Among its activities the BIAL Foundation manages the BIAL Award, created in 1984, one of the most important awards in the Health field in Europe. The BIAL Award rewards both the basic and the clinical research distinguishing works of major impact in medical research.
The BIAL Foundation also assigns Scientific Research Scholarships for the study of neurophysiological and mental health in people, arousing the interest of researchers in the areas of Psychophysiology and Parapsychology.
To date the BIAL Foundation has supported 461 projects, more than 1000 researchers, with research groups in twenty-seven countries, resulting, until April 2013, in about 600 full papers, out of which 172 published in indexed international journals with an average impact factor of 3.6 and a substantial number of citations (1665).
Since 1996 the BIAL Foundation organizes the Symposia entitled "Behind and Beyond the Brain", a Forum that gathers well renowned neurosciences speakers and the BIAL Foundation Fellows which are spread around the world.
Classified as an institution of public utility, the BIAL Foundation includes among its patrons the Portuguese President, the Portuguese Universities Rectors' Council and the Portuguese Medical Association.

Substantial evidence supports the involvement of the human medial temporal lobe (MTL) in episodic memory. Although differential roles have been attributed to left and right MTL in mediating verbal and visual memory, few studies have directly shown a clear material-specific dissociation. This issue becomes particularly relevant in the pre-operative evaluation of temporal lobe epilepsy (TLE) patients with hippocampal sclerosis (HS). Here, we used functional magnetic resonance imaging (fMRI) to compare the neural correlates of verbal and visual episodic memory during both encoding and recognition tasks, in a group of normal subjects and in TLE patients with unilateral right and left HS. Abstract words and line drawings designed so as to separate the verbal and visual components of the material were used [Jones-Gotman, 1997]. Statistical analysis for the control group shows a left-lateralized pattern of activation, including the left anterior hippocampus and the left parahippocampus, for the verbal material. Activity during visual encoding was largely bilateral and included the right posterior hippocampus and both parahippocampi. Results in patients suggest greater MTL activity on the side contralateral to the lesion. These results are consistent with data from neuropsychological tests using the same material. The relative unimpairment of right TLE patients on the visual memory task could be explained by the posterior location of right hippocampal activation relative to the more anterior lesion site. This is in contrast with the impairment of left TLE patients on the verbal memory task, which involves the anterior portion of the left hippocampus. In conclusion, this study supports material-dependent regional dissociations of MTL function which may help explain neuropsychological findings in patients with MTL damage.

Abstract/Results: RESULTS:
This study relied on nine epilepsy surgery candidates implanted with foramen ovale electrodes. 1.5-3Hz rhythmic slow activity (RSA) was a predominant activity pattern during REM sleep in most patients. This activity was also found to be highly synchronous both intra- and interhemispherically. Positive half-waves of the 1.5–3Hz RSA were identified by a semi-automatic algorithm during REM sleep. High-frequency activity was assessed as root mean square (RMS) for 11 consecutive 20 Hz–wide frequency bands between 20 and 240Hz. Calculating individual spectra revealed a broad but definite peak in the high-frequency band in seven cases (hemispheres). Statistical analysis revealed a clear phase-coupling of high-frequency activity in all patients and for most of the high-frequency bands studied. This phase relation was similar across the high frequency bands within the same patient and hemisphere. This allowed us to use individually defined fixed intervals of 0.1 s to statistically compare RMS values corresponding to the peak and the trough of the triggered RMS curve. Typically the preferred phase occurred before the RSA peak used as trigger. In most cases modulation statistics exhibited a U-shaped curve with highest significance levels in the middle frequency bands (60–80Hz and 80–100Hz). Compared to these bands modulation generally weakened across both decreasing and increasing frequency ranges. Such a phase-coupling between delta and gamma activity is similar to that seen between theta and gamma in rodents. We consider this commonality to be an additional reason for regarding delta rather than theta as the human analogue of RSA in animals.

