The BIAL Foundation also assigns Scientific Research Scholarships for the study of neurophysiological and mental health in people, arousing the interest of researchers in the areas of Psychophysiology and Parapsychology.
To date the BIAL Foundation has supported 461 projects, more than 1000 researchers, with research groups in twenty-seven countries, resulting, until April 2013, in about 600 full papers, out of which 172 published in indexed international journals with an average impact factor of 3.6 and a substantial number of citations (1665).

Abstract/Results: ABSTRACT:
Several authors have shown that the hippocampus responds to painful stimulation and suggested that prolonged painful conditions could lead to abnormal hippocampal functioning. The aim of the present study was to evaluate whether the induction of persistent peripheral neuropathic pain would affect basic hippocampal processing such as the spatial encoding performed by CA1 place cells. These place cells fire preferentially in a certain spatial position in the environment, and this spatial mapping remains stable across multiple experimental sessions even when the animal is removed from the testing environment. To address the effect of prolonged pain on the stability of place cell encoding, we chronically implanted arrays of electrodes in the CA1 hippocampal region of adult rats and recorded the multichannel neuronal activity during a simple food-reinforced alternation task in a U-shaped runway. The activity of place cells was followed over a 3-week period before and after the establishment of an animal model of neuropathy, spared nerve injury. Our results show that the nerve injury increased the number of place fields encoded per cell and the mapping size of the place fields. In addition, there was an increase in in-field coherence while the amount of spatial information content that a single spike conveyed about the animal location decreased over time. Other measures of spatial tuning (in-field firing rate, firing peak and number of spikes) were unchanged between the experimental groups. These results demonstrate that the functioning of spatial place cells is altered during neuropathic pain conditions.

Abstract/Results: RESULTS:
This research project aims at understanding the alterations that occur following the onset of prolonged and stressful pain conditions, in the connectivity of brain areas that process rewarding and aversive stimuli and in areas critical for learning and memory.
For achieving our goal we use a combination of novel decision-making and working-memory operant tasks together with state-of-the-art multielectrode neurophysiology recordings in awake freely moving animals. In a typical experiment we chronically implant 16-32 tungsten microwires in up to 4 brain areas.
The results of this project showed that:
a) the neuronal firing rate in the orbitofrontal cortex was correlated with the probability of choosing a low versus high-risk food reward in each trial, and that chronic pain reduced the fraction of risk-aversive neurons;
b) neuropathic pain induces an increase in the number of place fields encoded by hippocampal neurons;
c) chronic pain changes the circadian behavioral state leading to a disruption of sleep patterns, and that there was a large decrease in the functional connectivity between the somatosensory cortex and somatosensory lateral thalamus;
d) pain increases neuronal activity in the amygdala triggering a decrease in prefrontal activation and impairing decision-making;
e) pain induces an impairment of working memory performance, decrease in single neuron activity in the mPFC, and reduction in the frontohippocampal connectivity correlated with correct performance.
f) congenital lack of pain in Prrxl1 KO mice causes a behavioural and neurophysiological pattern of brain activity that is the inverse of what we observed in animals with either neuropathic or inflammatory chronic pain.

It has been recently described that disruption of the neural mechanisms of emotion-based decision making occurs in both chronic pain patients and in animal models of pain; moreover, it also has been shown that chronic pain causes morphological and functional changes in the prefrontal cortex that may be crucial for this decision-making dysfunction. However, it is not known whether pain alone is capable of altering the neuronal encoding of decision exhibited by prefrontal neurons. We have previously shown that naïve animals have risk-averse performance in the rodent gambling task, whereas chronic pain animals reverse their choice preference and become risk prone. Using this paradigm, we chronically implanted arrays of multielectrodes and recorded from neuronal ensembles in the orbitofrontal cortex of freely moving animals performing 4 sessions of the rodent gambling task: 2 in control conditions and 2 after the onset of inflammatory pain induced by complete Freund’s adjuvant injection. Our results show that the instantaneous neuronal firing rate was correlated with the probability of choosing a specific lever in 62.5% of the neurons; however, although in the control sessions 61% of the neurons encoded the reward magnitude, after the pain onset only 16% of the neurons differentiated small from large rewards. Moreover, we found that the fraction of risk-sensitive neurons recorded in each session predicted the overall risk bias of the animal. Our data suggest that orbitofrontal cortex encoding of risk preference is compromised in chronic pain animals.

