There is accumulating evidence from different methodological approaches that the placebo effect is a neurobiological phenomenon. Behavioral, psychophysiological, and neuroimaging results have largely contributed to accepting the placebo response as real. A major aspect of recent and future advances in placebo research is to demonstrate linkages between behavior, brain, and bodily responses. This article provides an overview of the processes involved in the formation of placebo responses by combining research findings from behavioral, psychophysiological, and neuroimaging methods. The integration of these different methodological approaches is a key objective, motivating our scientific pursuits toward a placebo research that can inform and guide important future scientific knowledge.

Abstract/Results: ABSTRACT:
The present study tested the impact of an acoustic prepulse on pain caused by an electrical shock to the arm. If pain can be inhibited by a prepulse, then prepulses might be added to diagnostic and therapeutic medical interventions in which pain is unavoidable in order to decrease patient discomfort and increase patient compliance. College students had shocks (150V, 0.5ms duration) delivered to the upper arm.On some trials this shockwas preceded by a 40ms broadband noise at 80 dB, with a lead interval of 80ms.Midway through the 16-trial session, participants were told that the prepulse would either increase or decrease the pain of the shock (N520 per instruction group). The only difference between Blocks 1 and 2 were these instructions; the actual prepulses were not changed. Participants rated the pain and unpleasantness of each shock on a 100mm visual analog scale (a separate sheet for each trial). Eyeblink EMG was measured from orbicularis oculi ipsilateral to the shocked arm. Rated pain in the shock-alone condition increased from Block 1 to Block 2 by 38%. In Block 1, the prepulse increased pain by 19%. In Block 2, the prepulse increased pain by 6% in the Increase group, and decreased pain by 5% in the Decrease group, relative to the control condition in Block 2. EMG activity elicited by the shock was not affected by the prepulse. These data show that an acoustic prepulse did not inhibit the pain of an intense shock to the arm, perhaps because the timing between the two stimuli was too long. A second study with a shorter lead interval will also be reported.

Abstract/Results: ABSTRACT:
There is accumulating evidence from different fields of mechanistic placebo research that placebo effects result from real neurobiological processes. This minisymposium highlights the most important parts of the puzzle of the placebo effect and reviews these at different levels: neurophysiology, neuroanatomy, autonomic activity, behavior, and cognition. Speakers will emphasize the role of verbal suggestion, learning, empathy, and negative emotions in placebo responses.
Placebo effects refer to the reduction in symptoms after information that an effective treatment has been administrated, even if the treatment is inactive. L. Colloca will show that placebo effects may be learned via classical conditioning and that a placebo response can also follow the observation of another person who demonstrates pain relief in response to a treatment. K. Meissner will present evidence that placebo interventions can affect peripheral organ functions regulated by the autonomic nervous system, such as gastric motility, lung function, blood pressure, and the diameter of coronary arteries. Such placebo responses can be rather specific in nature and reflect the highly specific organization of autonomic pathways in the brain. M. Flaten will discuss whether placebo analgesia could be due to reduction in stress and negative emotions, which reduce feelings of pain. Event related potentials (ERPs) to painful stimulation are reduced by information that a painkiller has been administrated, and the reduction in ERPs is related to reduction in feelings of stress. A. Watson will present evidence that placebos reduce anticipatory brain (EEG) responses to painful stimuli, and that personality traits such as dispositional optimism and low state anxiety appear to be significant predictors of placebo responses. U. Bingel will provide evidence from fMRI studies that placebo analgesia involves top-down activation of endogenous analgesic activity via the descending modulatory system. The additional contribution of emotional regulation processes and areas such as the limbic system is a fundamental step that needs to be further addressed. T. Wager will present studies showing that placebo treatments elicit endogenous opioid release, engage frontal cortical-brainstem circuits during the anticipation of pain that predict the magnitude of placebo analgesia in new individuals, and reduce pain-related fMRI responses. These findings are replicable across laboratories and methods (PET, fMRI, EEG), are independent of attention and distraction effects, and exist even during active drug administration.

