BACKGROUND
Whereas psychophysiological insomnia is the most common health complaint and has serious consequences, the neural correlates of altered perception and consciousness are insufficiently known.
AIM
We here aimed to evaluate electroencephalographic (EEG) markers of hyperarousal, cortical inhibition and attentional filtering during resting state.
METHOD
High-density EEG and electrocardiography (ECG) were recorded in up to 51 people complaining about insomnia and 43 matched controls. Analyses including source estimation quantified the spatiotemporal, spectral, and long-range temporal correlation (LRTC) properties of the EEG during resting state, including Heartbeat Evoked Potentials (HEP).
RESULTS
People that complain about insomnia showed less power in a narrow upper alpha band around 11.7 Hz over bilateral frontal and left temporal regions. They moreover showed more power in a broad beta frequency range (16.3-40 Hz) across the scalp. In addition, they showed a larger amplitude late HEP component than controls at frontal electrodes 376-500 ms after the R-wave peak in anterior cingulate/medial frontal cortices. Finally, individuals who experienced worse sleep quality had stronger LRTC.
CONCLUSIONS
The findings suggest hyperarousal, insufficient cerebral cortical inhibition and attentional filtering, and insufficient interoceptive adaptation of their brain responses to the ever-present heartbeats. The findings provide insight into the neural correlates of sleep complaints including enhanced awareness that could underlie sleep state misperception.

INTRODUCTION
Advances in ultra-high field (uhf) fMRI now allow for probing functional responses at the level of mesoscopic structures such as columns and layers (e.g. [1-5]). This offers the opportunity to relate responses at the level of cortical building blocks to perception. Doing so, however, requires using stimuli that allow for dissociating sensory input and perceptual interpretation.
Here, we presented subjects with a bistable motion quartet while recording responses in human motion complex (hMT+). The "motion quartet" gives rise to two mutually exclusive interpretations: either horizontal or vertical motion. Importantly, the percepts switch over time, while the sensory input remains constant throughout.
In previous studies, we found that the level of signal measured in hMT+ as a whole is reflective of such perceptual switches [6]. We also showed that it is feasible to map columnar-level clusters within hMT+ that prefer specific axes of motion [7]. Combining these two research lines here, we investigated the role of mesoscopic hMT+ clusters in perceived motion direction using uhf fMRI at 7 Tesla.
METHODS
Seven subjects (median age = 25 y, 2 females) participated in the study. Area hMT+ was localised with standard moving and static dot stimuli [8] using a standard GE EPI sequence at 1.6 mm isotropic resolution. We then zoomed in hMT+ using a sub-millimetre GE EPI sequence (TE = 25.6 ms; FA = 69°; TR = 2000 ms; resolution: 0.8 mm isotropic). Subjects were presented with either the bistable motion quartet ("ambiguous motion") or a motion display in which squares moved unambiguously either along the horizontal or vertical motion trajectory ("physical motion"). For both stimulus types, participants reported the perceived direction of motion by button presses.
Two participants were excluded from analyses due to excessive head motion or inappropriate task performance. Data were analysed using the standard general linear model. All analyses were limited to voxels that responded more to moving than static dots (p < 0.005), based on hMT+ localiser data. For the ambiguous and physical motion experiments, voxels were considered part of the vertical cluster if their time courses were modulated by vertical motion and if they responded more to vertical than to horizontal motion (t > 1.5). Voxels were considered part of the horizontal cluster if reverse conditions applied. For event-related averaging, functional signals were extracted from identified motion clusters, converted to percentage signal change, and averaged for horizontal and vertical stimulus condition.
RESULTS
For both the physical and ambiguous motion experiments, we observed clusters of voxels in hMT+ preferring either horizontal or vertical motion stimuli. Upon qualitative inspection there was considerable overlap in voxel preferences between the two experiments (Figure 1). This overlap was also shown in a quantitative analysis (Figure 2). Event-related averages indicated that signal time courses in horizontal and vertical clusters reflected the perceived motion direction, for both the physical (Figure 3) and the ambiguous (Figure 4) motion display. Thus, when subjects indicated a switch in perception from vertical to horizontal motion, this was paralleled by an increase in signal in the horizontal hMT+ cluster and a decrease in the vertical cluster. Reversely, when subjects reported a switch from horizontal to vertical motion, signal increased in the vertical cluster and decreased in the horizontal one.
CONCLUSIONS
We found preliminary evidence that the content of conscious perception of motion stimuli can be read out from dynamic activation changes of direction-selective, mesoscopic clusters in human MT+. Previous studies [9] demonstrated a coupling between visual awareness and activity in different macroscopic brain areas. Our study indicates for the first time that uhf fMRI studies might allow for disentangling the contribution to visual awareness from mesoscopic clusters within one specialized brain area.

