Abstract/Results: ABSTRACT:
Neurofeedback, one of the primary examples of self-regulation, designates a collection of techniques that train the brain and help to improve its function. Since coming on the scene in the 1960s, electroencephalography-neurofeedback has become a treatment vehicle for a host of mental disorders; however, its clinical effectiveness remains controversial. Modern imaging technologies of the living human brain (e.g., real-time functional magnetic resonance imaging) and increasingly rigorous research protocols that utilize such methodologies begin to shed light on the underlying mechanisms that may facilitate more effective clinical applications. In this paper we focus on recent technological advances in the field of human brain imaging and discuss how these modern methods may influence the field of neurofeedback. Toward this end, we outline the state of the evidence and sketch out future directions to further explore the potential merits of this contentious therapeutic prospect.

Neuroimaging researchers tacitly assume that body-position scantily affects neural activity. However, whereas participants in most psychological experiments sit upright, many modern neuroimaging techniques (e.g., fMRI) require participants to lie supine. Sparse findings from electroencephalography and positron emission tomography suggest that body position influences cognitive processes and neural activity. Here we leverage multi-postural magnetoencephalography (MEG) to further unravel how physical stance alters baseline brain activity. We present resting-state MEG data from 12 healthy participants in three orthostatic conditions (i.e., lying supine, reclined at 45°, and sitting upright). Our findings demonstrate that upright, compared to reclined or supine, posture increases left-hemisphere high-frequency oscillatory activity over common speech areas. This proof-of-concept experiment establishes the feasibility of using MEG to examine the influence of posture on brain dynamics. We highlight the advantages and methodological challenges inherent to this approach and lay the foundation for future studies to further investigate this important, albeit little-acknowledged, procedural caveat.

Cognitive neuroscientists rarely consider the influence that body position exerts on brain activity; yet, postural variation holds important implications for the acquisition and interpretation of neuroimaging data. Whereas participants in most behavioral and EEG experiments sit upright, many prominent brain imaging techniques (e.g., fMRI) require participants to lie supine. Here we demonstrate that physical comportment profoundly alters baseline brain activity as measured by magnetoencephalography (MEG)-an imaging modality that permits multipostural acquisition. We collected resting-state MEG data from 12 healthy participants in three postures (lying supine, reclining at 45°, and sitting upright). Source-modeling analysis revealed a broadly distributed influence of posture on resting brain function. Sitting upright versus lying supine was associated with greater high-frequency (i.e., beta and gamma) activity in widespread parieto-occipital cortex. Moreover, sitting upright and reclining postures correlated with dampened activity in prefrontal regions across a range of bandwidths (i.e., from alpha to low gamma). The observed effects were large, with a mean Cohen's d of 0.95 ( SD = 0.23). In addition to neural activity, physiological parameters such as muscle tension and eye blinks may have contributed to these posture-dependent changes in brain signal. Regardless of the underlying mechanisms, however, the present results have important implications for the acquisition and interpretation of multimodal imaging data (e.g., studies combining fMRI or PET with EEG or MEG). More broadly, our findings indicate that generalizing results-from supine neuroimaging measurements to erect positions typical of ecological human behavior-would call for considering the influence that posture wields on brain dynamics.

Whereas modern brain imaging often demands holding body positions incongruent with everyday life, posture governs both neural activity and cognitive performance. Humans commonly perform while upright; yet, many neuroimaging methodologies require participants to remain motionless and adhere to non-ecological comportments within a confined space. This inconsistency between ecological postures and imaging constraints undermines the transferability and generalizability of many a neuroimaging assay. Here we highlight the influence of posture on brain function and behavior. Specifically, we challenge the tacit assumption that brain processes and cognitive performance are comparable across a spectrum of positions. We provide an integrative synthesis regarding the increasingly prominent influence of imaging postures on autonomic function, mental capacity, sensory thresholds, and neural activity. Arguing that neuroimagers and cognitive scientists could benefit from considering the influence posture wields on both general functioning and brain activity, we examine existing imaging technologies and the potential of portable and versatile imaging devices (e.g., functional near infrared spectroscopy). Finally, we discuss ways that accounting for posture may help unveil the complex brain processes of everyday cognition.

Neurofeedback appears both to improve normal brain function1 and to treat a wide range of mental disorders, including attention deficit hyperactivity disorder (ADHD), epilepsy, depression, anxiety, insomnia, autism spectrum disorder, and alcoholism. However, despite a relatively long history, the medical community continues to question the clinical utility of this technique. To earn widespread recognition as evidence-based medicine, neurofeedback must meet three challenges: first, perform at least on par with standard-of-care treatments in randomised controlled trials for each disorder that neurofeedback purports to help; second, consistently outperform highly comparable placebo control conditions (eg, sham neurofeedback); and third, establish a clear mechanism for the claimed therapeutic benefits.

Advocates of neurofeedback make bold claims concerning brain regulation, treatment of disorders, and mental health. Decades of research and thousands of peer-reviewed publications support neurofeedback using electroencephalography (EEG-nf); yet, few experiments isolate the act of receiving feedback from a specific brain signal as a necessary precursor to obtain the purported benefits. Moreover, while psychosocial parameters including participant motivation and expectation, rather than neurobiological substrates, seem to fuel clinical improvement across a wide range of disorders, for-profit clinics continue to sprout across North America and Europe. Here we highlight the tenuous evidence supporting EEG-nf and sketch out the weaknesses of this approach. We challenge classic arguments often articulated by proponents of EEG-nf and underscore how psychologists and mental health professionals stand to benefit from studying the ubiquitous placebo influences that likely drive these treatment outcomes.