OBJECTIVES:
Placebo analgesia refers to the observation that pain is often reduced after administration of a placebo with information that it contains a powerful painkiller. The information has been found to generate an expectation of reduced pain, hypothesized to activate descending, endorphin mediated pain inhibitory mechanisms. This series of experiments tested the idea that reduced stress and negative emotions mediate the effect of the expectation of pain relief.

In all studies, placebo analgesic responding was defined as the difference in pain levels between a group or condition were subjects received inactive treatment with information that it reduced pain, and a group or condition where no treatment and no information was administrated. Pain was induced in healthy volunteers via a thermode where temperatures were increased (Experiments 1-4) or decreased (Experiment 5) to painful levels. Pain was recorded by visual analogue scales (VAS), by event related potentials (ERPs), or heart rate variability. Emotions were recorded by VAS and the Fear of Pain Questionnaire. Expectations were induced via information (Experiment 1-4) or via a classical conditioning procedure where pain levels were reduced after administration of placebo capsules to induce an expectation of reduced pain after intake of the capsules (Experiment 5). Placebos were capsules containing lactose (Experiments 1,2,4,5) or other procedures that could affect pain (diffuse noxious inhibitory control, Experiment 3).
RESULTS:
Information that a painkiller had been administrated reduced pain and event related potentials to painful stimulation. Regression analyses found that high levels of stress or negative emotions prior to onset of the experiment, predicted reduced placebo analgesia. Individuals that were fearful of pain had higher stress levels, more pain, and displayed reduced placebo analgesia. Increased pain after placebo administration was
observed in some of these individuals. Fear of pain predicted event related potential amplitude.
CONCLUSIONS:
Placebo responses could be seen in subjective and neurophysiological response to painful stimulation. Higher levels of negative emotions prior to painful stimulation reduced placebo analgesic responding, and subjective stress levels were lower in subjects receiving placebo compared to subjects not receiving any treatment. Thus, reduced negative emotions could be one mechanism in placebo analgesia.

Abstract/Results: ABSTRACT:
Early reports often ignored pain as an important symptom in multiple sclerosis (MS). Pain prevalence figures in MS from European countries other than Portugal range between 40 and 65%. To our knowledge there is no published data in English on pain in MS in Portugal. We describe the demographic and clinical characteristics, with an emphasis on pain, of 85 MS patients followed-up in a Portuguese hospital, contributing to pain epidemiology in MS. Patients were interviewed sequentially after their regular appointments at the MS clinic; patients with pain completed The Brief Pain Inventory and The McGill Pain Questionnaire (MPQ). The prevalence of pain found was 34%. Headache and back pain were the most common anatomical sites described, followed by upper and lower limbs. Intensity of pain in an 11-point scale was, for the maximum pain intensity 6.7?±?1.8, for the minimum pain intensity 2.2?±?2.0, for the mean pain intensity 4.5?±?1.5, and for the actual pain intensity 2.4?±?2.9. Pain interfered significantly with general activity, mood, work, social relations, and enjoyment of life. All MS patients with pain employed words from both the sensory and affective categories of the MPQ to describe it. Patient pain descriptions' included the word "hot-burning" in 59% of the cases, common in the report of central pain, but neuropathic pain medications were only used by 10% of them. Pain is an important symptom in Portuguese patients with MS, not only because of the high prevalence found, concordant with other European countries, but also because of its interference with quality-of-life.

