BACKGROUND
Whereas psychophysiological insomnia is the most common health complaint and has serious consequences, the neural correlates of altered perception and consciousness are insufficiently known.
AIM
We here aimed to evaluate electroencephalographic (EEG) markers of hyperarousal, cortical inhibition and attentional filtering during resting state.
METHOD
High-density EEG and electrocardiography (ECG) were recorded in up to 51 people complaining about insomnia and 43 matched controls. Analyses including source estimation quantified the spatiotemporal, spectral, and long-range temporal correlation (LRTC) properties of the EEG during resting state, including Heartbeat Evoked Potentials (HEP).
RESULTS
People that complain about insomnia showed less power in a narrow upper alpha band around 11.7 Hz over bilateral frontal and left temporal regions. They moreover showed more power in a broad beta frequency range (16.3-40 Hz) across the scalp. In addition, they showed a larger amplitude late HEP component than controls at frontal electrodes 376-500 ms after the R-wave peak in anterior cingulate/medial frontal cortices. Finally, individuals who experienced worse sleep quality had stronger LRTC.
CONCLUSIONS
The findings suggest hyperarousal, insufficient cerebral cortical inhibition and attentional filtering, and insufficient interoceptive adaptation of their brain responses to the ever-present heartbeats. The findings provide insight into the neural correlates of sleep complaints including enhanced awareness that could underlie sleep state misperception.

BACKGROUND
The mechanisms of hyperarousal, the key symptom of insomnia, have remained elusive. Recently, restless REM sleep emerged as a robust signature of insomnia. Given the role of REM sleep in emotion regulation, we hypothesized that restless REM sleep could interfere with the overnight resolution of emotional distress, thus contributing to accumulation of arousal.
AIMS
We aimed to investigate whether restless REM sleep impedes overnight distress resolution, and accumulates as hyperarousal.
METHOD
Observational and experimental studies employed psychometrics, EEG and fMRI. We first validated a proxy measure for restless REM sleep, and assessed it in a large sample along with measures of distress dissolving and hyperarousal. We then implemented an internet-protocol to induce self-conscious distress and assessed distress dissipation across 5 consecutive days. The same distress-inducing protocol was used in a MRI scanner while conditioning it to a simultaneously provided odor. The odor was again given during part of the subsequent sleep period, to induce targeted memory reactivation. A second MRI scan of distress induction evaluated how neural correlates of its impact changed overnight.
RESULTS
Of the association between restless REM sleep and hyperarousal, 62.4% was mediated specifically by reduced overnight resolution of emotional distress in insomnia. In people with insomnia, a night's sleep could even increase distress, rather than resolve it. MRI findings indicated activation of the salience network during the induction self-conscious emotions, that ameliorated with sleep but not with a similar interval of being awake.
CONCLUSION
Restless REM sleep interferes with overnight maintenance of balanced salience network activation.

Psychophysiological insomnia (PI) is one of the most frequent sleep disorders. In this study we tested whether differences in terms of neural activation are present between a group of PI patients and a healthy-control group while they are exposed to idiosyncratic ruminations and worries, evoked visually by words, so as to explore their hypothetical link with default-mode network (DMN) dysfunction in PI. We recruited five PI patients diagnosed according to the International Classification of Sleep Disorders, version 2 (ICSD-2) of American Academy of Sleep Medicine (AASM) and five age- and sex-matched healthy controls. Patients were recruited at the outpatient Sleep Medicine Centre of the Coimbra University Hospital Centre. We used a functional magnetic resonance imaging (fMRI) block-design paradigm where the participants visualized lists of words related to past/present and future concerns and also emotionally neutral words. The results suggested that the PI patients showed a failure of the DMN to deactivate. Moreover, when these patients were exposed to words concerning both past/present ruminations and future worries, there was a pronounced and significant over-recruitment of brain areas related to DMN and self-referential processing when they were compared to healthy volunteers. The differences between the patient and control groups were also evident in self-report measures. In sum, despite the relatively small sample size, our study clearly suggests that in PI there is a dysfunction in brain regions pertaining to self-referential processing, which is corroborated by an overall pattern of hyperarousal in brain regions comprising the DMN. These data may be useful in the improvement of pathophysiological models, diagnostic and therapeutic interventions for insomnia.
