Among its activities the BIAL Foundation manages the BIAL Award, created in 1984, one of the most important awards in the Health field in Europe. The BIAL Award rewards both the basic and the clinical research distinguishing works of major impact in medical research.
The BIAL Foundation also assigns Scientific Research Scholarships for the study of neurophysiological and mental health in people, arousing the interest of researchers in the areas of Psychophysiology and Parapsychology.
To date the BIAL Foundation has supported 461 projects, more than 1000 researchers, with research groups in twenty-seven countries, resulting, until April 2013, in about 600 full papers, out of which 172 published in indexed international journals with an average impact factor of 3.6 and a substantial number of citations (1665).
Since 1996 the BIAL Foundation organizes the Symposia entitled "Behind and Beyond the Brain", a Forum that gathers well renowned neurosciences speakers and the BIAL Foundation Fellows which are spread around the world.
Classified as an institution of public utility, the BIAL Foundation includes among its patrons the Portuguese President, the Portuguese Universities Rectors' Council and the Portuguese Medical Association.

Abstract/Results: ABSTRACT:
The present experiment investigated whether administration of placebo affected heart rate variability during heat pain. It was hypothesized that the ratio of low frequent to high frequent (LF/HF) heart rate variability would decrease after administration of an inert substance together with information that it was a powerful painkiller. In a within subjects design, 20 participants (10 females) were tested on two separate days, one day for the placebo condition and one day for control. In the placebo condition, the participants received two capsules containing 75mg lactose each during the second of five pain tests, with information that the pills were a high dose of a standard overthe-counter analgesic with high pain analgesic effect on heat pain. In the
control condition, the same subjects underwent the same five pain tests, but without placebo administration. Pain tests consisted of heat pain (+46C, duration 240 seconds) to the forearm. ECG was recorded continuously for spectral analysis of heart rate variability. Subjective pain intensity, pain unpleasantness, stress and arousal were measured on VAS scales during each pain test. In addition, mood was measured by the SAM. Results revealed that the LF/HF ratio during painful stimulation decreased significantly in the
placebo condition compared to the control condition after placebo administration (F(1,19)=8.76, p<.01), but this effect was only significant for the first two pain tests after placebo administration. Moreover, a main effect of placebo on heart rate variability was found (F(1,19)=8.43, p<.01), with lower ratio of LF/HF in the placebo condition. There was lower pain intensity (F(1,19=14.99, p<.01) and pain unpleasantness (F(1,19)=4.74, p=.04) in the placebo condition compared to the control condition. Subjective stress during pain was decreased after placebo administration, and there was lower subjective stress in the placebo condition compared to the control condition (F(1,19)=13.06, p<.01). There were no significant effects on arousal or mood. The results from the present experiment suggest that placebo analgesia is accompanied by a reduction in cardiac autonomic activation and a reduction of subjective negative emotions.

In the control condition, the same subjects underwent the same five pain tests, but without placebo administration. Pain tests consisted of heat pain (+46C, duration 240 seconds) to the forearm. ECG was recorded continuously. Subjective pain intensity, pain unpleasantness, stress and arousal were measured on VAS scales during each pain test. In addition, mood was measured by the SAM. Results revealed that the LF/HF ratio during painful stimulation decreased significantly in the placebo condition after placebo administration (F (1, 57) = 7.08, p = 0.01). There was lower pain intensity (F (1,62) = 20.53, p = 0.01) in the placebo condition compared to the control condition. Subjective stress during pain was decreased after placebo administration (F (1, 62) = 7.45, p = 0.01), and there was lower subjective stress in the placebo condition compared to the control condition (F (1, 62) = 5.39, p = 0.02). There were no significant effects on pain unpleasantness, arousal or mood. The results from the present experiment suggest that placebo analgesia is accompanied by a reduction in cardiac autonomic activation and a reduction of subjective stress.

