Abstract/Results: ABSTRACT:
AIM
Investigate the mental states of the dying in the South of England and in Rotterdam Holland.
METHOD
One year prospective and five year retrospective questionnaire and interview study of Carers of the dying. TV, Radio and newspaper request for stories, collected and classified.
RESULTS
110 carers questioned in 3 UK and 3 Dutch Hospices and Nursing Homes. Characteristics of experiences defined – Death Bed Visions, Transiting to a new reality, Terminal Lucidity, Death Bed coincidences and many phenomena at the time of death e.g. light in the room, shapes seen leaving the body, animals upset and clocks stopping. Experiences were same in Holland and UK.
CONCLUSION
End of Life Experiences very common. Carers requested more teaching as not part of present syllabus. Very comforting to the patients. These papers stimulated more research. Early findings confirmed by other studies.

Abstract/Results: ABSTRACT:
Cases of terminal lucidity (TL) in children have recently been documented in contemporary literature, yet no studies have examined the impact on caregivers of witnessing TL in children. Using an online survey, this study assessed the impact of witnessing TL in children on seven caregivers who described a total of 11 cases of TL in children. Findings revealed that witnessing TL in children was considered a spiritual experience that influenced caregivers' belief systems. All caregivers had disclosed what they had witnessed to others, resulting in a mixture of skeptical and accepting reactions. Findings suggest witnessing TL in children may initiate a spiritual transformation that has similar implications for enhancing end-of-life care and creating 'space' for TL to be perceived as a natural part of the dying process in terminally ill children.

Terminal lucidity in children: A contemporary case collection

Abstract/Results: ABSTRACT:
he unanticipated occurrence of unusually enhanced mental clarity just before death has been reported across time and cultures and has come to be known by the term “terminal lucidity.” Cases that appear to be characteristic of terminal lucidity in children have been sporadically documented in historical and more recent literature, yet no studies have systematically examined the characteristics of terminal lucidity in children. Employing a 42-item online survey, this study collected case reports of terminal lucidity in 11 children aged 16 years and under. We recorded disease progression and treatment regime, behavioral and emotional changes prior to and during terminal lucidity, the proximity of terminal lucidity to death, and the terminal lucidity duration. Results revealed that terminal lucidity tended to occur within the final hours to minutes before death of the child and typically manifested as notable changes in mental abilities, as well as marked behavioral and emotional changes. Terminal lucidity did not seem to be precluded by any changes in medical regime and seemed to happen in spite many children being in semi- or comatose states just prior to the lucidity episode. Such results suggest a surge of mental clarity in terminally ill children does occur in spite of medical expectations that it should not, which may have implications for enhancing end-of-life care in terminally ill children, as well as for developing understandings about the nature of consciousness at the end-of-life.

Final report - Mapping the characteristics and impacts of terminal lucidity in children
Publication year: 2025
Abstract/Results:
ABSTRACT:
Background
Terminal lucidity (TL) is an unanticipated surge of mental clarity that can occur in the final hours, minutes, or seconds before death. TL has been documented in adult populations, but little is known about TL in children. This was the first known study to systematically examine TL in children.
Aims
Collect cases of TL in children aged 16 years and under from across the globe and examine whether consistent characteristics and antecedents exist across them. An additional aim was to examine the impact of witnessing TL in children on caregivers.
Methods
Seven respondents who had witnessed TL in a child were recruited through purposive sampling and completed a 42-item online survey. Some respondents reported more than one case, resulting in 11 case reports of TL in children.
Results
Differences in mental lucidity were evident before and during TL. Most children had severe mental impairment before TL yet had no or mild mental impairment during TL. Some children lost the ability to interact with their surroundings and people before TL but regained these abilities during TL. Respondents believed medication could not be responsible for the changes. Witnessing TL in children was considered a spiritual experience that influenced caregivers’ belief systems.
Conclusions
Features typical of TL identified in adult samples are also common in children’s cases. TL has important implications for understanding the process of death and for end-of-life care, including for example, whether an escalation or transition of treatment is necessary. Witnessing TL in children may initiate a spiritual transformation in caregivers, thus influencing how they deliver end-of-life care.