OBJECTIVES:
To investigate interhemispheric propagation of mesial temporal lobe epilepsy seizures in patients undergoing long-term video-EEG monitoring with combined scalp and foramen ovale electrodes.
AIM OF THE STUDY:
To reveal possible interhemispheric propagation patterns in mesial temporal lobe epilepsy, to improve presurgical evaluation of temporal epileptic patients.
METHODS:
Sixty-five seizures from 20 patients were analyzed. We defined two contralateral seizure propagation patterns: Type I for those seizures that spread to the contralateral foramen ovale electrodes earlier than to the contralateral scalp electrodes, and type II for the opposite.
PARTICIPANTS:
Twenty drug resistant epileptic patients were investigated in frame of their presurgical evaluation.
RESULTS:
The majority of seizures (80%) were classified as type I. Inter-foramen ovale electrode propagation time was significantly shorter for type I compared to type II seizures. Ninety percent of patients had either type I or type II seizures only. Patients with type I seizures significantly more often had mesiotemporal structural alterations evident on magnetic resonance imaging scans, and became more often seizure-free after surgery compared to patients with type II seizures whose surgical outcome was less favorable or surgery could not be indicated because of independent bilateral ictal seizure-onset.
CONCLUSIONS:
The two types of contralateral propagation patterns we are describing seem to represent two subtypes of mesial temporal lobe epilepsy with different morphological and prognostic features. The predominance of type I over type II seizures together with shorter propagation times for type I seizures indicate a role of a more direct and dominant interhemispheric pathway in mesial temporal lobe epilepsy.

Sleep-associated memory consolidation is thought to rely on coordinated information transfer between the hippocampus and neocortex brought about during slow wave sleep (SWS) by distinct local field potential oscillations. Specifically, findings in animals have led to the concept that ripples originating from hippocampus combine with spindles to provide a fine-tuned temporal frame for a persistent transfer of memory-related information to the neocortex. The present study focused on characterizing the temporal relationship between parahippocampal ripple activity (80-140 Hz) and spindles recorded from frontal, parietal and parahippocampal cortices in 12 epilepsy patients implanted with parahippocampal foramen ovale electrodes. Overall, parietal and parahippocampal spindles showed closer relationships to parahippocampal ripple activity than frontal spindles, with the latter following parietal and parahippocampal spindles at a variable delay of up to 0.5 s. On a timescale of seconds, ripple activity showed a continuous increase before the peak of parietal and parahippocampal spindles, and decreased thereafter. At a fine timescale of milliseconds, parahippocampal ripple activity was tightly phase-locked to the troughs of these spindles. The demonstration of spindle phase-locked ripple activity in humans is consistent with the idea of a temporally fine-tuned hippocampus-to-neocortex transfer of information taking place during SWS.

This book is a comprehensive overview of the main current concepts in brain cognitive activities at the global, collective (or network) level, with a focus on transitions between normal neurophysiology and brain pathological states. It provides a unique approach of linking molecular and cellular aspects of normal and pathological brain functioning with their corresponding network, collective and dynamical manifestations that are subsequently extended to behavioral manifestations of healthy and diseased brains. This book introduces a high-level perspective, searching for simplification amongst the structural and functional complexity of nervous systems by consideration of the distributed interactions that underlie the collective behavior of the system. The authors hope that this approach could promote a global comprehensive understanding of high-level laws behind the elementary biological processes in the neuroscientific community, while, perhaps, introducing elements of biological complexities to the mathematical/computational readership. Abstract/Results: RESULTS:
Alterations of consciousness are critical factors in the diagnosis of epilepsy and seizure disorders. With this alteration in consciousness, some persons report unusual experiences that resemble mystical experience. This study identified and characterized the subjective experiences associated with seizure activity, using standardized quantitative instruments.
More than half the patients in this study recalled some subjective experience in association with their seizures, but none of their descriptions met criteria for mystical experience or for near-death experience. Those few features of mystical or near-death experience that were reported were not associated primarily with temporal lobe function, nor with the right or left side of the brain. Among 100 patients, 7 reported out-of-body experiences associated with their seizures. There were no differentiating traits associated with reports of out-of-body experience, in terms of either demographics, medical history including seizure risk factors and precipitants, seizure characteristics including localization and type of seizure, or neuropsychological functioning including ability to recall subjective experiences associated with their seizures.