The hypothesis put forth is that expectations of treatment effects reduce negative emotions and thereby reduce symptoms, e.g. pain. Negative emotions increase pain, and it is hypothesized that placebos reduce pain by reducing negative emotions, i.e. feelings of nervousness, fear and anxiety. Placebo analgesia has been shown to be mediated via opioid activity, and relaxation increases opioid activity. The placebo acquires its relaxing effect due to verbal information that pain will be reduced, or due to associations between the placebo and the reduction in pain after effective treatment. Thus, the placebo signals that unpleasantness will be less after administration of the placebo. This involves negative reinforcement which is due to activation of a dopaminergic system that has been found to be activated during placebo analgesia and is involved in positive emotions. The nocebo effect of increased pain is, consistent with this model, because of increased fear and anxiety. The new aspect of the presented model is the hypothesis that expectations reduce negative emotions, and that negative reinforcement that involves the dopaminergic reinforcement system should be a contributor to placebo responses.

Fear of pain (FOP) and its effect on placebo analgesia was investigated. It was hypothesized that FOP should interfere with placebo-mediated pain inhibition and result in weaker placebo responding in pain intensity, pain unpleasantness, stress, and event-related potentials to contact heat pain. Thirty-three subjects participated in a balanced 2 condition (natural history, placebo)×3 test (pretest, posttest 1, posttest 2) within-subject design, tested on 2 separate days. FOP was measured by the Fear of Pain Questionnaire and subjective stress by the Short Adjective Check List. Placebo effects were found on reported pain unpleasantness and N2 and P2 amplitudes. FOP was related to reduced placebo responding in pain unpleasantness, but this was only evident for the subjects who received the placebo condition on day 1. Subjects who received the placebo condition on day 1 experienced more pretest stress than those who received the placebo condition on day 2 (ie, reversed condition order), and this explained the interaction effect on placebo responding. FOP was related to reduced placebo responding on P2 amplitude, whereas placebo responding on N2 amplitude was unaffected by FOP. Higher placebo responses on N2 and P2 amplitudes were both related to higher placebo analgesic magnitude in pain unpleasantness. In conclusion, increased FOP was found to reduce subjective and electrophysiological placebo analgesic responses.

Fear was induced by the anticipation of electric shock in order to investigate whether fear reduced the effectiveness of a placebo intervention on reported pain and the acoustic startle reflex. Thirty-three subjects participated in a 3 Condition (Natural History [NH], Placebo [P], Placebo + Fear [PF]) x 3 Test (Pretest, Posttest 1, Posttest 2) within-subject design, tested on 3 separate days. Measures of fear were fear of pain (FOP), measured by the Fear of Pain Questionnaire (FPQ-III); fear-potentiated startle; and a self-report measure that assessed the effectiveness of the fear induction procedure. In the pain intensity data, there was a trend towards a placebo effect. This trend was abolished by induced fear, and was most pronounced in subjects who were highest in measures of fear. The placebo manipulation also caused a reduction in startle reflex amplitude. This effect was abolished by induced fear, and was strongest amongst high FOP subjects. In conclusion, induced fear abolished placebo analgesia, and this effect was strongest in subjects who had high scores on measures of fear.

Abstract/Results: ABSTRACT:
The placebo response mimics the effects of drugs and provides relief from e.g. pain, when information suggesting pain relief is provided. However, under some circumstances, drug-antagonistic reactions are observed to signals that a painkiller will be administered. The prediction of the type of response to be observed, agonistic or antagonistic to the drug treatment, has proven difficult. Several theoretical models have been proposed to solve this problem. This chapter presents some of these models and the empirical findings supporting and not supporting them. It is concluded that a model in which the state of the organism at the time of drug presentation is taken into account may explain why sometimes drug antagonistic and sometimes drug agonistic responses are observed.