This article summarizes key advances in hypnosis research during the past two decades, including (i) clinical research supporting the efficacy of hypnosis for managing a number of clinical symptoms and conditions, (ii) research supporting the role of various divisions in the anterior cingulate and prefrontal cortices in hypnotic responding, and (iii) an emerging finding that high hypnotic suggestibility is associated with atypical brain connectivity profiles. Key recommendations for a research agenda for the next decade include the recommendations that (i) laboratory hypnosis researchers should strongly consider how they assess hypnotic suggestibility in their studies, (ii) inclusion of study participants who score in the middle range of hypnotic suggestibility, and (iii) use of expanding research designs that more clearly delineate the roles of inductions and specific suggestions. Finally, we make two specific suggestions for helping to move the field forward including (i) the use of data sharing and (ii) redirecting resources away from contrasting state and nonstate positions toward studying (a) the efficacy of hypnotic treatments for clinical conditions influenced by central nervous system processes and (b) the neurophysiological underpinnings of hypnotic phenomena. As we learn more about the neurophysiological mechanisms underlying hypnosis and suggestion, we will strengthen our knowledge of both basic brain functions and a host of different psychological functions.

Since its launch, applications to the BIAL grants have been increasing. Up to now 461 projects have been supported, involving more than 1000 researchers from 27 countries.
The approved applications have benefited from grants in amounts comprised between €5,000 and €50, 000. The amount to be granted is fixed by the Scientific board according to the needs of each project.
The supported projects have originated, until April 2013, in about 600 full papers, 172 out of which were published in indexed international journals with an average impact factor of 3.6 and a substantial number of citations (1665).
Among the BIAL Foundation fellows is worth highlighting the presence of scientists from prestigious universities from the United States, United Kingdom, Australia, Russia, Germany, Japan, France, Canada, and many others.

Consciousness and the double-slit interference pattern: Six experiments

ABSTRACT:
A double-slit optical system was used to test the possible role of consciousness in the collapse of the quantum wavefunction. The ratio of the interference pattern's double-slit spectral power to its single-slit spectral power was predicted to decrease when attention was focused toward the double slit as compared to away from it. Each test session consisted of 40 counterbalanced attention-toward and attention-away epochs, where each epoch lasted between 15 and 30 s. Data contributed by 137 people in six experiments, involving a total of 250 test sessions, indicate that on average the spectral ratio decreased as predicted (z=-4.36, p=6 x 10(-6)). Another 250 control sessions conducted without observers present tested hardware, software, and analytical procedures for potential artifacts; none were identified (z=0.43, p=0.67). Variables including temperature, vibration, and signal drift were also tested, and no spurious influences were identified. By contrast, factors associated with consciousness, such as meditation experience, electrocortical markers of focused attention, and psychological factors including openness and absorption, significantly correlated in predicted ways with perturbations in the double-slit interference pattern. The results appear to be consistent with a consciousness-related interpretation of the quantum measurement problem.