Cognitive deficits such as impaired decision-making can be a consequence of persistent pain. Normal functions of the intact amygdala and prefrontal cortex are required for emotion-based decision-making that relies on the ability to assess risk, attribute value, and identify advantageous strategies. We tested the hypothesis that pain-related cognitive deficits result from amygdala-driven impairment of medial prefrontal cortical (mPFC) function. To do this, we used electrophysiological single-unit recordings in vivo, patch clamp in brain slices, and various behavioral assays to show that increased neuronal activity in the amygdala in an animal model of arthritis pain was accompanied by decreased mPFC activation and impaired decision-making. Furthermore, pharmacologic inhibition (with a corticotropin-releasing factor 1 receptor antagonist) of pain-related hyperactivity in the basolateral amygdala (BLA), but not central amygdala (CeA), reversed deactivation of mPFC pyramidal cells and improved decision-making deficits. Pain-related cortical deactivation resulted from a shift of balance between inhibitory and excitatory synaptic transmission. Direct excitatory transmission to mPFC pyramidal cells did not change in the pain model, whereas polysynaptic inhibitory transmission increased. GABAergic transmission was reduced by non-NMDA receptor antagonists, suggesting that synaptic inhibition was glutamate driven. The results are consistent with a model of BLA-driven feedforward inhibition of mPFC neurons. In contrast to the differential effects of BLA versus CeA hyperactivity on cortical-cognitive functions, both amygdala nuclei modulate emotional-affective pain behavior. Thus, this study shows that the amygdala contributes not only to emotional-affective but also cognitive effects of pain. The novel amygdalo-cortical pain mechanism has important implications for our understanding of amygdala functions and amygdalo-cortical interactions.

Abstract/Results: ABSTRACT:
Individuals undergoing treatment for a symptom or disease may react to the mere expectation of receiving treatment. A number of studies have shown that patients in pain report reduced pain after administration of an inactive agent, as long as the patients believe that they are receiving active medication. This is not just a reporting bias, since the reduction of pain can be partly blocked by systemic administration of naloxone, an opioid antagonist. Additionally, there is reduced central nervous system activation to painful stimuli in individuals who have been given a placebo and told it is a painkiller. These findings suggest that the expectation of pain relief generates central nervous system opioid activity that inhibits pain transmission to the cerebral cortex. These data suggest a model where stimuli associated with treatment, e.g., verbal information about the treatment, or the behaviour of ingesting a tablet, evokes an expectation of reduction in symptoms. The expectation leads to changes in central nervous system activity that reduces the symptom. Thus, the expectations activate a homeostatic system that corrects perturbations to the system via negative feedback.

Abstract/Results: RESULTS:
The concept of immersion describes a graded psychological experience that corresponds to the way in which we engage with tasks or stimuli. Selective attention is the psychological mechanism at the heart of an immersive experience. It is also known that selective attention may play an important role during the perception of pain, there is evidence that distraction from painful stimuli can increase pain tolerance. This convergence has led to a line of research where immersion in virtual tasks (e.g. computer games) is utilized for therapeutic purposes and pain relief. The first experiment adopted an Evoked Response Potential (ERP) methodology based on the auditory oddball paradigm. We developed a methodology where participants played a computer game whilst being exposed to a series of tones that were irrelevant to the game. Our participants were exposed to the oddball paradigm during several game conditions. The degree of demand experienced by participants was manipulated to yield easy, hard and impossible levels of difficulty. Participants experienced these three levels of difficulty on three display types: a small 5” display, a large TV display and a Head-Mounted Display (HMD. Our findings revealed that immersion was highest during hard/impossible levels of demand that increased the intrinsic motivation of the participant. However, we found no effect of display type on immersion.
Our second experiment focused on the motivational aspects of immersion and we once again exposed our participants to easy, hard and impossible levels of demand. Instead of using an auditory oddball task to capture ERP modulation, participants were exposed to repeated painful stimuli based on contact heat applied to the forearm. The aim of this experiment was to investigate whether ERP responses to painful stimuli were modulated by the level of game demand. This experiment failed to reveal any statistically significant findings and there were a number of methodological issues that may have been responsible for this null finding.

