Abstract/Results: RESULTS:
The project was designed to evaluate the possibility of using gene therapy at the central nervous system as an analgesic tool.
Pain suppression through central electrical stimulation, although effective as an analgesic tool, is impracticable since serious secondary effects occur due to stimulation of neurons involved in other brain functions. Viral vector transduction stands nowadays as a way of overcoming this problem by directing manipulation specifically to pain-control neurons. We focused on the possibility of concomitantly inhibit a pain facilitatory center (DRt) and stimulate a pain inhibitory center (VLM) by the use of multiple transduction from each site with the replication-defective herpes simplex virus (HSV-1).
The migration pattern following injection of HSV-1 in the DRt and VLM was studied. In parallel, information on the membrane receptors present in the neurons to be targeted (DRt and VLM pain processing neurons) and on the neurotransmitters used by the neurons to be transduced was collected.
The results collected allowed us to conclude that HSV-1 is particularly well suited for the purpose of the project since it transduces just a few brain areas projecting to the VLM and DRt, allowing a limited but still amplified action upon each one, and areas other than the DRt and VLM that receive axonal terminations of transduced neurons are few. The neurochemical data point to the use of vectors coding for the GABAB receptor, GAD and noradrenalin at the DRt, and of vectors coding for GABAB and a2 receptor antisense molecules at the VLM. Behavior and pharmacological studies aimed at elucidating the effect of appropriate drugs upon injection in the sites of termination of the transduced neurons are however still needed.

Single-unit spinal recordings combined with application of glutamate into the medullary dorsal reticular nucleus were used to assess the action of this nucleus upon deep dorsal horn neurons in rats. Injection of high glutamate concentrations (10 and 100 mm) induced a dramatic and long-lasting increase of the responses of wide-dynamic range neurons to electrical stimulation of the sciatic nerve in the noxious range, without affecting ongoing discharges. Post-stimulus time histograms revealed that this increase concerned the post-discharge, but not A- or C-fibre-mediated responses, which remained unchanged independently of the stimulation frequency applied. The onset of the glutamate-induced response enhancement occurred with a concentration-dependent time delay and developed slowly until its maximum. These data indicate that the medullary dorsal reticular nucleus exerts a facilitating action upon deep dorsal horn wide-dynamic range neurons by enhancing their capacity to respond to peripheral stimulation through prolongation of their discharge. This action is accompanied by the strengthening of wind-up of deep dorsal horn wide-dynamic range neurons, hence providing a plausible substrate for chronic pain states. These results are in agreement with previous behavioural studies suggesting a pronociceptive role for the dorsal reticular nucleus [Almeida et al. (1996) Brain Res. Bull., 39, 7-15; Almeida et al. (1999) Eur. J. Neurosci., 11, 110-122], and support the involvement of a reverberating circuit, previously described in morphological studies [Almeida et al. (1993) Neuroscience, 55, 1093-1106; Almeida et al. (2000) Eur. J. Pain, 4, 373-387], which probably operates only at a certain threshold of activation.

Modulation of nociceptive transmission at the brainstem involves several neurochemical systems. The precise location and specific characteristics of nociceptive neurons activated in each system was never reported. In this study, the presence of GABA(B), mu-opioid, and neurokinin-1 (NK-1) receptors in brainstem nociceptive neurons was investigated by double-immunocytochemical detection of each receptor and noxious-evoked induction of the c-fos proto-oncogene. Noxious cutaneous mechanical stimulation significantly increased the proportions of neurons double-labelled for Fos and GABA(B) receptors in several brainstem regions, namely, the reticular formation of the caudal ventrolateral medulla (VLMlat and VLMrf), lateral reticular nucleus, spinal trigeminal nucleus, pars caudalis (Sp5C), nucleus of the solitary tract, dorsal reticular nucleus, ventral reticular nucleus, raphe obscurus nucleus and dorsal parabrachial nucleus (DPB). For mu-opioid receptors, the proportions of double-labelled neurons in noxious-stimulated animals were higher than in controls only in the VLMlat, VLMrf, Sp5C, DPB and A5 noradrenergic cell group. As for the NK-1 receptor, no significant differences were found between control and stimulated animals. According to these results, neurons expressing GABA(B), mu-opioid and NK-1 receptors at several pain control centres of the brainstem are differentially involved in processing nociceptive mechanical input. The data provide the definition of new supraspinal targets for selective modulation of nociceptive neurons in order to define better strategies of pain control.