Abstract/Results: ABSTRACT:
Neurofeedback with functional magnetic resonance imaging (fMRI-nf) heralds new hope concerning the self-regulation of brain and behavior. Theoretically, modulating specific brain activations promises to tailor cognition, emotion, thought, and action; however, reducing complex mental health issues to circumscribed neural regions remains elusive. Empirically, findings from the vast majority of fMRI-nf experiments suggest that we can self-regulate diverse brain activations, yet replication of behavioral findings remains sparse. Here we provide a systematic review that integrates and synthesizes information from 78 primary fMRI-nf research experiments – the bulk of currently available information. To disentangle placebo influences and establish the specific effects of neurofeedback, we highlight the need for double-blind, sham-controlled studies with rigorous and standardized statistical analyses. We further elucidate the value of comparing neural activation and behavior after neurofeedback to pre-training baseline measures, initial trials, and data from control participants. To drive fMRI-nf toward clinical inclusion, relevant studies must confirm the sustainability, transferability, and practicality of fMRI-nf in patient populations. We can change brain activations with fMRI-nf; however, additional carefully designed experiments would have to unravel whether this technique constitutes an effective biobehavioral intervention.

EEG neurofeedback appears to both improve normal brain function and treat a wide range of mental disorders, including attention deficit hyperactivity disorder, epilepsy, depression, anxiety, insomnia, autism spectrum disorder, and alcoholism. Despite a relatively long history, however, the medical community continues to question the clinical utility of this technique. To earn widespread appellation as evidence-based medicine, neurofeedback must meet three challenges: 1) perform at least on par with standard-of-care treatments in randomized controlled trials for each disorder where neurofeedback purports to help; 2) consistently outperform highly comparable placebo control conditions (e.g., sham neurofeedback); and 3) establish a clear mechanism for the claimed therapeutic benefits. In this talk, we will elucidate the crosstalk between placebo science and neurofeedback, including fMRI-nf.

We aimed to better understand how we can use real-time brain imaging to train individuals to actively modify their neural processes and, in turn, achieve specialized states of consciousness. Throughout history, effective brain self-regulation has been the purview of a select few (e.g., expert meditators, hypnotic virtuosos). We investigated whether it is possible to leverage brain imaging to guide naďve participants to adopt expert brain states.
Both hypnosis and meditation demand particular body postures. And yet, when researchers brain imaging these states of consciousness, they often overlook postural constraints. We brain imaged participants in a variety of common postures (standing, sitting, reclined, lying) and found that posture influences brain activity sufficiently that future efforts would benefit from accounting for this variable.
Upon seeking to identify the best neural targets for using neurofeedback to induce altered states of consciousness, we identified that neurofeedback relies heavily on non-specific effects. That is to say, oftentimes, the act of receiving neural feedback itself has little effect compared to all other elements involved in the procedure of neurofeedback (e.g., expectation and motivation).
To continue to advance the field of neurofeedback it would behoove researchers to employ robust control groups and accounts for various confounding factors such as body posture.

Palliative care spans a wide-ranging spectrum: from pain-management to spiritual support. As the demand for end-of-life care increases, so does the demand for innovative, effective, interventions. Mind-body techniques seem especially advantageous in a palliative context. Here we show that hypnosis serves an excellent adjunct therapy in palliative care to boost the efficacy of standard treatments. With the overarching goal of bridging clinical and scientific insights, we outline how five core principles of hypnosis can benefit the diverse needs of palliative care.

Since its launch, applications to the BIAL grants have been increasing. Up to now 461 projects have been supported, involving more than 1000 researchers from 27 countries.
The approved applications have benefited from grants in amounts comprised between €5,000 and €50, 000. The amount to be granted is fixed by the Scientific board according to the needs of each project.
The supported projects have originated, until April 2013, in about 600 full papers, 172 out of which were published in indexed international journals with an average impact factor of 3.6 and a substantial number of citations (1665).
Among the BIAL Foundation fellows is worth highlighting the presence of scientists from prestigious universities from the United States, United Kingdom, Australia, Russia, Germany, Japan, France, Canada, and many others.

Abstract/Results: ABSTRACT:
OBJECTIVE:
We propose that clinicians can use suggestion to help treat conditions such as ADHD. Methods: We use EEG neurofeedback as a case study, alongside evidence from a recent pilot experiment utilizing a sham MRI scanner to highlight the therapeutic potential of suggestion-based treatments.
RESULTS:
The medical literature demonstrates that many practitioners already prescribe treatments that hardly outperform placebo comparators. Moreover, the sham MRI experiment showed that, even with full disclosure of the procedure, suggestion alone can reduce the symptomatology of ADHD.
CONCLUSION:
Non-deceptive suggestion-based treatments, especially those drawing on accessories from neuroscience, may offer a safe complement and potential alternative to current standard of care for individuals with ADHD.

Imaging of the living human brain elucidates the neural dynamics of hypnosis; however, few reliable brain patterns emerge across studies. Here, we methodically assess neuroimaging assays of hypnosis to uncover common neural configurations using a twofold approach. First, we systematically review research on the neural correlates of hypnotic phenomena; then, we meta-analyze these collective data seeking specific activation and deactivation patterns that typify hypnosis. Anchored around the role of top-down control processes, our comprehensive examination focuses on the involvement of intrinsic brain networks known to support cognitive control and self-referential cognition, including the executive, salience, and default networks. We discuss how these neural dynamics may relate to contemporary theories of hypnosis and show that hypnosis correlates with activation of the lingual gyrus a brain region involved in higher order visual processing and mental imagery. Our findings help to better understand the neurobiological substrates comprising the appellation hypnosis.