AIM:
To investigate whether an active placebo drug, caffeine, could enhance the placebo analgesic response. It was tested whether the subjective, arousing effects of caffeine could enhance expectancies that active medication had been administrated, and increase placebo analgesia.
METHODS:
Administration of grapefruit juice with or without 4 mg/kg caffeine was crossed with information that the drink contained a potent painkiller or placebo in a balanced placebo within-subjects design. Pain was induced by a Nd:YAP 1340 Stimul laser and electroencephalography was recorded from 32 electrodes. Preliminary data are reported.
RESULTS:
A placebo response was seen on P2 amplitude. Caffeine had no analgesic response, and did not significantly increase arousal. Contrary to the hypothesis, the active placebo caffeine administered with information that it was an analgesic blocked the placebo response on P2 amplitude.
DISCUSSION:
The EEG data clearly showed a placebo analgesic response to information that a painkiller had been administrated, but caffeine did not enhance the placebo analgesic response. This could be due to the weak effect of caffeine on subjective arousal. The surprising observation that caffeine reduced the placebo analgesic response suggests that administration of a stimulant drug may interact with placebo analgesic responding.

An active placebo is a drug that has subjective side effects, but does not reduce the symptom under investigation. Since the active placebo has subjective effects, it may inform the participant that a drug, and not a placebo, has been administrated. Therefore, it was investigated whether side effects of a drug may provide information that can modulate expectancies towards treatment. Administration of 0 mg/kg bw or 4 mg/kg caffeine was crossed with information that the drink contained a potent painkiller or placebo in a balanced placebo within-subjects design (N=15). Four mg/kg caffeine was administrated to induce side effects, e.g., increased arousal, hypothesized to enhance expectations that an active drug had been administrated. An enhancement of expectations by caffeine, with a consequent increase in the placebo effect, would be evidence that drug side effects could increase the placebo effect. Pain was induced by a Nd:YAP 1340 Stimul laser before, and 30 minutes after administration of the drink. Electroencephalography was recorded from 32 electrodes. The results showed that caffeine heightened arousal, but expectancies of pain reduction were not significantly higher in the active placebo condition compared to the placebo condition. Caffeine did not enhance the credibility of the information, probably because arousal varies during the day and only small changes in arousal were induced by caffeine. Four mg/kg caffeine reduced reported pain, but not P2 amplitude.

People have always been fascinated by the extent to which belief or will may influence behavior. Proverbs, like “we tend to get what we expect,” and concepts, such as optimistic thinking or self-fulfilling prophecy, reflect this intuition of an important link between one’s dispositions and subsequent behavior. In other words, one’s predictions directly or indirectly cause them to become true. In a similar manner, every culture, country or religion has their own words for ‘expectation,’ ‘belief,’ ‘disappointment,’ ‘surprise,’ and generally all have the same meaning: under uncertainty, what one expects or believes is the most likely to happen. This relation between what caused a reaction in the past will probably cause it again in the future might not be realistic. If the expected outcome is not confirmed, it may result in a personal ‘disappointment’, and if the outcome fits no expectations, it will be a ‘surprise’. Our brain is hardwired with this heuristic capacity of learning the cause-effect relationship and to project its probability as the basis for much of our behavior, as well as cognitions. This experience-based expectation is a form of learning that helps the brain to bypass an exhaustive search in finding a satisfactory solution. Expectations may thus be considered an innate theory of causality; that is, a set of factors (causes) generating a given phenomenon (effects) influence the way we treat incoming information but also the way we retrieve the stored information. These expectancy templates may well represent one of the basic rules of how the brain processes information, affecting the way we perceive the world, direct our attention and deal with conflicting information. In fact, expectations have been shown to influence our judgments and social interactions, along with our volition to individually decide and commit to a particular course of action. However, people’s expectations may elicit the anticipation of their own automatic reactions to various situations and behaviors cues, and can explain that expecting to feel an increase in alertness after coffee consumption leads to experiencing the consequent physiologic and behavioral states. We call this behavior-response expectancy. This non-volitional form of expectation has been shown to influence cognitions such as memory, pain, visual awareness, implicit learning and attention, through the mediation of phenomena like placebo effects and hypnotic behaviors. Importantly,when talking about expectations, placebo and hypnosis, it is important to note that we are also talking about suggestion and its modulating capability. In other words, suggestion has the power to create response expectancies that activate automatic responses, which will, in turn, influence cognition and behavior so as to shape them congruently with the expected outcome. Accordingly, hypnotic inductions are a systematic manipulation of expectancy, similar to placebo, and therefore they both work in a similar way. Considering such assumptions, the major question we address in this PhD thesis is to know if these expectancy-based mechanisms are capable of modulating more high-level information processing such as cognitive conflict resolution, as is present in the well-known Stroop task. In fact, in a recent series of studies, reduction or elimination of Stroop congruency effects was obtained through suggestion and hypnotic induction. In this PhD thesis, it is asked whether a suggestion reinforced by placebos, operating through response-expectancy mechanisms, is able to induce a top-down cognitive modulation to overcome cognitive conflict in the Stroop task, similar to those results found using suggestion and hypnosis manipulation.

