Abstract/Results: ABSTRACT:
The study of client’s experience allows the research of behavior patterns therapeutically productive between therapists and clients. Knowing the importance of the psychotherapy process variables, such as therapeutic alliance and collaboration, to the psychotherapeutic context, this master thesis, exploratory in nature, aimed to characterize the evolution of client’s experience in the first four psychotherapy sessions, through the client’s perspective, and to explore the effect of therapeutic alliance and collaboration on the evolution of client’s experience in the first four sessions. The study included eight clients diagnosed with Major Depressive Disorder, treated in individual brief cognitive-behavioral psychotherapy. The therapeutic alliance was analyzed using the Working Alliance Inventory, the therapeutic collaboration with the Therapeutic Collaboration Coding System and the client’s experience through the Session Evaluation Questionnaire. Results shows that arousal progressed increasingly through the first four sessions, that the agreement on goals contributed positively and the disinterest response from the client contributed negatively to the evaluation of session depth, and that challenging intervention from the therapist affected negatively the evaluation of smoothness.

AIM:
Physiological reactivity can be modulating by cognitive and emotional processes. These processes play an important role during the first therapeutic sessions, a crucial moment to the formation and development of the collaborative interaction between therapist and client. The conceptual and methodological approach of therapeutic collaboration proposed by Ribeiro et al. (2012), considers the mutual contributions of both of the members of the therapeutic dyad and the bidirectional nature of therapeutic interaction. We also assume that these dyadic processes are translated into a physiological signature that is underlying the psychotherapy process. Our main objective is to characterize the therapeutic
collaboration and to describe the physiological processes underlying it, in the early phase of the therapeutic process of a
good outcome case.
METHOD:
The client was diagnosed with major depression using the Structured Clinical Interview for DSM Disorders. A senior therapist conducted the therapeutic process according the Cognitive Behavioral Therapy. We coded the therapeutic collaboration using the therapeutic collaboration coding system (TCCS). Both therapist and client will have their heart, and respiratory activity recorded throughout the sessions. The data coding is still in process.
RESULTS:
We expect to be able 1) to describe the collaboration processes and correlate them with the physiological patterns, 2) to identify what is happening in terms of collaboration through moments of the psychophysiological synchrony and dyssinchrony.
DISCUSSION:
Results will be discussed in terms of their implications for research and clinical practice.

Recognized the importance of the therapeutic alliance and the client and therapist experience in therapy for the therapeutic process and outcome, this master thesis aimed to understand the evolution of these variables on the client and therapist perspectives, through the therapy, and characterize the therapeutic collaboration in a micro level, in a poor outcome case. We used the Working Alliance Inventory to analyse the therapeutic alliance, the Therapeutic Collaboration Coding System to analyse the therapeutic collaboration, and the Session Evaluation Questionnaire to analyse the experience in therapy. Results suggested a discrepancy between the client and therapist perspectives on the evaluation of the therapeutic alliance and the experience in therapy. Results also indicated that the therapeutic interaction occurred mostly within the client’s Therapeutic Zone of Proximal Development, with a high frequency of challenge interventions and safety responses. Finally, in client's perspective, sessions with high scores in therapeutic alliance were simultaneously evaluated as deep, smooth and positive. The interactions within the Therapeutic Zone of Proximal Development with the client beyond the level proposed by the therapist only occurred in these types of sessions.

This research aimed to understand how therapeutic collaboration is characterized in the significant events that are useful for the change, identified by the perspective of the client and the therapist, throughout a clinical case considered as improved but not clinically significant. A case study methodology was used with a client with major depressive diagnosis and the intervention followed by the cognitive-behavioral psychotherapeutic model. All therapeutic sessions were audio-recorded and transcribed and subsequently coded according to the Therapeutic Collaboration Coding System. At the end of each session, the portuguese version of the Helpful Aspects of Therapy questionnaire was also administered to both participants to collect significant change events. The data was later analyzed using the State Space Grid method in order to describe which interactive episodes were the attractors. Client-therapist synchrony has been proposed as a crucial process in forming and developing the therapeutic alliance. However, research in the electrodermal activity synchrony area and its relationship with the therapeutic alliance has proved to be scarce. The present study aimed to analyze the association between electrodermal activity synchrony and the quality of the therapeutic alliance, throughout the therapeutic process. The study included 15 therapeutic dyads, consisting of 15 clients with a diagnosis of Major Depressive Disorder and 5 therapists. For each dyad, electrodermal activity was recorded throughout the therapeutic session and information about the therapeutic alliance was collected at the end of the therapeutic session, using the Working Alliance Inventory - Short Revised and the Working Alliance Inventory Short-Revised Form: Therapist's version, for the client and the therapist, respectively. This process was carried out over 16 sessions of Cognitive-Behavioral Therapy. The results obtained demonstrated the existence of in-phase electrodermal activity synchrony in the therapeutic dyads. Nevertheless, this synchrony was not associated with the evaluations of the therapeutic alliance, conducted by both the client and the therapist, in terms of overall scores or the different components (i.e., “bond”, “goals”, and “tasks”) of the therapeutic alliance.

Heart rate variability has been associated with several psychological conditions, including Major Depressive Disorder and Social Anxiety Disorder. The study of this association seems to bring important conclusions to psychology and psychotherapists. The aim of the present study was to analyze the relationship between these variables in psychotherapy sessions, as well as the association between heart rate variability and therapeutic alliance. The study included 42 participants (21 subjects in the experimental group with Major Depressive Disorder or Social Anxiety Disorder and 21 subjects in the control group). The Outcome Questionnaire 45.2 and the Working Alliance Inventory – Short Revised were used for data collection, and the cardiac activity from the experimental group was also recorded in order to analyze its variability. An analysis of comparison of means and a mixed generalized linear model were used, with the goal of making comparisons between the groups and verifying the evolution during the therapy, respectively. The results showed that there are differences in heart rate variability between groups but did not show a linear association between this variability and its association with symptomatology and therapeutic alliance.

Therapeutic Alliance (TA) has been consistently reported as a robust predictor of therapy outcomes and is one of the most investigated therapy relational factors. Research on therapists' and clients' contributions to the alliance development and the alliance-outcome relationship had shown mixed results. The relation of the therapist's and client's biological markers with the alliance is an important and under-investigated topic. Taking advantage of data mining techniques, this exploratory study aimed to investigate the role of different therapist and client factors, including heart rate (HR) and electrodermal activity (EDA), in relation to TA. Twenty-two dyads with 6 therapists and 22 clients participated in the study. The Working Alliance Inventory (WAI) was used to evaluate the client's and therapist's perception of the alliance at the end of each session and through the therapy processes. The Cross-Industry Standard Process for Data Mining (CRISP-DM) was used to explore patterns that may contribute to TA. Machine Learning (ML) models have been employed to provide insights into the predictors and correlates of TA. Our results showed that Linear Regression (LR) was the best technique for predicting the therapist's TA, with client "Diagnostic" and therapy "Termination" being identified as significant predictors of the therapist's TA. In addition, for clients' TA, the Random Forest (RF) was shown to have the best performance. The therapist's TA and therapy "Outcome" were observed as the most influential predictors for the client's TA. In addition, while the Heart Rate (therapist) was negatively associated with the therapist's TA, EDA in the client was a physiological indicator related to the client's TA. Overall, these findings can assist in identifying key factors that therapists should focus on to enhance the quality of therapeutic alliance. Results are discussed in terms of their consistency with empirical literature, innovative and interdisciplinary research on the therapeutic alliance field, and, in particular, the use of the Data Mining approach in a psychotherapy context.
