This article argues for the recognition of two categories of end-of-life experiences (ELEs) – transpersonal ELEs, which are related to the process of dying, and final-meaning ELEs, which prompt the dying to come to terms with unfinished issues. Both categories have existential or substantive qualities that appear to relieve the spiritual discomfort of the dying.

Abstract/Results: ABSTRACT:
Anecdotal evidence suggests that death may be heralded by phenomena which seem to comfort dying persons and prepare them spiritually for death. Medical practitioners have been slow to recognize these end of life experiences (ELEs) and there has been little research into the way carers respond when patients try to talk about phenomena they have experienced, and how these ELEs influence the carers’ work. A pilot study with a palliative care team revealed that patients regularly report these phenomena as an important part of their dying process, and that the experiences are far broader than the traditional image of an apparition at the end of the bed. The study suggested that many Palliative Carers are not trained to help them recognize the wider implications of ELEs, or to deal with difficult questions or situations, which arise from them. Many ELEs may go unreported because of this. The pilot study also suggested that ELEs are not drug-induced, and that patients would rather talk to nurses than doctors about their experiences. There are many subjective phenomena in common between ELEs and near death experiences suggesting that they may form part of a single process.

Results of the interviews raise concerns about the lack of education or training to help palliative care teams recognize the wider implications of DBP and deal with difficult questions or situations associated with them. Abstract/Results: ABSTRACT:
The attitude towards death and dying depends on the culture. In prehistoric times grave artifacts suggest a belief in the continuation of life. This belief in an afterlife has continued through different cultures and societies to the present day. The fear of death seems to have grown in parallel with those religions which have promised judgment at the time of death. In our modern Western secular society death is regarded as a medical failure, the rituals which used to attend it have largely been abandoned, and life is prolonged so that death has lost all dignity. It is now beginning to be recognised that dying may not be a simple switching off, but a process leading to death and the gradual dissolution of consciousness. This dissolution seems to involve experiences for the dying which are spiritual and important for them. A number of these phenomena raise the possibility that consciousness may not be limited to the brain, but extend beyond it. Fortunately, palliative care is now taught in medical schools, and treatment of the dying is now recognised to be as important as treatment for the living. This article looks at the history of death, the significance of the dying process for consciousness research, and the education needed for carers of the dying.

Abstract/Results: ABSTRACT:
Research literature has highlighted unusual phenomena occurring at the end of life. Palliative-care professionals often feel ill-prepared in managing these and in talking to patients and family members about them. This study aimed to explore the meanings and interpretations ascribed to these phenomena by palliative-care professionals. Eight participants were interviewed, and interpretative phenomenological analysis used to identify themes within their accounts. Four themes emerged from the analysis: (1) Who are we to say what's out there?: a connection with something beyond what can be seen; (2) It opened up conversations: the experience of talking about unusual experiences; (3) It knocked me sideways: managing the emotional impact of these experiences; and (4) The fact that she was so accepting made it easier: the value of acceptance in relation to unusual experiences. These findings are discussed within the context of existing literature and implications for palliative-care professionals are discussed.

OBJECTIVE:
Optimal communication is essential in ensuring that the palliative care needs of patients are met. This continues to be an area of concern for healthcare providers. The goal of our present review was to gain a deeper understanding of the communication experiences of patients with palliative care needs that have been identified within the qualitative literature.
METHOD:
A systematic search for qualitative research papers was undertaken in February of 2012. Five databases (ASSIA, CINAHL, MEDLINE, PsychArticles, and PsychINFO) were searched using the search terms [“palliative care” OR “terminal care” OR “end of life care”] AND [“experience” OR “perspective” OR “qualitative” OR “interview”] AND [“patients” OR “clients” OR “service-user”]. Meta-synthesis was conducted on the data within the found papers.
RESULTS:
A line-of-argument synthesis of 15 studies yielded four overarching themes: talking—facilitating and inhibiting factors; the importance of humanitarian qualities within communication encounters; perceptions of autonomy within communication experiences; and individual differences in preferences for honesty within interactions.
SIGNIFICANCE OF RESULTS:
Our findings are discussed in relation to existing literature and offer a deeper insight into the communication experiences of this clinical population. A number of clinical implications are offered for the healthcare professionals who are providing support to patients with palliative care needs.

The author conducted a systematic review following PRISMA guidelines to characterize end-of-life experiences and their effects on patients, families, healthcare professionals, and volunteers in palliative care settings. The databases used were PubMed, PsycInfo, and Web of Science. Of 7095 articles found, 12 met the eligibility criteria. The prevalence of end-of-life experiences ranged from 28 to 87% and dreams and visions were the most reported. Eight common themes were found: end-of-life experiences are not uncommon (100%); end-of-life experiences can be comforting or not (83.3%); spiritual transformation of end-of-life experiences (58.3%); hallucinations and end-of-life experiences (50%); normalization of end-of-life experiences (50%); training of health professionals (41.6%); end-of-life experiences as prognostic indicators (33.3%); contribution of end-of-life experiences in grief (25%). These experiences, whether comforting or distressing, play a therapeutic role in facilitating a peaceful death and aiding the grieving process.