Abstract/Results: ABSTRACT:
In this study, we introduce a new approach to process simultaneous Electroencephalography and functional Magnetic Resonance Imaging (EEG-fMRI) data in epilepsy. The method is based on the decomposition of the EEG signal using independent component analysis (ICA) and the usage of the relevant components' time courses to define the event related model necessary to find the regions exhibiting fMRI signal changes related to interictal activity. This approach achieves a natural data-driven differentiation of the role of distinct types of interictal activity with different amplitudes and durations in the epileptogenic process. Agreement between the conventional method and this new approach was obtained in 6 out of 9 patients that had interictal activity inside the scanner. In all cases, the maximum Z-score was greater in the fMRI studies based on ICA component method and the extent of activation was increased in 5 out of the 6 cases in which overlap was found. Furthermore, the three cases where an agreement was not found were those in which no significant activation was found at all using the conventional approach

OBJECTIVES:
To examine possible phase-relation between high-frequency (gamma) and rhythmic slow activity (RSA) during REM sleep in human mesiotemporal EEG recordings. To seek for analogies between the animal hippocampal theta and the human mesiotemporal RSA during REM sleep.
METHODS:
The study relied on nine epilepsy surgery candidates implanted with bilateral semi-invasive foramen ovale electrodes placed beneath the mesiotemporal surface. Positive half-waves of the 1.5–3 Hz RSA were identified by a semi-automatic algorithm during REM sleep. High-frequency activity was assessed for 11 consecutive 20 Hz–wide frequency bands between 20 and 240 Hz. To investigate time relations between RSA
oscillation and high-frequency activity the root mean square of each high-frequency band was averaged for intervals of 0.5 s around all detected RSA oscillation peaks.
RESULTS:
RSA at 1-5-3 Hz was a striking feature of most foramen ovale recordings during REM sleep. Increase in high frequency activity was phase coupled with RSA in most frequency bands and patients. The phase of the highest gamma activity increase was similar across increasing high frequency bands and typically occurred shortly (~0.1 s) before the RSA peak used as trigger. Highest level of modulation was found for the middle high frequency bands (60-80 Hz and 80-100 Hz) which gradually weakened across both decreasing and increasing high-frequency bands.
CONCLUSIONS:
Such a phase coupling closely resembles that seen between theta and gamma in rodents. We consider this commonality to be an additional reason for regarding the 1.5-3 Hz delta rather than theta as the human analogue of RSA in animals.

Alterations of consciousness are critical factors in the diagnosis of epilepsy and seizure disorders. With this alteration in consciousness, some persons report unusual experiences that have been thought to resemble spontaneous mystical experiences. This study was designed to identify and characterize the mystical experiences associated with seizure activity, through the use of a quantitative instrument with well-documented reliability and validity for assessing the content and quality of mystical states of consciousness. Ninety-eight patients with epilepsy completed a modified version of the Mysticism Scale, of whom 86 had EEGs recorded. Fifty-five per cent of the patients in this study recalled some subjective experience in association with their seizures. However, none of the patients' descriptions met the criteria for mystical experience. Although some features of mystical experience were reported in this study, particularly those suggestive of introvertive mysticism, they were not associated with any particular lobe of the brain nor were they lateralized to either the right or left side. Mysticism Scale scores were not significantly associated with demographics, medical history including seizure risk factors and precipitants, or seizure characteristics including localization and type of seizure.

