Sleep in normal adults is characterised by highly consistent state-transitions in the brain over time. Compared to the descent to sleep, which is, at least partly, a voluntary act, the switching between sleep stages appears almost mechanistic. The temporal order and relationship between the brain states of various sleep stages are remarkably constant. Describing the whole-brain activity of individual sleep stages was one of the first merits of electroencephalography (EEG), and more advanced forms of neuroimaging have expanded our understanding of the spatiotemporal unfolding of sleep. Yet, the mechanisms underlying, and brain regions orchestrating the transitions between wakefulness and the various sleep states remain unresolved. Understanding this may lead to important insights into not only the fundamental principles of the human brain function but also the causes of sleep disorders. Viewing the brain as an intricately connected network, in which activity occurs as a result of communication between parts of this network has helped the investigation of spontaneous brain activity. By combining analysis of structural imaging data, such as diffusion tensor imaging (DTI), and functional imaging data, such as functional Magnetic Resonance Imaging and magnetoencephalography (MEG), computational modelling has successfully been applied to describe how spontaneous dynamics can arise from the structural properties of the network. Modelling of whole-brain activity can assist in elucidating the causal links facilitating the transitions between brain states of sleep. In computational terms the aim is to understand the interplay between integration and segregation in the brain and to find the important binding regions that are necessary and sufficient for network transitions between states. In the current study we used MEG to measure whole-brain activity of 11 healthy adults that went through the different phases of sleep. We obtained the spatiotemporal dynamics of brain activity by extracting the slow fluctuating changes in the Hilbert power envelope of frequency filtered and beamformed time-series. A Hidden Markov Model (HMM) makes it possible to resolve non-stationarity of functional networks. Thus each sleep stage was tested as individual transient states of the network. Finally, we applied a whole-brain computational model that allowed us to identify the necessary and sufficient brain regions binding information across the brain and facilitating the transitions between brain states during sleep.

During non-REM sleep, neuronal firing is grouped by the slow oscillations at around 1 Hz, and sleep spindles, 0.5-2s long bursts of oscillatory activity between 11 and 18 Hz. Spindles are thought to arise from the interplay between thalamic and cortical neurons and are hypothesized to be involved in a wide variety of fundamental processes including the solidification of learning during sleep. However many of the physiological mechanisms and characteristics of spindles remain poorly understood. The spatial extent of spindle activity, as one key feature, has been the object of substantial debate, with conflicting evidence pointing both to an increase in synchronization, assessed with scalp EEG, and to strong regional localization, as measured by MEG and intraparenchymal depth electrode recordings (iEEG). Here, we examine spindle localization and synchrony across broad areas of neocortex using recordings from, on average, 80 electrodes (ECoG) placed directly on the pial surface in six patients undergoing evaluation for intractable epilepsy. The research was approved by the local institutional review board and electrode placement was determined solely by clinical criteria. At least one hour of visually scored sleep was included in the analysis. Spindles were automatically detected on each electrode independently, based on custom code. Spindles were considered synchronous when two spindles on separate channels overlapped in time. We observed that spindles were more likely than expected by chance to appear at the same time over multiple cortical locations. However, when we estimated the distribution of spindles in relation to the number of active electrodes, we observed that there was a strong positive skew. This indicates that most spindles were synchronous only in one or a few channels at the same time but some spindles were widely distributed. This distinction between focal spindles and distributed spindles was further investigated by measuring the spreading over cortical regions with a 3D reconstruction of the electrode locations and by comparing it with the underlying anatomical structures. These results indicate a dynamic interplay between local properties of focal spindles and widespread synchrony of global spindles. We suggest that the multiple spatial scales at which sleep oscillations operate might provide a flexible framework to support the many functions ascribed to sleep and to spindles in particular, such as sensory gating, memory consolidation, and synaptic homeostasis.

The slow waves of sleep arise from the alternation of periods of relative neuronal silence and periods of high neuronal activity. Intriguingly, slow waves travel preferentially from anterior to posterior cortical regions. It has been proposed that phasic activity of subcortical regions that modulate cortical excitability may be involved in state transitions. The neuronal mechanisms contributing to the directional preference of traveling however remain elusive. Since the active phase of the cortical slow wave can be preceded by phasic activity in the locus coeruleus (LC), we hypothesized that the LC, due to the anteroposterior gradient of the length and consequently axonal propagation duration of its cortically projecting fibers, might depolarize frontal cortical neurons slightly earlier than more posterior neurons and thus prime the cortex for a preferential propagation of slow waves along the anterior-to-posterior axis. High-density electroencephalography (EEG) and functional Magnetic Resonance Imaging (fMRI) were recorded in 14 participants while sleeping. Slow waves were automatically detected and classified according to their direction of propagation on the scalp as traveling either in the anterior-to-posterior or posterior-to-anterior direction. Statistical parameter mapping (SPM) analysis was used to evaluate to which extent these two event types were preceded by phasic activation in the brainstem. A brainstem area compatible with the right LC was significantly activated in association with slow waves traveling in the anterior-to-posterior direction, but not significantly with slow waves traveling in the opposite, posterior-to-anterior, direction. Phasic activity in the LC may thus contribute to the bias of slow waves to travel in an anterior-to-posterior direction.

