief neuropsychological evaluation 12 months after infection, which included: Brief Visuospatial Memory Test-Revised, California Verbal Learning Test, and Symbol Digit Modalities Test. T scores < 38 were considered abnormal performance; cognitive dysfunction was defined as = 2 abnormal tests. Participants also completed Broadbent's Cognitive Failure Questionnaires (CFQ), Hospital Anxiety and Depression Scale, Modified Fatigue Impact Scale, and Short-Form Health Survey. CFQ = 43 was considered indicative of cognitive complaints.
Results:
Twenty participants (18.2%) had cognitive dysfunction and 36 (33.3%) had cognitive complaints. Cognitive dysfunction was related to lower education, preinfection history of headache/migraine, and acute COVID-19 symptoms of headache and sleep disturbance. Cognitive complaints were more likely to occur in women, those with fewer years of education, and acute COVID-19 symptoms of headache and sleep disturbance. Cognitive complaints were also significantly related to symptoms of anxiety, depression, and fatigue. Sex and psychopathology were not significant predictors of cognitive dysfunction. Modest associations were found between CFQ total score and cognitive test performance.
Discussion:
A subset of individuals develops cognitive difficulties in the context of post-COVID syndrome. Results may support the protective effect of education, a known proxy of cognitive reserve. COVID-19 infection symptoms of headache and sleep disturbance appear to be risk factors for long-term cognitive difficulties.