Studies of placebo analgesia necessarily involve the induction and reporting of pain. The pain report is the basic dependent variable in many studies of placebo analgesia, and reported pain should ideally reflect the pain experience. However, the pain report is subject to a number of different influences that threaten the internal validity of research on pain and, consequently, placebo analgesia. The study of placebo analgesia introduces several other issues, in terms of the design of studies that researchers must deal with. Many methodologic issues have been solved, but some important issues are still unresolved. The concept of expectation is central to studies of placebo effects, and poses special challenges in terms of its conceptual status and its measurement.

The BIAL Foundation was created in 1994 by Laboratórios BIAL in conjunction with the Council of Rectors of Portuguese Universities. BIAL’s Foundation mission is to foster the scientific study of Man from both the physical and spiritual perspectives.
Along the years the BIAL Foundation has developed an important relationship with the scientific community, first in Portugal and after worldwide. Today it is an institution of reference which aims to stimulate new researches that may help people, promote more health and contribute to new milestones to gain access to knowledge.
Among its activities the BIAL Foundation manages the BIAL Award, created in 1984, one of the most important awards in the Health field in Europe. The BIAL Award rewards both the basic and the clinical research distinguishing works of major impact in medical research.
The BIAL Foundation also assigns Scientific Research Scholarships for the study of neurophysiological and mental health in people, arousing the interest of researchers in the areas of Psychophysiology and Parapsychology.
To date the BIAL Foundation has supported 461 projects, more than 1000 researchers, with research groups in twenty-seven countries, resulting, until April 2013, in about 600 full papers, out of which 172 published in indexed international journals with an average impact factor of 3.6 and a substantial number of citations (1665).

BACKGROUND:
Information about the effectiveness of treatment has been found to reduce pain, termed placebo analgesia. It was investigated whether information that suggested that minimal pain would be experienced during filling therapy, could reduce the pain associated with that procedure. All patients received the same amount of
anaesthesia and similar procedures during filling therapy. It was predicted that extended information should reduce pain, and that the reduction in pain should be associated with a reduction in stress.
METHODS:
44 patients (24 females, 17-68 yrs) going through composite restoration therapy with anaesthesia in a molar or premolar, participated. The design was a 2 Information (Extended, Standard) by 4 Test mixed design. Patients were randomized to two groups: The Standard Info group (N=22) received information that was short but informative about the pain they might experience under and after treatment. Words like pain, sensitivity and unpleasant were used. The Extended Info group received information that and focused on the
positive effects. The Fear of Pain Questionnaire III was administrated. Subjective pain intensity and pain unpleasantness, and stress were recorded by numerical rating scales before, during, immediately after,
and 15 minutes after treatment. The patients were all given local anaesthesia in the form of xylocain dental adrenalin 2% + 12.5 microg/ml adrenalin.
RESULTS:
Pain was 2.6 points lower in the Extended Info group compared to the Standard Info group during treatment (p = .033). There were no differences in pain levels in the other Tests. Extended Info reduced stress slightly. Low tension before treatment correlated with lower pain during treatment (p = .004). High fear of pain correlated with high stress prior to treatment (p = .002).
CONCLUSIONS:
Providing positive information about the anaesthetic and the dental procedure reduced pain compared to standard information. Subjects high in fear of pain displayed the highest stress prior to the dental procedure, and high stress prior to the dental procedure correlated with high pain during the procedure. The information partly reduced pain by reducing stress prior to the dental procedure. The magnitude of the analgesic effect of the information was about 2.6 points on an 11-point numerical rating scale, clinically significant. Taken together the study shows that providing information that underscores the effectiveness of the painkiller, informs the patient that the damage is small, and that informs the patient that he/she has control of the situation, reduces pain induced by the dental procedure.