The notion that death represents a passing to an afterlife, where we are reunited with loved ones and live eternally in a utopian paradise, is common in the anecdotal reports of people who have encountered a “near-death experience” (NDE). These experiences are usually portrayed as being extremely pleasant including features such as a feeling of peacefulness, the vision of a dark tunnel leading to a brilliant light, the sensation of leaving the body, or the experience of a life review. NDEs are increasingly being reported as a clearly identifiable physiological and psychological reality of clinical and scientific significance. The definition and causes of the phenomenon as well as the identification of NDE experiencers are still matters of debate. The phenomenon has been thoroughly portrayed by the media, but the science of NDEs is rather recent and still lacking of rigorous experimental data and reproducible controlled experiments. It seems that the most appropriate theories to explain the phenomenon tend to integrate both psychological and neurobiological mechanisms. The paradoxical dissociation between the richness and intensity of the memory, probably occurring during a moment of brain dysfunction, offers a unique opportunity to better understand the neural correlates of consciousness. In this chapter, we will attempt to describe NDEs and the methods to identify them. We will also briefly discuss the NDE experiencers’ characteristics. We will then address the main current explicative models and the science of NDEs.

Empirical reports in mainstream journals that human cognition extends in ways that challenge the current boundaries of science (anomalous cognition) has been viewed with dismay by many who see it as evidence that science is broken. Here the authors make the case for the value of conducting and publishing well-designed studies investigating anomalous cognition. They distinguish between the criteria that justify entertaining the possibility of anomalous cognition from those required to endorse it as a bona fide phenomenon. In evaluating these 2 distinct thresholds, the authors draw on Bayes’s theorem to argue that scientists may reasonably differ in their appraisals of the likelihood that anomalous cognition is possible. Although individual scientists may usefully vary in the criteria that they hold both for entertaining and endorsing anomalous cognition, we provide arguments for why researchers should consider adopting a liberal criterion for entertaining anomalous cognition while maintaining a very strict criterion for the outright endorsement of its existence. Grounded in an understanding of the justifiability of disparate views on the topic, the authors encourage humility on both the part of those who present evidence in support of anomalous cognition and those who dispute the merit of its investigation.

BACKGROUND:
Patients with chronic disorders of consciousness face a significant lack of treatment options.
OBJECTIVE:
We aimed at investigating the feasibility and the behavioral effects of home-based transcranial direct current stimulation (tDCS), applied by relatives or caregivers, in chronic patients in minimally conscious state (MCS).
METHODS:
Each participant received, in a randomized order, 20 sessions of active and 20 sessions of sham tDCS applied over the prefrontal cortex for 4 weeks; separated by 8 weeks of washout. Level of consciousness was assessed using the Coma Recovery Scale-Revised before the first stimulation (baseline), at the end of the 20 tDCS sessions (direct effects) and 8 weeks after the end of each stimulation period (long-term effects). Reported adverse events and data relative to the adherence (i.e., amount of sessions effectively received) were collected as well.
RESULTS:
Twenty-seven patients completed the study and 22 patients received at least 80% of the stimulation sessions. All patients tolerated tDCS well, no severe adverse events were noticed after real stimulation and the overall adherence (i.e., total duration of stimulation) was good. A moderate effect size (0.47 and 0.53, for modified intention to treat and per protocol analysis, respectively) was observed at the end of the 4 weeks of tDCS in favor of the active treatment.
CONCLUSIONS:
We demonstrated that home-based tDCS can be used adequately outside a research facility or hospital by patients' relatives or caregivers. In addition, 4 weeks of tDCS moderately improved the recovery of signs of consciousness in chronic MCS patients.