Alterations of consciousness are critical factors in the diagnosis of epileptic seizures. With these alterations in consciousness, some persons report sensations of separating from the physical body, experiences that may in rare cases resemble spontaneous out-of-body experiences. This study was designed to identify and characterize these out-of-body-like subjective experiences associated with seizure activity. Fifty-five percent of the patients in this study recalled some subjective experience in association with their seizures. Among our sample of 100 patients, 7 reported out-of-body experiences associated with their seizures. We found no differentiating traits that were associated with patients' reports of out-of-body experiences, in terms of either demographics; medical history, including age of onset and duration of seizure disorder, and seizure frequency; seizure characteristics, including localization, lateralization, etiology, and type of seizure, and epilepsy syndrome; or ability to recall any subjective experiences associated with their seizures. Reporting out-of-body experiences in association with seizures did not affect epilepsy-related quality of life. It should be noted that even in those patients who report out-of-body experiences, such sensations are extremely rare events that do not occur routinely with their seizures. Most patients who reported out-of-body experiences described one or two experiences that occurred an indeterminate number of years ago, which precludes the possibility of associating the experience with the particular characteristics of that one seizure or with medications taken or other conditions at the time.

Abstract/Results: ABSTRACT:
INTRODUCTION:
The ability to process multiple domains of the human face it is a well-developed capability in humans, contributing significantly to social interaction. The extraction of emotional content out of facial features is one such domain, which involves well-known brain structures, whose detailed contribution is nevertheless poorly characterized.
OBJECTIVES:
Our emphasis is on detection and functional characterization of the brain areas involved in emotional processing of faces, with particular focus on the contribution of frontal lobes to processing of basic facial emotion expressions, such as fear and happiness. Methods: Participants were ten healthy volunteers and five patients with occipital lobe epilepsies. Mapping neurovascular (BOLD) responses to fear, happy and neutral facial expressions were obtained through functional Magnetic Resonance Imaging (fMRI).
RESULTS:
The results from the group of volunteers were used as a base for the construction of a quantitative database that included the following brain structures: fusiform gyrus, insula, amygdala, cingulate gyrus, and frontal-orbital cortex. The maximum Z-score obtained from each ROI as responses to the contrast fear-neutral faces produced higher statistically significant activations in the areas selected. Responses in clinical patients revealed focal impairments only in right hemisphere epilepsies, consistent with a hemispheric asymmetry for emotional processing.
CONCLUSIONS:
The frequent association of some brain structures involved in facial emotion processing in epilepsy suggests that the proposed mapping protocol can be clinically useful to gain deeper insights into the anatomical-functional correlations of this disease.

Abstract/Results: ABSTRACT:
Objectives: Dynamic changes in psychophysiological arousal are directly expressed in the sympathetic innervation of the skin. This activity can be measured as tonic and phasic fluctuations in electrodermal activity [Galvanic Skin Response (GSR)/skin conductance]. Biofeedback training can enable an individual to gain voluntary control over this autonomic response and its central correlates. Theoretically, control of psychophysiological arousal may be harnessed as a therapy for epilepsy, to mitigate pre-ictal states. Evidence is accumulating for the clinical efficacy of GSR biofeedback training in the management of drug resistant epilepsy. In this review, we analyse current evidence of efficacy with GSR biofeedback and evaluate the methodology of each study.
Method: We searched published literature pertaining to interventional studies of GSR biofeedback for epilepsy, through MEDLINE and Cochrane databases (1950-2018). Using percentage seizure reduction as an indicator of therapeutic efficacy induced by GSR biofeedback, we used meta-analytic methods to summarize extant findings. We also compare and contrast study design with relevance to the interpretation of outcomes.
Results: Out of 21 articles retrieved for GSR/EDA/Skin conductance biofeedback, four studies were identified as interventional trials, involving 99 patients with drug-resistant epilepsy in total. Three of these studies included a control group and a positive therapeutic effect of biofeedback was reported in each of these. The difference in seizure frequency percentage (Biofeedback-Control) was between -54.4 and -74.0% with an overall weighted mean difference of -64.3% (95% CI: -85.4 to -43.2%). The response rates (proportion of patients manifesting > 50% reduction in seizure frequency) varied from 45 to 66% across studies.
Significance: This timely evaluation highlights the potential value of GSR biofeedback therapy, and informs the optimal study design of larger scale studies that are now required to more definitively establish the utility of this non-invasive, non-pharmacological interventional approach for drug-resistant epilepsy.