Background
Pain induced by electrical stimuli has been found in previous research to be reduced by brief, weak electrical pulses, termed prepulses, presented 40 to 60 ms prior to the painful electrical stimulus.
Methods
The present experiment investigated the generality of this effect by presenting weak acoustic stimuli simultaneously with, or 80 or 1000 ms prior to, painful electric shocks. In the second half of the experimental session, each participant (N = 119) was told that the acoustic stimuli would either increase or decrease the pain induced by the electric shock, to investigate automatic and controlled cognitive processes in the modulation of pain.
Results
Acoustic stimuli presented simultaneously with painful stimulation increased pain slightly (4 mm on a 100 mm scale). Acoustic stimuli presented 80 and 1000 ms prior to painful stimuli had no effect on pain. Information that acoustic stimuli would increase pain did so in females, but only when the acoustic stimulus was presented 80 ms prior to the painful stimulus.
Conclusions
The effect of the acoustic stimuli and of information was weak. Failure to replicate previous findings of decreased pain by weak prepulses was most likely due to the sensory modality of the prepulse stimuli. It is recommended that further studies of pain modulation by brief stimulation use electrical and not acoustic prepulse stimuli.

BACKGROUND
The stimulation of the temporal areas of the brain with mild electromagnetic pulsing has been associated with experiences of a sensed presence.
AIMS
This study explored the roles of paranormal belief, time of day and wearing a sham God Helmet on state of consciousness and the frequency of reported anomalous experiences. The contributing roles of synesthesia, hyperaesthesia and locus of control (LOC) were also explored.
METHOD
Thirty-two skeptics and 35 believers took part in an orientation session and three study sessions: a morning baseline (no helmet/baseline) session, a morning sham helmet session and an afternoon sham helmet session. Participants relaxed in a Faraday chamber for 30 minutes with the instruction to observe and verbalize their experiences. Exit interviews explored subjective experiences. Participants completed the Phenomenology of Consciousness Inventory (PCI). An inductive thematic analysis identified a coding scheme for types of anomalous experiences.
RESULTS
Believers scored higher than disbelievers on Altered Experiences (AE), Positive Affect (PA) and Imagery (IM), but there were no differences in scoring on any of PCI dimensions between the three study conditions. Transcripts were blind coded for the occurrence of 15 types of anomalous experience. A series of chi square analyses identified more anomalous experiences for believers than skeptics. There were also more visual presence, flashes of color and auditory hallucinations in the helmet sessions compared to the baseline. Belief, anomalous experiences, hyperaesthesia and synesthesia correlated with some PCI variables and anomalous experiences.
CONCLUSIONS
Individual differences play a strong role in the aetiology of anomalous experiences when people wear a sham God helmet.

It has been claimed that applying weak complex electromagnetic patterns to the temporal lobes in a “God Helmet” stimulates the intrusion of right-hemispheric processes to awareness, resulting in exceptional experiences (ExEs). We explored the roles of wearing a sham helmet, time of day, and individual differences (paranormal belief, synesthesia, locus of control, hyperesthesia, and prior anomalous experiences) in alterations in consciousness and ExEs in the absence of neural stimulation. Thirty-two skeptics and 35 paranormal believers completed baseline, sham (morning), and sham (afternoon) conditions. Participants relaxed in a Faraday chamber for 30?minutes. Exit interviews explored subjective experiences and participants completed the Phenomenology of Consciousness Inventory (PCI). A mixed-model analysis of covariance found that believers scored higher than skeptics on some PCI dimensions, there was no influence of study conditions on PCI scores, and there was no interaction between belief and study conditions. An inductive thematic analysis identified a coding scheme for ExE. Believers reported more ExEs than skeptics. Regression models supported roles for hyperesthesia in alterations in consciousness and synesthesia in ExEs.

