The BIAL Foundation also assigns Scientific Research Scholarships for the study of neurophysiological and mental health in people, arousing the interest of researchers in the areas of Psychophysiology and Parapsychology.
To date the BIAL Foundation has supported 461 projects, more than 1000 researchers, with research groups in twenty-seven countries, resulting, until April 2013, in about 600 full papers, out of which 172 published in indexed international journals with an average impact factor of 3.6 and a substantial number of citations (1665).

Abstract/Results: RESULTS:
The objective of this study was the investigation of the relationship between spirituality, magical/paranormal ideation and religious coping in an unselected population of patients with OCD or another anxiety disorder. The sample for this study was recruited in the Department of Anxiety Disorders in the University Clinic for Psychiatry and Psychotherapy in Hamburg, Germany. Psychometric data have been collected by the Revised Paranormal Belief Scale, Magical Ideation Scale and Brief Multidimensional Measure of Religiousness/Spirituality in patients with OCD (n=49), other AD (n=36) and healthy samples (n=35).
The preliminary results showed no significant differences among the three groups with respect to the most factors, as well as no significant differences between the OCD and the anxiety groups in all factors. The only statistical significant difference was found between Healthy and non-Healthy individuals with respect to the factor of negative coping, whereby the healthy samples reached lower scores in negative religious coping than the anxiety and OCD groups.
Our study does not support prior studies, suggesting an important role of these traits in the etiopathogenesis of anxiety and OCD. On the other hand, our study validated prior study results showing a significant correlation between negative religious coping and both anxiety and obsessive-compulsive disorders. Our results underline a possible major role of negative religious coping in anxiety disorders and support a potential incorporation of R/S aspects in the psychotherapeutic treatment, targeting mainly better cognitive coping strategies for patients.

The relation between religiosity/spirituality (R/S), personal beliefs, and mental health has been extensively studied. However, concerning anxiety disorders (ADs), empirical evidence is scarce. This study investigated the differences in R/S and magical/paranormal ideation among obsessive-compulsive disorder patients (OCD; n = 49), patients with other ADs (n = 36), and healthy controls (HCs; n = 35). Our results suggest negative religious coping as being the only parameter showing significantly higher scores in OCD and AD participants in comparison with HCs. Negative religious coping reflects negative functional expressions of R/S in stressful situations. Logistic regression also suggested negative religious coping as the strongest predictor of group affiliation to the nonhealthy group. Further results show no significant differences between other R/S, magical, and paranormal ideation traits among groups. This study underlines an important role of negative religious coping in ADs yet does not clearly indicate a specific causality. Religious-sensitive treatment targeting cognitive aspects of negative religious coping are discussed.

Background
In the last decades the relationship between religiosity/spirituality (R/S), personal beliefs and mental health has been extensively studied, indicating a significant correlation of these variables. However, the specific relation of R/S to anxiety disorders has been less investigated.
Objective
The objective of this prospective study is the investigation of the relation of R/S, magical ideation (MI) and paranormal beliefs (PB) to anxiety disorders in general and OCD in particular, in order to specifically determine a possible impact of these variables on psychopathology scores. In addition, the relation between R/S, PB and MI can be systematically investigated.
Design & method
Unselected samples with OCD and other anxiety disorders have been equally assessed within the first week after admission with a face-to-face interview including the following instruments: MINI, HAMD, HAMA and Y-BOCS, OCI-R, STAI, Brief Multidimensional Measure of Religiosity/Spirituality, Magical Ideation Scale and Revised Paranormal Beliefs Scale. Forty healthy participants with no psychiatric history served as controls.
Results
Groups did not differ in any aspect of R/S, MI, or PB. Almost all scales referring to R/S, PB and MI were positively correlated to each other. The results showed a strong positive correlation between high scores of negative religious coping and high psychopathology scores in depression and anxiety. On the other hand, there was a significant positive correlation between MI scores and initial OCD and anxiety scores.
Conclusion
This study verifies a significant correlation between personal beliefs and psychopathology in OCD and anxiety disorders. Implications for further research are discussed.