Abstract/Results: ABSTRACT:
BACKGROUND:
Recent research on psychedelics and hypnosis demonstrates the value of both methods in the treatment of a range of psychopathologies with overlapping applications and neurophenomenological features. The potential of harnessing the power of suggestion to influence the phenomenological response to psychedelics toward more therapeutic action has remained unexplored in recent research and thereby warrants empirical attention.
AIMS:
Here we aim to elucidate the phenomenological and neurophysiological similarities and dissimilarities between psychedelic states and hypnosis in order to revisit how contemporary knowledge may inform their conjunct usage in psychotherapy.
METHODS:
We review recent advances in phenomenological and neurophysiological research on psychedelics and hypnosis, and we summarize early investigations on the coupling of psychedelics and hypnosis in scientific and therapeutic contexts. Results/outcomes: We highlight commonalities and differences between psychedelics and hypnosis that point to the potential efficacy of combining the two in psychotherapy. We propose multiple research paths for coupling these two phenomena at different stages in the preparation, acute phase and follow-up of psychedelic-assisted psychotherapy in order to prepare, guide and integrate the psychedelic experience with the aim of enhancing therapeutic outcomes.
CONCLUSIONS/INTERPRETATION:
Harnessing the power of suggestion to modulate response to psychedelics could enhance their therapeutic efficacy by helping to increase the likelihood of positive responses, including mystical-type experiences.

Abstract/Results: ABSTRACT:
We applied continuous flash suppression (CFS) during an interocular transfer paradigm to evaluate the importance of awareness and the contribution of early versus late visual structures in size recognition. Specifically, we tested if size judgements of a visible target could be influenced by a congruent or incongruent prime presented to the same or different eye. Without CFS, participants categorised a target as “small” or “large” more quickly when it was preceded by a congruent prime – regardless of whether the prime and target were presented to the same or different eye. Interocular transfer enabled us to infer that the observed priming was mediated by late visual areas. In contrast, there was no priming under CFS, which underscores the importance of awareness. We conclude that awareness and late visual structures are important for size perception and that any subconscious processing of the stimulus has minimal effect on size recognition.

Detection and interpretation of signs of "covert command following" in patients with disorders of consciousness (DOC) remains a challenge for clinicians. In this study, we used a tactile P3-based BCI in 12 patients without behavioral command following, attempting to establish "covert command following." These results were then confronted to cerebral metabolism preservation as measured with glucose PET (FDG-PET). One patient showed "covert command following" (i.e., above-threshold BCI performance) during the active tactile paradigm. This patient also showed a higher cerebral glucose metabolism within the language network (presumably required for command following) when compared with the other patients without "covert command-following" but having a cerebral glucose metabolism indicative of minimally conscious state. Our results suggest that the P3-based BCI might probe "covert command following" in patients without behavioral response to command and therefore could be a valuable addition in the clinical assessment of patients with DOC.

Advances in medicine and intensive care have led to an increase in the number of patients surviving a severe brain injury. Although some patients recover quickly, others remain in a state of altered consciousness. These patients can progress from a coma to a vegetative state or unresponsive wakefulness syndrome (VS/UWS) and later to a minimally conscious state (MCS). In this review, we first describe the different behavioral and neuroimaging methods used in the diagnosis of patients in an altered state of consciousness. We then describe the factors that can influence the prognosis and recovery of these patients, as well as the treatment and therapeutic management that can be proposed in order to improve their state of consciousness. Finally, we conclude this review by highlighting ethical considerations and end-of-life issues.

OBJECTIVE
The aim of the study is to explore the possibility of oral feeding in unresponsive wakefulness syndrome/vegetative state (UWS/VS) patients.
METHOD
We reviewed the clinical information of 68 UWS/VS patients (mean age 45 ± 11; range 16-79 years) searching for mention of oral feeding. UWS/VS diagnosis was made after repeated behavioural assessments using the Coma Recovery Scale-Revised. Patients also had complementary neuroimaging evaluations (positron emission tomography, functional magnetic resonance imaging and electroencephalography and diffusion tensor imaging).
RESULTS
Out of the 68 UWS/VS patients, only two could resume oral feeding (3%). The first patient had oral feeding (only liquid and semi liquid) in addition to gastrostomy feeding and the second one could achieve full oral feeding (liquid and mixed solid food). Clinical assessments concluded that they fulfilled the criteria for a diagnosis of UWS/VS. Results from neuroimaging and neurophysiology were typical for the first patient with regard to the diagnosis of UWS/VS but atypical for the second patient.
CONCLUSION
Oral feeding that implies a full and complex oral phase could probably be considered as a sign of consciousness. However, we actually do not know which components are necessary to consider the swallowing conscious as compared to reflex. We also discussed the importance of swallowing assessment and management in all patients with altered state of consciousness.