The BIAL Foundation was created in 1994 by Laboratórios BIAL in conjunction with the Council of Rectors of Portuguese Universities. BIAL’s Foundation mission is to foster the scientific study of Man from both the physical and spiritual perspectives.
Along the years the BIAL Foundation has developed an important relationship with the scientific community, first in Portugal and after worldwide. Today it is an institution of reference which aims to stimulate new researches that may help people, promote more health and contribute to new milestones to gain access to knowledge.
Among its activities the BIAL Foundation manages the BIAL Award, created in 1984, one of the most important awards in the Health field in Europe. The BIAL Award rewards both the basic and the clinical research distinguishing works of major impact in medical research.
The BIAL Foundation also assigns Scientific Research Scholarships for the study of neurophysiological and mental health in people, arousing the interest of researchers in the areas of Psychophysiology and Parapsychology.
To date the BIAL Foundation has supported 461 projects, more than 1000 researchers, with research groups in twenty-seven countries, resulting, until April 2013, in about 600 full papers, out of which 172 published in indexed international journals with an average impact factor of 3.6 and a substantial number of citations (1665).
Since 1996 the BIAL Foundation organizes the Symposia entitled "Behind and Beyond the Brain", a Forum that gathers well renowned neurosciences speakers and the BIAL Foundation Fellows which are spread around the world.
Classified as an institution of public utility, the BIAL Foundation includes among its patrons the Portuguese President, the Portuguese Universities Rectors' Council and the Portuguese Medical Association.

In 1994 the BIAL Foundation launched a programme of science research grants with the aim of encouraging the research into Man’s physical and mental processes, namely in fields still largely unexplored but which warrant further scientific analysis, as Psychophysiology and Parapsychology.
Since its launch, applications to the BIAL grants have been increasing. Up to now 461 projects have been supported, involving more than 1000 researchers from 27 countries.
The approved applications have benefited from grants in amounts comprised between €5,000 and €50, 000. The amount to be granted is fixed by the Scientific board according to the needs of each project.
The supported projects have originated, until April 2013, in about 600 full papers, 172 out of which were published in indexed international journals with an average impact factor of 3.6 and a substantial number of citations (1665).
Among the BIAL Foundation fellows is worth highlighting the presence of scientists from prestigious universities from the United States, United Kingdom, Australia, Russia, Germany, Japan, France, Canada, and many others.
The BIAL grants are promoted biannually.

This manuscript considers the central but neglected role of the autonomic nervous system in the expression and control of seizures in epilepsy (small) and tics in burette Syndrome (TS). In epilepsy, consideration of autonomic involvement is typically confined to differential diagnoses (e.g., syncope), or in relation to Sudden Unexpected Death in Epilepsy (SUDEP). Investigation is more limited in burette Syndrome. The role of the autonomic nervous system in the generation and prevention of epileptic seizures is largely overlooked. Emotional stimuli such as anxiety and stress are potent causes of seizures and tic activity in epilepsy and TS, respectively. This manuscript will describe a possible neural mechanism by which afferent autonomic projections linked to cognition and behavior influence central thalamo-cortical regulation, which appears to be an important means for controlling both seizure and tic activity. It also summarizes the link between the integrity of the default mode network and autonomic regulation in patients with epilepsy as well as the link between impaired motor control and autonomic regulation in patients with TS. Two neurological conditions; epilepsy and TS were chosen, as seizures and tics represent parameters that can be easily measured to investigate influences of autonomic functions. The FDA biofeedback approach is anticipated to gain a strong position within the next generation of treatment for epilepsy, as a non-invasive technique with minimal side effects. This approach also takes advantage of the current practical opportunity to utilize growing digital health technology.