OBJECTIVE:
Chronic pain is believed to be related to a dysfunction of descending pain modulatory mechanisms. Functioning of descending pain modulation can be assessed by various methods, including conditioned pain modulation (CPM). CPM refers to the inhibition of one source of pain by a second noxious stimulus, termed the conditioning stimulus. This procedure can activate an endogenous pain inhibitory mechanism that inhibits early nociceptive processing. Chronic pain and anxiety disorders are more prevalent among females and it has been hypothesized that females react with more negative emotions towards unpleasant stimuli and this might be part of the explanation of greater pain sensitivity in females. The present study investigated whether expectations modulate the effect of conditioning stimulation on pain, subjective stress, and heart rate. In addition, we investigated whether the modulation of CPM by expectations differed between males and females.
METHODS:
Seventy-two subjects (including 36 women) received six noxious heat stimuli to the forearm. During three of these stimuli, a conditioning stimulus (cold-water bath) was applied to the contralateral arm in order to activate CPM. One third of the subjects were told that this would reduce pain (analgesia group), one-third that it would increase pain (hyperalgesia group), and one third received no information about its effect (no info group).
RESULTS:
Information that conditioning stimulation decreased or enhanced pain had the corresponding effect in females, but not in males. Conditioning stimulation increased stress, but not heart rate in females in the hyperalgesia group. A higher expectation of analgesia and lower stress during conditioning stimulation was associated with larger inhibitory CPM.
CONCLUSION:
These results suggest that reduced inhibitory CPM can be due to contextually induced cognitive and emotional factors and not necessarily a dysfunction of descending inhibitory pathways.

Four studies investigated whether negative emotions (stress, anxiety, fear) reduced or abolished placebo analgesia. Several studies have shown less placebo analgesia in females compared to males, which could be due to more negative emotions in females. Study 1 showed that higher fear of pain reduced subjective and electrophysiological placebo analgesic responses. Sensation of pain can be inhibited by applying a second noxious stimulus elsewhere on the body. This procedure activates endogenous pain inhibitory mechanisms. Study 2 investigated whether expectations modulated this effect on tonic pain. Negative information about the effect of conditioning stimulation induced stress in females, and higher fear of pain was associated with increased stress during conditioning stimulation in females. In Study 3 fear was induced by the anticipation of electric shock to investigate whether fear reduced placebo analgesia. Induced fear abolished placebo analgesia, and this was strongest in subjects with high scores on measures of fear. Study 4 used functional resonance imaging (fMRI). It was hypothesized that differences in placebo responding between males and females should be correlated with larger placebo-related neural responses in the thalamus, the ACC and the insular cortex in males compared to females. During anticipation of pain, females had higher activation in the left cerebellum and the left hippocampus compared to males, areas that in previous research have been implicated in emotional processing. In sum, the four studies indicate that placebo analgesia is stronger in males than in females. This is due to more fear and anxiety in females in anticipation of the painful stimulation.

Background
Pain induced by electrical stimuli has been found in previous research to be reduced by brief, weak electrical pulses, termed prepulses, presented 40 to 60 ms prior to the painful electrical stimulus.
Methods
The present experiment investigated the generality of this effect by presenting weak acoustic stimuli simultaneously with, or 80 or 1000 ms prior to, painful electric shocks. In the second half of the experimental session, each participant (N = 119) was told that the acoustic stimuli would either increase or decrease the pain induced by the electric shock, to investigate automatic and controlled cognitive processes in the modulation of pain.
Results
Acoustic stimuli presented simultaneously with painful stimulation increased pain slightly (4 mm on a 100 mm scale). Acoustic stimuli presented 80 and 1000 ms prior to painful stimuli had no effect on pain. Information that acoustic stimuli would increase pain did so in females, but only when the acoustic stimulus was presented 80 ms prior to the painful stimulus.
Conclusions
The effect of the acoustic stimuli and of information was weak. Failure to replicate previous findings of decreased pain by weak prepulses was most likely due to the sensory modality of the prepulse stimuli. It is recommended that further studies of pain modulation by brief stimulation use electrical and not acoustic prepulse stimuli.

