ABSTRACT:
BACKGROUND AND AIM
Transcranial electrical stimulation (tES) seemS a promising technique to improve learning in young and elderly healthy subjects, but its mechanisms of action are not well understood. Here, we aim to demonstrate how tES in combination with a perceptual learning (PL) paradigm modulates cortical excitability and performance in young healthy subjects. Moreover, we want to explore behavioral and neurophysiological tES effects in PL in healthy aging to investigate modifications in cortical excitability and connectivity in the lifespan. Finally, we aim to highlight correlations between excitability, connectivity and behavioral performance.
METHOD
Healthy young (N=45) and elderly (N=36) subjects performed a PL task, before, during and after tES (1.5 mA over Oz) in three stimulation conditions: transcranial random noise stimulation - tRNS, anodal transcranial direct current stimulation - atDCS, sham. TMS-EEG coregistration preceded and followed the PL+tES.
RESULTS
In young, tRNS abolished the behavioral learning effect observed in the sham condition, while A-tDCS had no effect. In elderly, no effect of stimulation was found. The analysis of TMS evoked potentials (TEPs) showed a pre-post modulation in both the age groups (central cluster, ~90-140ms), regardless of the stimulation condition.
CONCLUSIONS
Neurophysiological results (TEPs) suggest that young and elderly have a different baseline visual system connectivity, which may influence tES effects. The presence of neuromodulation effects only in young subjects add evidences about the inadequacy of the young brain as a model to test the effect of tES, when the interest is in developing protocols of stimulation efficacious in aging.

Transcranial electrical stimulation (tES) is a potentially viable tool for boosting cognitive performance in aging. However, most knowledge on tES effects is based on studies involving young adults. Here, we applied tES (transcranial random noise stimulation [tRNS] as an effective stimulation and anodal transcranial direct current stimulation [atDCS] as a “control” stimulation) to the visual cortex during visual perceptual learning (VPL) in healthy young and older individuals. Moreover, we measured transcranial magnetic stimulation–evoked potentials to investigate the neurophysiological underpinnings of tES effects. We found that only the tRNS in the young, but not in the older, subjects modulated VPL, by decreasing performance. Transcranial magnetic stimulation–evoked potentials revealed age-related changes in connectivity, that is, a stronger activation of the prefrontal cortex after visual cortex stimulation, and a stronger modulation of the prefrontal cortex after VPL in the older subjects. These results may indicate that task performance in older adults relies on the recruitment of a wider network and a crucial contribution of the anterior portion of the brain, which may dramatically influence tES effects in aging.

Abstract/Results: ABSTRACT:
The application of anodal transcranial direct current stimulation (AtDCS) is generally associated with increased neuronal excitability and enhanced cognitive functioning. Nevertheless, previous work showed that applying this straight reasoning does not always lead to the desired results at behavioural level. Here, we investigated electrophysiological markers of AtDCS-mediated effects on visuo-spatial contextual learning (VSCL). In order to assess cortical excitability changes after 3 mA AtDCS applied over posterior parietal cortex, event-related potentials (ERPs) were collected during task performance. Additionally, AtDCS-induced effects on cortical excitability were explored by measuring TMS-evoked potentials (TEPs) collected before AtDCS, after AtDCS and after AtDCS and VSCL interaction. Behavioural results revealed that the application of AtDCS induced a reduction of VSCL. At the electrophysiological level, ERPs showed enhanced cortical response (P2 component) in the group receiving Real-AtDCS as compared to Sham-AtDCS. Cortical responsiveness at rest as measured by TEP, did not indicate any significant difference between Real- and Sham-tDCS groups, albeit a trend was present. Overall, our results suggest that AtDCS increases cortical response to incoming visuo-spatial stimuli, but with no concurrent increase in learning. Detrimental effects on behaviour could result from the interaction between AtDCS- and task-mediated cortical activation. This interaction might enhance cortical excitability and hinder normal task-related neuroplastic phenomena subtending learning.

Neurophysiological markers can overcome the limitations of behavioural assessments of Disorders of Consciousness (DoC). EEG alpha power emerged as a promising marker for DoC, although long-standing literature reported alpha power being sustained during anesthetic-induced unconsciousness, and reduced during dreaming and hallucinations. We hypothesized that EEG power suppression caused by severe anoxia could explain this conflict. Accordingly, we split DoC patients (n = 87) in postanoxic and non-postanoxic cohorts. Alpha power was suppressed only in severe postanoxia but failed to discriminate un/consciousness in other aetiologies. Furthermore, it did not generalize to an independent reference dataset (n = 65) of neurotypical, neurological, and anesthesia conditions. We then investigated EEG spatio-spectral gradients, reflecting anteriorization and slowing, as alternative markers. In non-postanoxic DoC, these features, combined in a bivariate model, reliably stratified patients and indexed consciousness, even in unresponsive patients identified as conscious by an independent neural marker (the Perturbational Complexity Index). Crucially, this model optimally generalized to the reference dataset. Overall, alpha power does not index consciousness; rather, its suppression entails diffuse cortical damage, in postanoxic patients. As an alternative, EEG spatio-spectral gradients, reflecting distinct pathophysiological mechanisms, jointly provide a robust, parsimonious, and generalizable marker of consciousness, whose clinical application may guide rehabilitation efforts.

Background
Touch is strongly related to the bodily self, forming the boundary between one’s own and others’ bodies. Previous studies investigated the relationship between touch and body ownership (the feeling that body-parts belong to us) by addressing how somatosensory experiences contribute to discriminate one’s own body from the external world. This line of research corroborates the idea that tactile awareness (the conscious experience of tactile events) contributes to the emergence of the sense of body ownership, so that “I believe this body to be mine because I perceive touch on it”.
Aims
Here, we investigated the opposite question: “Do I feel this body perceiving touch because I believe it to be mine?”.
Method
To this aim, we capitalized on the rubber hand illusion to alter the sense of body ownership and explore its role in gating tactile awareness. Hence, repeated periods of illusion induction were randomly followed by tactile events delivered either onto the fake (visual-touch) or the own (real-touch) hand.
Results
Leveraging this procedure, we demonstrate a diametrical body ownership-dependent modulation of tactile awareness at both behavioural and neurophysiological levels. Indeed, somatosensory processing (subjective perception and its neurophysiological blueprint) increases when participants observe the fake (embodied) hand while it decreases when participants receive a tactile stimulation on the real (disembodied) hand.
Conclusions
These results reveal that the brain is endowed with the inner potentiality of sensing beyond the boundaries of the own body thus experiencing phantom touches. This evidence bears potential implications for the development of neuroprosthetic devices and rehabilitative training to recover impaired somatosensation.

The integration of transcranial magnetic stimulation and electroencephalography (TMS-EEG) has been sought to explore connectivity and cortical excitability in healthy and pathological subjects. Although the number of studies in this field is continuously increasing, there is still no consensus on the pre-processing pipeline. The goal of this work is to study the effect of two preprocessing parameters, (i) the interpolation window size and (ii) the regularization parameter of the source-estimate-utilizing noise-discarding (SOUND) algorithm, on the resulting TMS-evoked potentials (TEPs) and five peaks of interest. The comparison of four combinations of parameters showed the effect of the parameters on the TMS residual artifacts, which resulted in major differences between TEPs in the early windows after the TMS pulses. The work showed how the interpolation window size and the regularization parameter influence the TEPs results and proposed a combination of parameters that retrieve information of interest, decreasing the amount of noise in data.
