Brain imaging methods have contributed to shed light on the possible mechanisms of recovery and cortical reorganization after early brain insult. The idea that a functional left hemisphere is crucial for achieving a normalized pattern of language development after left perinatal stroke is still under debate. We report the case of a 3.5-year-old boy born at term with a perinatal ischemic stroke of the left middle cerebral artery, affecting mainly the supramarginal gyrus, superior parietal and insular cortex extending to the precentral and postcentral gyri. Neurocognitive development was assessed at 25 and 42 months of age. Language outcomes were more extensively evaluated at the latter age with measures on receptive vocabulary, phonological whole-word production and linguistic complexity in spontaneous speech. Word learning abilities were assessed using a fast-mapping task to assess immediate and delayed recall of newly mapped words. Functional and structural imaging data as well as a measure of intrinsic connectivity were also acquired. While cognitive, motor and language levels from the Bayley Scales fell within the average range at 25 months, language scores were below at 42 months. Receptive vocabulary fell within normal limits but whole word production was delayed and the child had limited spontaneous speech. Critically, the child showed clear difficulties in both the immediate and delayed recall of the novel words, significantly differing from an age-matched control group. Neuroimaging data revealed spared classical cortical language areas but an affected left dorsal white-matter pathway together with right lateralized functional activations. In the framework of the model for Social Communication and Language Development, these data confirm the important role of the left arcuate fasciculus in understanding and producing morpho-syntactic elements in sentences beyond two word combinations and, most importantly, in learning novel word-referent associations, a building block of language acquisition.

The idea that a functional left hemisphere is crucial for achieving a normalized pattern of language development after left perinatal stroke is still under debate. Here, we report an interesting case of a 3 year-old child born at term with a left perinatal ischemic stroke of the left middle cerebral artery, affecting the superior temporal gyrus and extending to the supra-marginal gyrus. We evaluated the child for cognitive, social and linguistic abilities at 3 years of age. Linguistic learning abilities were also assessed using a fast-mapping task adapted for preschoolers and mimicking the first steps of language acquisition. Moreover, we acquired functional and structural imaging data as well as a measure of intrinsic connectivity. Despite having linguistic receptive functions within the normal range, the child had clear difficulties in the productive aspects of language, was impaired in learning new word-referent associations, showed a right lateralized pattern of BOLD activation during a passive listening task and had a disrupted left arcuate fasciculus. These data suggest an important role of the left hemisphere and specifically of the arcuate fasciculus for the learning of novel word-referent associations, one of the building block of vocabulary acquisition.

BACKGROUND
Little is known on the effect of early brain lesion on the acquisition of language in infants. Patients who suffered a left hemisphere injury during prenatal or perinatal periods are of great interest, as they present individual differences in their degree of language recovery. Only few fMRI data collected in children with perinatal left-hemisphere brain lesions have brought evidence that the undamaged right-hemisphere is able to take over language functions as revealed by near to normal linguistic processing, thus supporting a model of recovery based on inter-hemispheric transfer of function. It is still an open question to which extent language reorganization and plasticity processes occurring after an early brain insult is enough for normal language development.
AIMS
We aimed at better understanding how functional and structural connectivity during brain maturation is reorganized after perinatal ischemic stroke and its impact in language development.
METHOD
Neurocognitive development was assessed at 25-42 months of age after perinatal stroke (N=10). Word learning abilities were also assessed using a fast-mapping task. Functional and structural (DTI)-MRI imaging was measured (including intrinsic connectivity).
RESULTS
The combination of functional and structural MRI as well as language and language learning measures, showed that children who had a stroke affecting crucial language areas, including white-matter pathways of the left hemisphere, are especially vulnerable to the negative impact of the stroke on their later language outcomes.
CONCLUSION
These results suggest that the amount of right lateralized reorganization induced by early left lesion predicts better outcomes in language development.

Among its activities the BIAL Foundation manages the BIAL Award, created in 1984, one of the most important awards in the Health field in Europe. The BIAL Award rewards both the basic and the clinical research distinguishing works of major impact in medical research.
The BIAL Foundation also assigns Scientific Research Scholarships for the study of neurophysiological and mental health in people, arousing the interest of researchers in the areas of Psychophysiology and Parapsychology.
To date the BIAL Foundation has supported 461 projects, more than 1000 researchers, with research groups in twenty-seven countries, resulting, until April 2013, in about 600 full papers, out of which 172 published in indexed international journals with an average impact factor of 3.6 and a substantial number of citations (1665).
Since 1996 the BIAL Foundation organizes the Symposia entitled "Behind and Beyond the Brain", a Forum that gathers well renowned neurosciences speakers and the BIAL Foundation Fellows which are spread around the world.
Classified as an institution of public utility, the BIAL Foundation includes among its patrons the Portuguese President, the Portuguese Universities Rectors' Council and the Portuguese Medical Association.

The approved applications have benefited from grants in amounts comprised between €5,000 and €50, 000. The amount to be granted is fixed by the Scientific board according to the needs of each project.

Brain imaging methods have contributed to shed light on the mechanisms of recovery after early brain insult. The assumption that the unaffected right hemisphere can take over language functions after left perinatal stroke is still under debate. Here, we report how patterns of brain structural and functional reorganization were associated with language outcomes in a group of four-year-old children with left perinatal arterial ischemic stroke (PAIS). Specifically, we gathered specific fine-grained developmental measures of receptive and productive aspects of language as well as standardized measures of cognitive development. We also collected structural neuroimaging data as well as functional activations during a passive listening story-telling fMRI task and a resting state session (rs-fMRI). Children with a left perinatal stroke showed larger lateralization indices of both structural and functional connectivity of the dorsal language pathway towards the right hemisphere that, in turn, were associated with better language outcomes. Importantly, the pattern of structural asymmetry was significantly more right-lateralized in children with a left perinatal brain insult than in a group of matched healthy controls. These results strongly suggest that early lesions of the left dorsal pathway and the associated perisylvian regions can induce the interhemispheric transfer of language functions to right homolog regions. This study provides combined evidence of structural and functional brain reorganization of language networks after early stroke with strong implications for neurobiological models of language development.

Brain imaging methods such as functional Magnetic Resonance Imaging (fMRI) and Diffusion Tensor Imaging (DTI) have already been used to decipher the functional and structural brain changes occurring during normal language development. However, little is known about the differentiation of the language network after an early lesion. While in adults, stroke over the left hemisphere generally induces post-stroke aphasia, it is not always the case when a stroke occurs in the perinatal period, thus revealing a remarkable plastic power of the language network during early development. In particular, the role of perilesional tissues, as opposed to undamaged brain areas in the functional recovery of language functions after an early insult, remains unclear. In this review article, we provide an overview of the extant literature using functional and structural neuroimaging data revealing the signatures of brain plasticity underlying near-normal language development.