Reorganization of central networks and plasticity of neuronal representations have been implicated in recent years in the dynamic expression of somatosensory responses. The functional properties of spinal cells were shown to change in the scale of minutes after peripheral high-intensity stimulations and to undergo profound alterations in their responses in experimental models of chronic pain. These observations, however, are restricted to recordings from individual cells, and no information exists on how these changes may be reflected on the activity of somatosensory neuronal networks involved in pain processing. To understand how spinal cord networks may be altered after the onset of hyperalgesia, we extracellularly recorded from groups of five to nine neighboring neurons in the hindlimb representation area of the dorsal horn. The multineuronal activity evoked by cutaneous innocuous and noxious stimulation was compared before and for 3 h after the subcutaneous injection of diluted formalin. Formalin caused immediate changes in response properties and mechanical threshold of activation for the majority of the neurons and induced the incorporation of previously unresponsive neighboring neurons to the functional network. Analysis of the temporal correlation within the neuronal population revealed that formalin-induced inflammation increased the functional coherence of the network to the nonnociceptive stimulation but not to the painful stimuli. This increase in the tactile acuity of populations of nociceptive neurons may be a basis for the emergence of touch-evoked pain.

Abstract/Results: ABSTRACT:
Nociceptive spinofugal pathways terminating at the caudal medulla oblongata target the dorsoreticular nucleus (DRt), the nucleus tractus solitarii (NTS) and the caudal ventrolateral reticular formation (VLM). While the spino-DRt pathway is involved in triggering pain-facilitating effects from the DRt, the spino-NTS and spino-VLM pathways have been implicated in nociceptive-cardiovascular integration. Spinal dorsal horn neurones involved are distributed through laminae I and IV-V. Curiously, the medullary projections of the two neuronal populations are in all cases segregated, pointing to differences in the role played by each of them. In the DRt, lamina I targets the ipsilateral dorsal-most portion, immediately beneath the cuneate nucleus, and laminae IV-V the remaining ventral two-thirds, bilaterally. In the NTS, lamina I targets the medial commissural subnucleus, bilaterally, and laminae IV-V the lateral subnucleus, ipsilaterally. In the VLM, lamina I targets the reticular formation (VLMlat) located between the lateral reticular nucleus (LRt) and the spinal trigeminal nucleus, pars caudalis, and laminae IV-V the lateral portion of the LRt. Furthermore, the structural types of lamina I neurones participating in the spino-NTS and spino-VLM pathways are the same (fusiform, pyramidal and flattened), while those projecting to the DRt differ, belonging mainly in the multipolar type followed by the pyramidal and flattened types. Both the NTS and VLM exert anti-nociceptive effects as revealed by the depression of nociceptive reflexes and inhibition of nociceptive responses of spinal neurones upon local stimulation. Direct pathways connect the NTS commissural subnucleus and the VLMlat with both superficial and deep spinal dorsal horn laminae. The VLMlat is also connected with the spinal cord through a disynaptic pathway relaying in the pontine A5 noradrenergic group, which is thought to convey the spinal a2-adrenoreceptor-mediated anti-nociception elicited from the VLM.
The well-known role of the NTS commissural subnucleus and the VLM in cardiovascular control, together with the demonstration that they are sites of termination of nociceptive spinal afferents, has been taken as a strong indication of the involvement of both areas in nociceptive and cardiovascular integration, namely in the genesis of autonomic reactions to pain. Taking into account their role in pain control, it is also possible that both areas can modulate spinal nociceptive transmission in response to changes in cardiovascular parameters. Hypertensive subjects, either humans or experimental animals, were shown to have higher pain thresholds than normotensive subjects. In spite of the large amount of data supporting those hypotheses, experimental studies aimed at their evaluation are lacking. We have recently addressed this issue by investigating the role of the VLM in hypertension-induced hypoalgesia. In rats in which hypertension was produced by either pharmacological (L-NAME) or surgical (renal artery constriction) means, spinal c-fos induction following noxious mechanical stimulation of the skin was reduced compared with normotensive animals. Lesioning the VLM in hypertensive animals prior to noxious stimulation prevented the decrease in spinal c-fos expression. Since the hypertension-induced hypoalgesia is mediated by spinal a2-adrenoreceptors, and the noradrenergic anti-nociceptive effects of the VLM are conveyed by the VLMlat-A5-spinal circuit, the observed VLM-dependent decrease of spinal c-fos expression in response to rises in blood presure is most likely accounted for by the VLMlat. On the other hand, the VLMlat by itself is capable of exerting the vasodepressor effects normally attributed to the VLM, as revealed by decrease in blood pressure and heart rate upon local stimulation and the opposite effect after lesioning. The VLMlat thus seems to be not only the VLM site in charge of pain control through both direct or indirect spinopetal pathways, but also an important centre for nociceptive and cardiovascular integration.