Different behavioural signs of consciousness can distinguish patients with an unresponsive wakefulness syndrome from patients in minimally conscious state (MCS). The Coma Recovery Scale-Revised (CRS-R) is the most sensitive scale to differentiate the different altered states of consciousness and eleven items detect the MCS. The aim of this study is to document the prevalence of these items. We analysed behavioural assessments of 282 patients diagnosed in MCS based on the CRS-R. Results showed that some items are particularly frequent among patients in MCS, namely fixation, visual pursuit, and reproducible movement to command, which were observed in more than 50% of patients. These responses were also the most probably observed items when the patients only showed one sign of consciousness. On the other hand, some items were rarely or never observed alone, e.g., object localisation (reaching), object manipulation, intelligible verbalisation, and object recognition. The results also showed that limiting the CRS-R assessment to the five most frequently observed items (i.e., fixation, visual pursuit, reproducible movement to command, automatic motor response and localisation to noxious stimulation) detected 99% of the patients in MCS. If clinicians have only limited time to assess patients with disorders of consciousness, we suggest to evaluate at least these five items of the CRS-R.

ABSTRACT:
BACKGROUND
Near-Death Experiences (NDEs) refer to mental events with highly emotional and mystical aspects classically occurring in altered states of consciousness following a real or perceived life-threatening situation.
Aims: The aim of this project was to better characterize the NDE phenomenon and their experiencers though the integration of both a psychological and a neuropsychological approach. We conducted 7 studies aiming to investigate the NDE memory, 2 studies aiming to study the NDE experiencers’ personality and cognitive characteristics, and 3 studies aiming to investigate the neurobiological basis of the NDE phenomenon.
METHOD
Using qualitative (e.g. thematic analysis) and quantitative (e.g. natural language processing tool) analyses of hundreds of written narratives, we extracted common features, identified their order of appearance, and we compared them with experiences elicited by different types of psychoactive drugs. We also retrospectively assessed experiencers’ cognitive and personality variables, the phenomenological characteristics of the memory and assessed to what extent they are considered self-defining using validated questionnaires. Finally, we reproduced NDE-like features in controlled laboratory settings in ‘naïve’ healthy subjects and NDE experiencers by administrating DMT, medically-controlled vaso-vagal syncopes and using hypnosis, while performing high-density EEG monitoring to measure cerebral electrical changes.
RESULTS&CONCLUSIONS
NDE memories comprise recurrent time-bounded and transversal features with no prototypical sequences, and are associated with an important amount of phenomenological characteristics –correlated with the richness of the experience itself. Besides, they are self-defining, meaning that they are central to experiencers’ identities. A natural language processing tool enabled to identify, via semantic similarity, the long-standing link between certain drugs and NDEs. Indeed, the NMDA receptor antagonist ketamine consistently resulted in reports most similar to those associated with NDEs. Ketamine was followed by a series of serotonergic psychedelics (e.g. DMT) and could be used as a reversible experimental model for NDE phenomenology. In addition, our findings support the view that a particularly strong engagement in fantasy and a high illusory recollection susceptibility might make people more likely to report such experiences when exposed to suitable physiological and psychological conditions. Finally, we observed significant overlaps in nearly all of the NDE-like phenomenological features induced in controlled laboratory settings, compared with reports of ‘actual’ NDEs. These studies suggest that NDE features are brain-mediated events resulting from dynamic changes in the brain’s activity and connectivity.