The BIAL Foundation was created in 1994 by Laboratórios BIAL in conjunction with the Council of Rectors of Portuguese Universities. BIAL’s Foundation mission is to foster the scientific study of Man from both the physical and spiritual perspectives.
Along the years the BIAL Foundation has developed an important relationship with the scientific community, first in Portugal and after worldwide. Today it is an institution of reference which aims to stimulate new researches that may help people, promote more health and contribute to new milestones to gain access to knowledge.
Among its activities the BIAL Foundation manages the BIAL Award, created in 1984, one of the most important awards in the Health field in Europe. The BIAL Award rewards both the basic and the clinical research distinguishing works of major impact in medical research.
The BIAL Foundation also assigns Scientific Research Scholarships for the study of neurophysiological and mental health in people, arousing the interest of researchers in the areas of Psychophysiology and Parapsychology.
To date the BIAL Foundation has supported 461 projects, more than 1000 researchers, with research groups in twenty-seven countries, resulting, until April 2013, in about 600 full papers, out of which 172 published in indexed international journals with an average impact factor of 3.6 and a substantial number of citations (1665).
Since 1996 the BIAL Foundation organizes the Symposia entitled "Behind and Beyond the Brain", a Forum that gathers well renowned neurosciences speakers and the BIAL Foundation Fellows which are spread around the world.
Classified as an institution of public utility, the BIAL Foundation includes among its patrons the Portuguese President, the Portuguese Universities Rectors' Council and the Portuguese Medical Association.

Abstract/Results: ABSTRACT:
OCD has been hypothesized to involve the failures in both cognitive and behavioral inhibitory processes. There is evidence that the hyperactivation of cortical-subcortical pathways may be involved in the failure of these inhibitory systems associated with OCD. Despite this consensus on the role of frontal-subcortical pathways in OCD, recent studies have been showing that brain regions other than the frontal-subcortical loops may be needed to understand the different cognitive and emotional deficits in OCD. Some studies have been finding evidence for decreased metabolic activity in areas such as left inferior parietal and parieto-occipital junction suggesting the possible existence of visual processing deficits. While there has been inconsistent data regarding visual processing in OCD, recent studies have been claiming that these patients have abnormal patterns of visual processing social rich stimuli, particularly emotional arousing stimuli. Thus, in this article, we hypothesize that the fronto-subcortical activation consistently found in OCD may be due to a deactivation of occipital/parietal regions associated with visual-perceptual processing of incoming social rich stimuli. Additionally, this dissociation may be more evident as the emotional intensity of the social stimulus increases.

Self-report measures are often used in research and clinical practise as they efficiently gather a large amount of information. With growing numbers of self-report measures available to target single constructs, it is important to revisit one's choice of instrument to be sure that the most valid and reliable measure is employed. The Maudsley Obsessive-Compulsive Inventory (MOCI) and the Obsessive-Compulsive Inventory-Revised (OCI-R) were administered to 223 female participants: 30 inpatients with anorexia nervosa (AN), 62 community cases with AN, 69 community cases weight restored from AN and 62 healthy controls. Both measures distinguished between clinical and healthy groups; however, the OCI-R showed superior internal reliability. Additionally, the OCI-R measures six (to the MOCI's four) obsessive-compulsive constructs, and uses a more sensitive response format (likert scale vs. categorical). It is recommended that the OCI-R be employed as the self-report instrument of choice for assessing obsessive-compulsive pathology in those with AN.

OBJECTIVES:
Religion is a complex human universality, affecting many different parameters of human thinking and behaviour, suggested to also play a major role with respect to physical and mental human health. Although the relationship between religiosity/spirituality (R/S) and mental health has been extensively studied in general in the last two decades and underlined by many biological studies, a literature review shows that the relation of anxiety disorders to R/S is less investigated, than in other psychiatric disorders. Nevertheless religion has often been thought to play some role in the genesis of obsessive–compulsive disorder. At last there are many findings supporting the thesis that beside R/S, magical ideation (MI) and paranormal beliefs (PB) can also have an influence on psychiatric disorders and especially anxiety and OCD, while R/S seems to also have a
special relationship to magical ideation and paranormal beliefs. The objective of the study is to investigate the relation of R/S, MI and PB to each other and to anxiety disorders in general and OCD in particular. The primary aim is to determine resilience and risk factors for the pathogenesis and treatment outcome of anxiety and obsessive-compulsive symptoms.
METHODS:
The study sample included unselected inpatient and outpatient patients with OCD and other anxiety disorders, recruited the Department for Anxiety Spectrum Disorders of the Clinic for Psychiatry and Psychotherapy in the University Medical Centre Hamburg-Eppendorf, Germany.
Patients have been equally interviewed and assessed by self-report measures at admission and discharge. All results were compared between two groups and in contrast to a control group of healthy individuals without history of mental illness. Variables of R/S, MI and PB will be analyzed with respect to anxiety and ocd scores, as well as to treatment outcome.
RESULTS:
Preliminary results of the project will be presented.
CONCLUSIONS:

Intrusive thoughts and compulsive urges to perform stereotyped behaviours are typical symptoms of obsessive-compulsive disorder. Emerging evidence suggests a cognitive bias towards habit formation at the expense of goal-directed performance in obsessive-compulsive disorder. In this study, we test this hypothesis using a novel individualized ecologically valid symptom provocation design: a live provocation functional magnetic resonance imaging paradigm with synchronous video-recording of behavioural avoidance responses. By pairing symptom provocation with online avoidance responses on a trial-by-trial basis, we sought to investigate the neural mechanisms leading to the compulsive avoidance response. In keeping with the model of habit formation in obsessive-compulsive disorder, we hypothesized that this disorder would be associated with lower activity in regions implicated in goal-directed behaviours and higher activity in regions implicated in habitual behaviours. Fifteen patients with obsessive-compulsive disorder and 15 healthy control volunteers participated in this functional magnetic resonance imaging study. Online stimuli were individually tailored to achieve effective symptom provocation at neutral, intermediate and strong intensity levels. During the symptom provocation block, the participant could choose to reject or terminate the provoking stimuli resulting in cessation of the symptom provocation. We thus separately analysed the neural correlates of symptom provocation, the urge to avoid, rejection and relief. Strongly symptom-provoking conditions evoked a dichotomous pattern of deactivation/activation in patients, which was not observed either in control conditions or in healthy subjects: a deactivation of caudate-prefrontal circuits accompanied by hyperactivation of subthalamic nucleus/putaminal regions. This finding suggests a dissociation between regions engaged in goal-directed and habitual behaviours. The putaminal hyperactivity during patients’ symptom provocation preceded subsequent deactivation during avoidance and relief events, indicating a pivotal role of putamen in regulation of behaviour and habit formation in obsessive-compulsive disorder. Effective connectivity analysis identified the ventromedial prefrontal cortex/orbitofrontal cortex as the main structure in this circuitry involved in the modulation of compulsivity in obsessive-compulsive disorder. These findings suggest an imbalance in circuitry underlying habitual and goal-directed action control, which may represent a fundamental mechanism underlying compulsivity in obsessive-compulsive disorder. Our results complement current models of symptom generation in obsessive-compulsive disorder and may enable the development of future therapeutic approaches that aim to alleviate this imbalance.

Biofeedback is applied to target excessive and/or deficient physiological signals to help patients identifying and self-managing their symptoms. Biofeedback has been employed in psychiatric disorders, including obsessive-compulsive disorder (OCD), mainly by using neural signals - neurofeedback. Recently, OCD has been integrated into the obsessive-compulsive and related disorders (OCD&RD) category (body dysmorphic, hoarding, trichotillomania/hair-pulling, and excoriation/skin-picking disorders). The efficacy of biofeedback for OCD&RD is still unknown. Our work provides a complete overview of publications assessing the therapeutic efficacy of biofeedback in OCD&RD with a systematic review and meta-analysis. We found ten studies involving 102 OCD participants (three randomized controlled trials) mostly applying neurofeedback (one publication used thermal biofeedback). Five neurofeedback studies were selected for meta-analysis (89 patients; two randomized controlled trials). The overall effect size within the treatment group varied between medium to large, but high heterogeneity and inconsistency values were found. The methodological quality was low indicating a high risk of bias. In conclusion, a beneficial effect of neurofeedback for OCD patients was found but also critical limitations on methodology, high heterogeneity among studies, and a putative reporting bias. Future research following high-quality guidelines should be conducted to address the efficacy of biofeedback approaches for OCD&RD.