Previous studies have shown that chronic pain induces changes in the spinal cord, namely in what concerns the neurochemistry of dorsal horn circuits. It has not been yet addressed whether similar changes occur supraspinally namely at the endogenous pain modulatory system. In this study, we investigated the brainstem regions expressing the protooncogene c-fos as a result of mobilization of a chronically inflamed joint and further studied the expression of NK1 receptors in those neurons of those regions.
Male Wistar rats were injected in the tiobiotarsal joint with 50 µl of saline (control) or Complete´s Freund Adjuvant (monoarthritic). After 14 days of daily training the animals were subjected to 4 minutes of repeated extensions of the inflamed hindlimb and brainstem frozen sections were double-immunoreacted for Fos protein and NK1 receptors.
Monoarthritic rats presented an increase of Fos-immunoreactive neurons in the caudal ventrolateral medulla, lateral, dorsal and ventral reticular nuclei, spinal trigeminal nucleus, nucleus of the solitary tract, raphe obscurus, gracile, cuneate, ambiguous, vestibular and tegmental nuclei. No differences were found in the number of Fos neurons expressing NK1 receptors.
Mobilization of a chronically inflamed paw induces functional activation of several areas of the endogenous pain control system, reinforcing the importance of the system in chronic pain. In a manner similar to what occurs in acute pain (Pinto et al., 2003), NK1 receptors do not appear to be involved in supraspinal processing of chronic pain.