The BIAL Foundation was created in 1994 by Laboratórios BIAL in conjunction with the Council of Rectors of Portuguese Universities. BIAL’s Foundation mission is to foster the scientific study of Man from both the physical and spiritual perspectives.
Along the years the BIAL Foundation has developed an important relationship with the scientific community, first in Portugal and after worldwide. Today it is an institution of reference which aims to stimulate new researches that may help people, promote more health and contribute to new milestones to gain access to knowledge.
Among its activities the BIAL Foundation manages the BIAL Award, created in 1984, one of the most important awards in the Health field in Europe. The BIAL Award rewards both the basic and the clinical research distinguishing works of major impact in medical research.
The BIAL Foundation also assigns Scientific Research Scholarships for the study of neurophysiological and mental health in people, arousing the interest of researchers in the areas of Psychophysiology and Parapsychology.
To date the BIAL Foundation has supported 461 projects, more than 1000 researchers, with research groups in twenty-seven countries, resulting, until April 2013, in about 600 full papers, out of which 172 published in indexed international journals with an average impact factor of 3.6 and a substantial number of citations (1665).
Since 1996 the BIAL Foundation organizes the Symposia entitled "Behind and Beyond the Brain", a Forum that gathers well renowned neurosciences speakers and the BIAL Foundation Fellows which are spread around the world.
Classified as an institution of public utility, the BIAL Foundation includes among its patrons the Portuguese President, the Portuguese Universities Rectors' Council and the Portuguese Medical Association.

In 1994 the BIAL Foundation launched a programme of science research grants with the aim of encouraging the research into Man’s physical and mental processes, namely in fields still largely unexplored but which warrant further scientific analysis, as Psychophysiology and Parapsychology.
Since its launch, applications to the BIAL grants have been increasing. Up to now 461 projects have been supported, involving more than 1000 researchers from 27 countries.
The approved applications have benefited from grants in amounts comprised between €5,000 and €50, 000. The amount to be granted is fixed by the Scientific board according to the needs of each project.
The supported projects have originated, until April 2013, in about 600 full papers, 172 out of which were published in indexed international journals with an average impact factor of 3.6 and a substantial number of citations (1665).
Among the BIAL Foundation fellows is worth highlighting the presence of scientists from prestigious universities from the United States, United Kingdom, Australia, Russia, Germany, Japan, France, Canada, and many others.
The BIAL grants are promoted biannually.

Abstract/Results: ABSTRACT:
Individuals undergoing treatment for a symptom like pain expect that the treatment will reduce the pain. Many studies show that healthy volunteers or patients in pain report less pain after inactive treatment, if they believe that active medication has been administrated. The reduction of pain can be partly blocked by systemic administration of naloxone, an opioid antagonist. There is reduced central nervous system activation to painful stimuli in individuals who have been given a placebo and told it is a painkiller. These findings suggest that the expectation of pain relief generates central nervous system opioid activity that inhibits pain transmission to the cerebral cortex. Expectations may thus lead to changes in central nervous system activity that reduces pain. It is proposed that expectations activate a homeostatic system that corrects perturbations to the system via negative feedback. The nocebo effect is the opposite of the placebo effect, and is due to induction of negative emotions. Part of the treatment of many symptoms and diseases is due to autonomic adjustments controlled by the central nervous system. The involvement of emotional processes in placebo effects could have important consequences for interpretation of data from randomized controlled trials.

Abstract/Results: ABSTRACT:
OBJECTIVES
The present review investigated whether there are systematic sex differences in the placebo and the nocebo effect.
METHODS
A literature search was conducted in multiple electronic databases. Studies were included if the study compared a group or condition where a placebo was administered to a natural history group or similar cohort.
RESULTS
Eighteen studies were identified – 12 on placebo effects and 6 on nocebo effects. Chi-square tests revealed that 1) males responded more strongly to placebo treatment, and females responded more strongly to nocebo treatment, and 2) males responded with larger placebo effects induced by verbal information, and females responded with larger nocebo effects induced by conditioning procedures.
CONCLUSION
This review indicates that there are sex differences in the placebo and nocebo effects, probably caused by sex differences in stress, anxiety, and the endogenous opioid system.