Abstract/Results: ABSTRACT:
Patients with obsessive-compulsive disorder (OCD) have increased rates of neurological soft signs (NSS) when compared to healthy controls. However, previous findings have been confounded by the presence of co-morbidity with disorders themselves associated with increased NSS, such as schizophrenia. Moreover, it remains unclear whether NSS in OCD reflect a vulnerability to this disorder. This study aimed to examine: 1) the severity of NSS in patients with OCD alone, in patients with OCD and co-morbid psychosis (schizophrenia or bipolar disorders), and in healthy controls; and b) whether unaffected first-degree relatives of patients with OCD also demonstrate a higher prevalence rate of NSS than healthy controls. NSS were assessed with the Cambridge Neurological Inventory (CNI) in 100 patients with OCD, 38 patients with OCD and psychosis (22 with bipolar disorders and 16 with schizophrenia), and 101 healthy controls. Forty-seven unaffected first-degree relatives of patients with OCD only were also administered the CNI. Patients with OCD showed significantly higher scores in motor coordination and total NSS than controls, and patients with OCD co-morbid with psychosis also showed significantly higher scores in motor coordination and total NSS than controls. Although there were no differences in NSS between patients with OCD only and OCD and psychosis as a whole, patients with OCD co-morbid with schizophrenia showed significantly higher scores in motor coordination than patients with OCD, patients with OCD and bipolar disorder, and healthy controls. Unaffected first-degree relatives only showed a higher prevalence rate than healthy controls in specific motor coordination signs, such as Opposition and Extinction. These findings suggest that patients with OCD exhibit more NSS than healthy controls, and that motor coordination signs may be even more extensive when OCD is co-morbid with psychosis. Some of these abnormalities may be indicative of a vulnerability to these disorders, as indicated by their presence in un-affected first-degree relatives.

BACKGROUND
Obsessive-compulsive disorder (OCD) has consistently been linked to abnormal frontostriatal activity. The electrophysiological disruption in this circuit, however, remains to be characterized.
OBJECTIVE/HYPOTHESIS
The primary goal of this study was to investigate the neuronal synchronization in OCD patients. We predicted aberrant oscillatory activity in frontal regions compared to healthy control subjects, which would be alleviated by deep brain stimulation (DBS) of the nucleus accumbens (NAc).
METHODS
We compared scalp EEG recordings from nine patients with OCD treated with NAc-DBS with recordings from healthy controls, matched for age and gender. Within the patient group, EEG activity was compared with DBS turned off vs. stimulation at typical clinical settings (3.5 V, frequency of stimulation 130 Hz, pulse width 60 µs). In addition, intracranial EEG was recorded directly from depth macroelectrodes in the NAc in four OCD patients.
RESULTS
Cross-frequency coupling between the phase of alpha/low beta oscillations and amplitude of high gamma was significantly increased over midline frontal and parietal electrodes in patients when stimulation was turned off, compared to controls. Critically, in patients, beta (16–25 Hz) -gamma (110–166 Hz) phase amplitude coupling source localized to the ventromedial prefrontal cortex, and was reduced when NAc-DBS was active. In contrast, intracranial EEG recordings showed no beta-gamma phase amplitude coupling. The contribution of non-sinusoidal beta waveforms to this coupling are reported.
CONCLUSION
We reveal an increased beta-gamma phase amplitude coupling in fronto-central scalp sensors in patients suffering from OCD, compared to healthy controls, which may derive from ventromedial prefrontal regions implicated in OCD and is normalized by DBS of the nucleus accumbens. This aberrant cross-frequency coupling could represent a biomarker of OCD, as well as a target for novel therapeutic approaches.

PURPOSE
Insight is currently considered to be responsible for 20% to 40% of the cases of ineffective obsessive-compulsive disorder (OCD) treatment. As 15% to 36% of patients with OCD have reduced insight, we aimed to identify some of the clinical determinants of insight in OCD.
RESULTS
Our sample consisted of 57 OCD patients, of which 34 men (59%) and 23 women (41%). All individuals completed a two-phase interview consisting of a clinical assessment, emotional awareness evaluation and insight measurement, using the Brown Assessment of Beliefs Scale (BABS). The insight score correlated significantly with negative emotion recognition (p < 0.0001) and empathic concern (p = 0.003).
MAJOR CONCLUSIONS
Our results support the hypothesis that insight in OCD is related to emotional awareness, specifically emotion recognition and empathic concern. Future research should investigate the extent to which poor insight and impaired emotional awareness can be modified by psychological or pharmacological therapies and whether this will enhance treatment response.