Abstract/Results: ABSTRACT:
The pain system is endowed with control mechanisms that modulate the ascending transmission of nociceptive input by either depressing or enhancing the responsiveness of nociceptive spinal neurons. Although effective, manipulation of brain pain-control centers as an analgesic procedure has no clinical value since it simultaneously affects neurons involved in brain functions other than pain control. In this project we explore the possibility of using viral vectors as a way of targeting specifically pain control neurons through transfection of neuronal cells that establish synaptic contacts with them. The targets chosen were the VLM and DRt because of their potent inhibitory and facilitatory effects, respectively.
The studies performed so far aimed at defining a transfection strategy that results in stimulation of the VLM and inhibition of the DRt. A replication-defective Herpes Simplex Virus was chosen since, by being retrogradely transported, it allows transfection of several brain regions projecting to the VLM and DRt, hence amplifying the effect upon each target. The migration dynamics of the virus was studied in order to identify both the brain regions that are putative sources of synaptically mediated pharmacological actions upon each target (soma exhibiting the virus product) and the regions that also receive input from the transfected neurons (axonal arborizations exhibiting the virus product). These data help in selecting the regions to be transfected from the VLM and the DRt as those that project mainly or solely to one of the two areas and do not project significantly to other brain regions. The results point to the A 5 and RVL as areas to be transfected from the VLM, and to the cerebelum, NTS, Ve and Cu as areas to be transfected from the DRt.
Complementary studies of the neurochemical nature of the transfected neurons are now in progress as a way of defining the viral construction that better conciliates the desired action upon the neurons in one target without affecting either the other target or neurons that are not involved in pain control. It was already verified that the LC contains noradrenergic neurons transfected from the VLM and DRt, and the A1
and A5 groups contain noradrenergic neurons transfected from the DRt. The surface receptor properties of VLM and DRt nociceptive neurons were evaluated in order to identify the neurotransmitters whose synthesis should be amplified or blocked by the vectors acting upon each target. GABA B receptors were found to occur in nociceptive neurons of both the VLM and DRt, and opioid receptors only in the VLM. These data point to the use of a pré-pro-enkephalin and GABAB antisense construction at the VLM and a GABA precursor construction at the DRt.

Abstract/Results: ABSTRACT:
The studies performed in grant 84/04 "THE CONSCIOUSNESS OF PAIN" led to significant advances in our understanding of the neurobiological mechanisms that induce cognitive alterations under chronic pain conditions. Our studies have introduced for the first time the use of complex cognitive tasks in animal models of chronic pain. The relevance of our studies is notorious not only from the quality of the scientific publications in which we have reported our results, but also from the impact that these studies have achieved in the Pain Research scientific community as inferred by the many invitations to present our results in international meetings.
Specifically, in this Project we have:
1 – developed for the first time a cognitive task for emotional decision making under ambiguity – the Rodent Gambling Task – that is sensitive to prefrontal and amygdalar functioning just like is observed in the human brain;
2 – demonstrated that prefrontal executive function assessed in our decision-making task is impaired in chronic pain animals, and that this pain-induced impairment is as severe as after a prefrontal lesion;
3 – demonstrated that chronic pain changes the neurochemical balance of dopamine and serotonin in the prefrontal cortex, amygdala, hippocampus, and ventral striatum;
4 – demonstrated that chronic pain impairs working memory without affecting preformance dependent on long-term memory.
5 – demonstrated that chronic pain impairs attentional processing, without affecting choice impulsivity as some authors had previously suggested;
6 – developed a novel software for acquition of behavioral animal data and control of experimental arenas that was freely available for download by the neuroscience community.

Emotional–affective and cognitive dimensions of pain are less well understood than nociceptive and nocifensive components, but the forebrain is believed to play an important role. Recent evidence suggests that subcortical and cortical brain areas outside the traditional pain processing network contribute critically to emotional–affective responses and cognitive deficits related to pain. These brain areas include different nuclei of the amygdala and certain prefrontal cortical areas. Their roles in various aspects of pain will be discussed. Biomarkers of cortical dysfunction are being identified that may evolve into therapeutic targets to modulate pain experience and improve pain-related cognitive impairment. Supporting data from preclinical studies in neuropathic pain models will be presented. Neuroimaging analysis provides evidence for plastic changes in the pain processing brain network. Results of clinical studies in neuropathic pain patients suggest that neuroimaging may help determine mechanisms of altered brain functions in pain as well as monitor the effects of pharmacologic interventions to optimize treatment in individual patients. Recent progress in the analysis of higher brain functions emphasizes the concept of pain as a multidimensional experience and the need for integrative approaches to determine the full spectrum of harmful or protective neurobiological changes in pain.