Brain activity is the result of a dynamic interplay between cortical and thalamic neuronal populations and this often takes the shape of an oscillation. A characteristic phenomenon that is generated by the thalamocortical network during sleep is the spindle, a 0.5-2 s long burst of oscillations between 11 and 16 Hz. While animal and computational studies have highlighted the importance of the reciprocal connections between cortex and thalamus, the spatio-temporal properties of the spindle on the cortex remain largely unexplored. We here investigated the evolution of the spindle oscillations over large cortical areas, measured on the human neocortex. Spindles were described by their instantaneous amplitude and phase over neighboring electrodes. We collected recordings from NREM sleep stage 2, using electrocorticography in eight patients undergoing evaluation for intractable epilepsy. Electrodes covered large parts of one hemisphere and were spaced 1 cm apart. The research was approved by the local institutional review board and electrode placement was determined solely by clinical criteria. Based on the instantaneous amplitude in the spindle frequency band for each electrode, we computed the covariance on overlapping 1s long intervals of these time-series. We observed recurring patterns of spatio-temporal organization, which include focal, widespread and global spindling activities. Spindles were more likely to synchronize over multiple cortical areas if they included the lateral prefrontal cortex, a putative pivot of spindle activity. In addition, we observed a drastic phase reset at the beginning of the sleep spindle. We interpret this phase reset as the putative thalamic input to the cortical mantle. The spatial extent of this phenomenon was compatible with the two anatomically defined thalamocortical pathways: the spatially diffuse matrix pathway and the locally selective core pathway. These results highlight the importance of thalamocortical projections in shaping the spindle evolution and define the spatial extent of the functional influence of these projections. These findings will help elucidate the mechanisms underlying spindle generation and the functional role of thalamocortical loops, their possible role in sleep maintenance and memory, and their disruption in neurological disorders.

Sleep spindles arise from the interaction of thalamic and cortical neurons. Neurons in the thalamic reticular nucleus (TRN) inhibit
thalamocortical neurons, which in turn excite the TRN and cortical neurons. A fundamental principle of anatomical organization
of the thalamocortical projections is the presence of two pathways: the diffuse matrix pathway and the spatially selective core
pathway. Cortical layers are differentially targeted by these two pathways with matrix projections synapsing in superficial layers
and core projections impinging on middle layers. Based on this anatomical observation, we propose that spindles can be classified
into two classes, those arising from the core pathway and those arising from the matrix pathway, although this does not exclude the fact that some spindles might combine both pathways at the same time.We find evidence for this hypothesis in EEG/MEG studies, intracranial recordings, and computational models that incorporate this difference.This distinction will prove useful in accounting for the multiple functions attributed to spindles, in that spindles of different types might act on local and widespread spatial scales. Because spindle mechanisms are often hijacked in epilepsy and schizophrenia, the classification proposed in this review might provide valuable information in defining which pathways have gone awry in these neurological disorders.

Since their discovery almost one century ago, sleep spindles, 0.5–2 s long bursts of oscillatory activity at 9–16 Hz during NREM sleep, have been thought to be global and relatively uniform throughout the cortex. Recent work, however, has brought this concept into question but it remains unclear to what degree spindles are global or local and if their properties are uniform or location-dependent. We addressed this question by recording sleep in eight patients undergoing evaluation for epilepsy with intracranial electrocorticography, which combines high spatial resolution with extensive cortical coverage.
We find that spindle characteristics are not uniform but are strongly influenced by the underlying cortical regions, particularly for spindle density and fundamental frequency. We observe both highly isolated and spatially distributed spindles, but in highly skewed proportions: while most spindles are restricted to one or very few recording channels at any given time, there are spindles that occur over widespread areas, often involving lateral prefrontal cortices and superior temporal gyri. Their co-occurrence is affected by a subtle but significant propagation of spindles from the superior prefrontal regions and the temporal cortices towards the orbitofrontal cortex.
This work provides a brain-wide characterization of sleep spindles as mostly local graphoelements with heterogeneous characteristics that depend on the underlying cortical area. We propose that the combination of local characteristics and global organization reflects the dual properties of the thalamo-cortical generators and provides a flexible framework to support the many functions ascribed to sleep in general and spindles specifically.