Hypnosis is a unique form of top-down regulation in which verbal suggestions are capable of eliciting pronounced changes in a multitude of psychological phenomena. Hypnotic suggestion has been widely used both as a technique for studying basic science questions regarding human consciousness but also as a method for targeting a range of symptoms within a therapeutic context. Here we provide a synthesis of current knowledge regarding the characteristics and neurocognitive mechanisms of hypnosis. We review evidence from cognitive neuroscience, experimental psychopathology, and clinical psychology regarding the utility of hypnosis as an experimental method for modulating consciousness, as a model for studying healthy and pathological cognition, and as a therapeutic vehicle. We also highlight the relations between hypnosis and other psychological phenomena, including the broader domain of suggestion and suggestibility, and conclude by identifying the most salient challenges confronting the nascent cognitive neuroscience of hypnosis and outlining future directions for research on hypnosis and suggestion.

Advocates of neurofeedback make bold claims concerning brain regulation, treatment of disorders, and mental health. Decades of research and thousands of peer-reviewed publications support neurofeedback using electroencephalography (EEG-nf); yet, few experiments isolate the act of receiving feedback from a specific brain signal as a necessary precursor to obtain the purported benefits. Moreover, while psychosocial parameters including participant motivation and expectation, rather than neurobiological substrates, seem to fuel clinical improvement across a wide range of disorders, for-profit clinics continue to sprout across North America and Europe. Here we highlight the tenuous evidence supporting EEG-nf and sketch out the weaknesses of this approach. We challenge classic arguments often articulated by proponents of EEG-nf and underscore how psychologists and mental health professionals stand to benefit from studying the ubiquitous placebo influences that likely drive these treatment outcomes.

OBJECTIVE:
We propose that clinicians can use suggestion to help treat conditions such as ADHD. Methods: We use EEG neurofeedback as a case study, alongside evidence from a recent pilot experiment utilizing a sham MRI scanner to highlight the therapeutic potential of suggestion-based treatments.
RESULTS:
The medical literature demonstrates that many practitioners already prescribe treatments that hardly outperform placebo comparators. Moreover, the sham MRI experiment showed that, even with full disclosure of the procedure, suggestion alone can reduce the symptomatology of ADHD.
CONCLUSION:
Non-deceptive suggestion-based treatments, especially those drawing on accessories from neuroscience, may offer a safe complement and potential alternative to current standard of care for individuals with ADHD.

Abstract/Results: ABSTRACT:
Disbelief in paranormal phenomena has been relatively under-researched in the literature. Thirty-three strong disbelievers participated in a laboratory study designed to encourage exceptional experiences (ExE). Participants relaxed under sensory deprivation conditions on three occasions. On two out of three occasions, they wore a sham head device, which has been previously associated with ExE. Twenty-four participants reported ExE. Verbal descriptions of ExE were analyzed using Thematic Analysis (TA). Four themes were identified: intensified thoughts and perception-like imagery, actorobserver separation, attributions of normality, and autonomy versus control. This study complements and extends the existing literature on the psychology of paranormal disbelief and skepticism.

To what extent can one’s mind promote direct changes to the body? Can one’s beliefs about the body become a physical reality, without mediating effects from behaviors? Specifically, can medical symptoms and the course of a disease be directly affected by a person’s mindset about the illness?
There is a vast literature about placebo and nocebo effects, that promote physical changes by creating the expectation of a change through a primer (for example, a fake pill). Placebos, however, often imply deception, or at least ambiguity, to be effective. The concept of Illness Expectation describes the expectations, both implicit and explicit, that a person who has received a diagnosis makes about the course of the disease. It can be characterized by different degrees of rigidity, and it is argued here that these expectations can ultimately lead to changes in the disease progression. These changes may happen through behavior modifications, or through a non-behavioral pathway, which may deserve exploration efforts from the scientific literature.

Abstract/Results: ABSTRACT:
OBJECTIVE
This study explores the hypothesis that cognitive expectations of catching influenza-like symptoms increase the chances of developing the symptoms over the winter season.
DESIGN
Self-reported data from 247 healthy volunteers were obtained twice, before and after the winter season. In the first assessment, expectations about developing influenza-like symptoms in the incoming months were charted. This data was matched with a post-winter assessment of the actual development of the symptoms.
RESULTS
The odds of developing symptoms were highly associated with the expectations declared months before (OR?=?1.776), and the association remained stable (OR?=?1.453) even when accounting for previous influenza-like illnesses and the perception of general health. In contrast with previous findings, perceived stress was not associated with symptom development.
CONCLUSIONS
These results support the hypothesis of a self-fulfilling prophecy mechanism related to influenza-like symptoms.