In 1994 the BIAL Foundation launched a programme of science research grants with the aim of encouraging the research into Man’s physical and mental processes, namely in fields still largely unexplored but which warrant further scientific analysis, as Psychophysiology and Parapsychology.
Since its launch, applications to the BIAL grants have been increasing. Up to now 461 projects have been supported, involving more than 1000 researchers from 27 countries.
The approved applications have benefited from grants in amounts comprised between €5,000 and €50, 000. The amount to be granted is fixed by the Scientific board according to the needs of each project.
The supported projects have originated, until April 2013, in about 600 full papers, 172 out of which were published in indexed international journals with an average impact factor of 3.6 and a substantial number of citations (1665).
Among the BIAL Foundation fellows is worth highlighting the presence of scientists from prestigious universities from the United States, United Kingdom, Australia, Russia, Germany, Japan, France, Canada, and many others.
The BIAL grants are promoted biannually.

Abstract/Results: ABSTRACT:
Somatosensory deep dorsal horn spinal neurons were previously shown to present in vitro a bistable state of activity in which a fixed firing rate is maintained over prolonged periods in the absence of stimulation. Those periods of enhanced spinal spontaneous discharge may play a role in the genesis or maintenance of hyperalgesic states, where episodes of durable spontaneous pain are commonly reported. Here we show in vivo that a small percentage of deep spinal neurons (4% of the recorded population) are capable of rapidly shifting between low-frequency and high-frequency levels of spontaneous activity. At least one of the transitions between the two states was induced by stimulation of the receptive field, making this an interesting and unique case in which stable firing rates are switched-on or -off by somatosensory stimuli.

Understanding basic neuronal mechanisms hold the hope for future treatment of brain disease. The 1st international conference on synapse, memory, drug addiction and pain was held in beautiful downtown Toronto, Canada on August 21–23, 2006. Unlike other traditional conferences, this new meeting focused on three major aims: (1) to promote new and cutting edge research in neuroscience; (2) to encourage international information exchange and scientific collaborations; and (3) to provide a platform for active scientists to discuss new findings. Up to 64 investigators presented their recent discoveries, from basic synaptic mechanisms to genes related to human brain disease. This meeting was in part sponsored by Molecular Pain, together with University of Toronto (Faculty of Medicine, Department of Physiology as well as Center for the Study of Pain). Our goal for this meeting is to promote future active scientific collaborations and improve human health through fundamental basic neuroscience researches. The second international meeting on Neurons and Brain Disease will be held in Toronto (August 29–31, 2007).

Cognitive deficits such as impaired decision-making can be a consequence of persistent pain. Normal functions of the intact amygdala and prefrontal cortex are required for emotion-based decision-making that relies on the ability to assess risk, attribute value and identify advantageous strategies. We tested the hypothesis that pain-related cognitive deficits result from amygdala-driven impairment of medial prefrontal cortical (mPFC) function. To do this, we used electrophysiological single-unit recordings in vivo, patch-clamp in brain slices, and various behavioral assays to show that increased neuronal activity in the amygdala in an animal model of arthritis pain was accompanied by decreased mPFC activation and impaired decision-making. Further, pharmacologic inhibition (with a CRF1 receptor antagonist) of pain-related hyperactivity in the basolateral amygdala (BLA), but not central amygdala (CeA), reversed deactivation of mPFC pyramidal cells and improved decision-making deficits. Pain-related cortical deactivation resulted from a shift of balance between inhibitory and excitatory synaptic transmission. Direct excitatory transmission to mPFC pyramidal cells did not change in the pain model whereas polysynaptic inhibitory transmission increased. GABAergic transmission was reduced by non-NMDA receptor antagonists, suggesting synaptic inhibition was glutamate-driven. The results are consistent with a model of BLA-driven feed-forward inhibition of mPFC neurons. In contrast to the differential effects of BLA versus CeA hyperactivity on cortical-cognitive functions, both amygdala nuclei modulate emotional-affective pain behavior. Thus this study shows that the amygdala contributes not only to emotional-affective but also cognitive effects of pain. The novel amygdalo-cortical pain mechanism has important implications for our understanding of amygdala functions and amygdalo-cortical interactions.
