Mind the psychedelic hype: Characterizing the risks and benefits of psychedelics for depression
Publication year: 2024
URL:
https://doi.org/10.3390/psychoactives3020014
Abstract/Results: ABSTRACT:
Rationale: Psychedelic research re-emerged from a period of suppression into the so-called psychedelic renaissance. In parallel, most media reporting has shifted from the overstatement of the risks of psychedelics to overly positive hype. As the empirical evidence is more equivocal than frequently portrayed, the conclusions about the effectiveness of psychedelics should be considered preliminary. Poor science communication about psychedelics’ therapeutic potential may lead potential participants or patients to feel misled and policy decisions to be misinformed. An evidence-informed characterization of their risks and benefits is needed. Objectives: This article assesses the state of psychedelic research for treating depression and the effect sizes of psychedelics on therapeutic outcomes, the risk of bias, and the prevalence of adverse effects. We review research on the risks and benefits of psychedelics and discuss how the following depression treatments have shown decreasing effect sizes over time: (1) cognitive behavioral therapy, (2) mindfulness interventions, (3) selective serotonin reuptake inhibitors, and (4) ketamine. We speculate that a similar trend may occur for psychedelic treatments. Results and conclusions: It is likely that larger and better-controlled psychedelic trials will demonstrate smaller effect sizes that are more comparable to other conventional and emerging treatments for mood disorders. Clear science communication is critical for setting public expectations and psychedelic policy. With this evidence-based assessment, we aim to cut through the misinformation about the benefits, risks, and future prospects of psychedelic treatments.

Near-death experience during emergency ketamine use: A case report
Publication year: 2025
URL:
https://doi.org/10.1002/brb3.70939
Abstract/Results: ABSTRACT:
Purpose
Survivors from acute life-threatening conditions occasionally report various alterations in subjective experiences, ranging from visual perceptual changes to multi-faceted phenomena that can be categorized as near-death experiences (NDEs). We report an NDE in a 73-year-old woman with an acute exacerbation of chronic obstructive pulmonary disease, requiring emergency intubation and mechanical ventilation for acute respiratory failure.
Method
The patient suffered from profound hypoxia and hypercapnia with uncompensated respiratory acidosis. Prehospital anesthesia was induced with an intravenous bolus of 500 mg of ketamine administered over 60 s, corresponding to 7 mg/kg of adjusted body weight. The patient eventually recovered and was weaned from ventilator after 24 h. Semi-structured interviews, including several standardized scales assessing potential subjective experience and post-event impact, were conducted at 2 weeks and 2 months post-event.
Finding
Findings revealed a vivid NDE (i.e., meeting the NDE Content scale criteria, with total scores of 34/80 and 32/80 at the first and second interviews, respectively), notably characterized by feelings of peacefulness and well-being, encounters with entities, and the perception of a bright light. Over time, the narrative evolved, with some elements no longer recalled at follow-up, while the patient reported a positive psychological transformation, including improved mood and increased empathy.
Conclusion
This case, set within its medical and pharmacological context, provides a unique opportunity to study NDEs and their precipitating context. The reported transformations underscore the importance of recognizing NDEs for their possible long-term effects on patients and relevance to medical practice.

Neurophysiological Markers of (Dis)Connectedness: Insights from Ketamine
Publication year: 2025
URL:
https://doi.org/10.1007/978-1-0716-4603-8_8
Abstract/Results: ABSTRACT:
Ketamine is a versatile N-methyl-D-aspartate (NMDA)-antagonist used in various clinical settings. Ketamine dose-dependently induces psychoactive effects: at low, anti-hyperalgesic doses, these are minor or not detectable, but at subanesthetic doses, they may include dissociative, psychotomimetic, and psychedelic events. At an anesthetic dose, ketamine leads to a state of “(full) dissociation” or disconnection rather than unconsciousness, setting it apart from other anesthetics. This unique set of characteristics makes ketamine a particularly interesting substance for studying consciousness in clinical settings. Our chapter reviews ketamine’s neurophysiological effects and neuroimaging data, its potential to inform consciousness research, and its relevance in studying brain disorders involving altered states of consciousness. We trace recent advancements in understanding ketamine’s unique brain effects and report investigations that have developed neurophysiological indexes for consciousness without behavioral output (e.g., disconnected consciousness). Specifically, we discuss the most recent tool, the Explainable Consciousness Indicator (ECI), which uses machine learning to assess the presence of awareness and wakefulness. Finally, we look at future investigations that may utilize ketamine as therapeutic approach to neurological conditions, such as disorders of consciousness.