The BIAL Foundation was created in 1994 by Laboratórios BIAL in conjunction with the Council of Rectors of Portuguese Universities. BIAL’s Foundation mission is to foster the scientific study of Man from both the physical and spiritual perspectives.
Along the years the BIAL Foundation has developed an important relationship with the scientific community, first in Portugal and after worldwide. Today it is an institution of reference which aims to stimulate new researches that may help people, promote more health and contribute to new milestones to gain access to knowledge.
Among its activities the BIAL Foundation manages the BIAL Award, created in 1984, one of the most important awards in the Health field in Europe. The BIAL Award rewards both the basic and the clinical research distinguishing works of major impact in medical research.
The BIAL Foundation also assigns Scientific Research Scholarships for the study of neurophysiological and mental health in people, arousing the interest of researchers in the areas of Psychophysiology and Parapsychology.
To date the BIAL Foundation has supported 461 projects, more than 1000 researchers, with research groups in twenty-seven countries, resulting, until April 2013, in about 600 full papers, out of which 172 published in indexed international journals with an average impact factor of 3.6 and a substantial number of citations (1665).
Since 1996 the BIAL Foundation organizes the Symposia entitled "Behind and Beyond the Brain", a Forum that gathers well renowned neurosciences speakers and the BIAL Foundation Fellows which are spread around the world.
Classified as an institution of public utility, the BIAL Foundation includes among its patrons the Portuguese President, the Portuguese Universities Rectors' Council and the Portuguese Medical Association.

In 1994 the BIAL Foundation launched a programme of science research grants with the aim of encouraging the research into Man’s physical and mental processes, namely in fields still largely unexplored but which warrant further scientific analysis, as Psychophysiology and Parapsychology.
Since its launch, applications to the BIAL grants have been increasing. Up to now 461 projects have been supported, involving more than 1000 researchers from 27 countries.
The approved applications have benefited from grants in amounts comprised between €5,000 and €50, 000. The amount to be granted is fixed by the Scientific board according to the needs of each project.
The supported projects have originated, until April 2013, in about 600 full papers, 172 out of which were published in indexed international journals with an average impact factor of 3.6 and a substantial number of citations (1665).
Among the BIAL Foundation fellows is worth highlighting the presence of scientists from prestigious universities from the United States, United Kingdom, Australia, Russia, Germany, Japan, France, Canada, and many others.
The BIAL grants are promoted biannually.

Social psychologists, placebo scientists, and consumer researchers often require deception in their studies, yet they receive little training on how to deceive effectively. Ineffective deception, however, can lead to suspicion and compromise the validity of research. The field of magic offers a potential solution; magicians have deceived audiences for millennia using a variety of robust techniques. As former professional magicians, we propose the Swiss cheese model of deception and argue that deception should be subtle yet elaborate. Subtle deception involves techniques such as fake mistakes, planted assumptions, and convincers. Elaborate deception involves layering many of these techniques rather than relying on a single cover story. We have demonstrated the potency of these principles by making participants believe implausible ideas, such as that a machine is controlling their mind or that the placebo they consumed was a psychedelic drug. These principles can help researchers reduce demand characteristics, improve blinding, and increase the generalisability of studies that require deception.

BACKGROUND
Illness expectations are cognitive schemas, both explicit and implicit, describing how symptoms are expected to be in the future. They can be particularly relevant to disease in a mind/body framework. Asthma is a condition in which the psychological aspects can highly influence the body, but no study has directly explored these specific expectations, and no dedicated assessment tools are available.
METHODS
We developed a questionnaire to assess the illness expectations, together with an ad hoc version of the Implicit Association Test (IAT). We tested its factorial structure, and the internal and test–retest validity, recruiting a sample of 183 asthmatic people. We also explored the convergent validity and the correlations with objective and subjective clinical assessments.
RESULTS
Data suggested a three-factorial structure of the questionnaire into expectations about future symptoms, change in current health status, and rigidity of these expectations. The questionnaire showed good psychometric properties and strong associations with the other considered outcomes, including implicit expectations. The implicit evaluation, however, lacked test–retest reliability.
CONCLUSION
The questionnaire is a valid tool to assess illness expectations in people with asthma. The two expectation scales are highly related, and the implicit expectations are moderately associated with the explicit ones. The lack of stability related to IAT results may reflect a lack of stability of the implicit expectations. The implications for the mind/body framework still need to be fully explored.