During sleep, the thalamus generates a characteristic pattern of transient, 11-15 Hz sleep spindle oscillations, which synchronize the cortex through large-scale thalamocortical loops. Spindles have been increasingly demonstrated to be critical for sleep-dependent consolidation of memory, but the specific neural mechanism for this process remains unclear. We show here that cortical spindles are spatiotemporally organized into circular wave-like patterns, organizing neuronal activity over tens of milliseconds, within the timescale for storing memories in large-scale networks across the cortex via spike-time dependent plasticity. These circular patterns repeat over hours of sleep with millisecond temporal precision, allowing reinforcement of the activity patterns through hundreds of reverberations. These results provide a novel mechanistic account for how global sleep oscillations and synaptic plasticity could strengthen networks distributed across the cortex to store coherent and integrated memories.

Alpha power predicts persistence of bistable perception

Perception is strongly affected by the intrinsic state of the brain, which controls the propensity to either maintain a particular perceptual interpretation or switch to another. To understand the mechanisms underlying the spontaneous drive of the brain to explore alternative interpretations of unchanging stimuli, we repeatedly recorded high-density EEG after normal sleep and after sleep deprivation while participants observed a Necker cube image and reported the durations of the alternating representations of their bistable perception. We found that local alpha power around the parieto-occipital sulcus within the first second after the emergence of a perceptual representation predicted the fate of its duration. An experimentally induced increase in alpha power by means of sleep deprivation increased the average duration of individual representations. Taken together, these findings show that high alpha power promotes the stability of a perceptual representation and suppresses switching to the alternative. The observations support the hypothesis that synchronization of alpha oscillations across a wide neuronal network promotes the maintenance and stabilization of its current perceptual representation. Elevated alpha power could also be key to the poorly understood cognitive deficits, that typically accompany sleep deprivation, such as the loss of mental flexibility and lapses of responsiveness.

Ictal and preictal power changes outside of the seizure focus correlate with seizure generalization

OBJECTIVE:
The treatment of focal epilepsies is largely predicated on the concept that there is a "focus" from which the seizure emanates. Yet, the physiological context that determines if and how ictal activity starts and propagates remains poorly understood. To delineate these phenomena more completely, we studied activity outside the seizure-onset zone prior to and during seizure initiation.
METHODS:
Stereotactic depth electrodes were implanted in 17 patients with longstanding pharmacoresistant epilepsy for lateralization and localization of the seizure-onset zone. Only seizures with focal onset in mesial temporal structures were used for analysis. Spectral analyses were used to quantify changes in delta, theta, alpha, beta, gamma, and high gamma frequency power, in regions inside and outside the area of seizure onset during both preictal and seizure initiation periods.
RESULTS:
In the 78 seizures examined, an average of 9.26% of the electrode contacts outside of the seizure focus demonstrated changes in power at seizure onset. Of interest, seizures that were secondarily generalized, on average, showed power changes in a greater number of extrafocus electrode contacts at seizure onset (16.7%) compared to seizures that remained focal (3.8%). The majority of these extrafocus changes occupied the delta and theta bands in electrodes placed in the ipsilateral, lateral temporal lobe. Preictally, we observed extrafocal high-frequency power decrements, which also correlated with seizure spread.
SIGNIFICANCE:
This widespread activity at and prior to the seizure-onset time further extends the notion of the ictogenic focus and its relationship to seizure spread. Further understanding of these extrafocus, periictal changes might help identify the neuronal dynamics underlying the initiation of seizures and how therapies can be devised to control seizure activity.

Small vessel disease and cognitive impairment: The relevance of central network connections

Central brain network connections greatly contribute to overall network efficiency. Here we examined whether small vessel disease (SVD) related white matter alterations in central brain network connections have a greater impact on executive functioning than alterations in non-central brain network connections. Brain networks were reconstructed from diffusion-weighted MRI scans in 72 individuals (75 ± 8 years) with cognitive impairment and SVD on MRI. The centrality of white matter connections in the network was defined using graph theory. The association between the fractional anisotropy (FA) of central versus non-central connections, executive functioning, and markers of SVD was evaluated with linear regression and mediation analysis. Lower FA in central network connections was more strongly associated with impairment in executive functioning than FA in non-central network connections (r = 0.41 vs. r = 0.27; P < 0.05). Results were consistent across varying thresholds to define the central subnetwork (>50%–10% connections). Higher SVD burden was associated with lower FA in central as well as non-central network connections. However, only central network FA mediated the relationship between white matter hyperintensity volume and executive functioning [change in regression coefficient after mediation (95% CI): -0.15 (-0.35 to -0.02)]. The mediation effect was not observed for FA alterations in non-central network connections [-0.03 (-0.19 to 0.04)]. These findings suggest that the centrality of network connections, and thus their contribution to global network efficiency, appears to be relevant for understanding the relationship between SVD and cognitive impairment.