Abstract/Results: ABSTRACT:
The vestibular system processes information about head movement and orientation. No unimodal vestibular cortex has been identified in the mammalian brain. Rather, vestibular inputs are combined with many other sensory signals in the cortex. This arrangement suggests that vestibular input could influence processing in other sensory modalities. Here we show that vestibular stimulation differentially modulates two submodalities of the somatosensory system, increasing sensitivity to tactile input, and independently reducing sensitivity to nociceptive input. These modulations of touch and pain can clearly be distinguished from supramodal attentional effects of vestibular stimulation, because they are bilateral and operate in different directions. Outside the artificial conditions of laboratory stimulation, the vestibular system codes movements of the head, indicating a new relation between the body and the external world. We suggest the vestibular system participates in a form of sensory signal management, changing the balance between the various sensory systems as the relation between the body and the external environment changes. This sensory rebalancing may be a crucial element in the brain's capacity to reorient towards novel or salient features in the environment.

Abstract/Results: ABSTRACT:
The vestibular system is unique among the senses because of the entirely multisensory nature of its cortical projections. Neuroanatomical and neuroimaging studies show that vestibular stimulation activates somatosensory areas, and particularly the so-called parieto-insular vestibular cortex (PIVC) in the monkey, while deactivating visual areas. This arrangement suggests that vestibular input could influence sensory processing in other sensory modalities. Human clinical studies reported dramatic perceptual changes following caloric vestibular stimulation (CVS), including transient remission of tactile imperception and chronic pain in brain-damaged patients. We recently observed in healthy participants that CVS decreases perceptual thresholds for touch, but increases thresholds for pain. These results suggested a direct role of vestibular inputs in multisensory interactions with touch and pain. Although an enhancement in the somatosensory evoked potentials (N80) has been demonstrated, the neural mechanisms underlying vestibular-induced analgesia remain unclear.
To address this issue we delivered nociceptive laser stimuli to the left hand dorsum before (‘Baseline’ condition) and immediately after left cold CVS (‘CVS’ condition), and recorded the corresponding brain responses (laser-evoked potentials, LEPs) together with the single-trial ratings of subjective pain intensity. To control for non-specific effects (e.g. time-dependent habituation) both LEPs and intensity ratings were also recorded one hour after CVS (‘Post’ condition).
A significant analgesic effect was induced by left CVS, as reported in lower pain ratings in ‘CVS’ compared to both ‘Baseline’ and ‘Post’ (p=0.016 and p=0.001, respectively). No differences in pain ratings were found between ‘Baseline’ and ‘Post’. Left CVS reduced the amplitude of standard LEP peaks, relative to ‘Pre’ and ‘Post’, both in the early time-window of the N1 wave (all p<0.01) and in the later time-windows of the N2 (all p<0.005) and P2 waves (all p<0.05). There were trends for habituation of the N2 and P2 peaks between 'Pre' and 'Post' conditions (p=0.06 and p=0.10, respectively).
These results provide the first evidence that left CVS reduces the cortical responses to nociceptive laser stimulation, including components localised to the primary somatosensory cortex. Thus, the vestibular system directly modulates activity in somatosensory pathways. This analgesic effect induced by left CVS could involve either subcortical gating of the nociceptive input, or by a direct modulation of evoked somatosensory cortex activity, by concurrent vestibular inputs to the same area.

No unimodal vestibular cortex has been identified in the mammalian brain, yet vestibular input projects to several cortical areas. Therefore vestibular input could influence processing in other sensory modalities. We recently observed that caloric vestibular stimulation (CVS) decreases perceptual thresholds for touch, but increases thresholds for pain, suggesting a direct role of vestibular inputs in multisensory interactions with pain. The neural mechanisms of such vestibular-induced analgesia remain unclear. To address this issue we delivered nociceptive laser stimuli to the left hand dorsum before ('Baseline') and immediately after left CVS ('CVS'), and recorded the corresponding brain responses (laser-evoked potentials, LEPs) together with the single-trial ratings of subjective pain intensity. To control for non-specific effects (e.g. time-dependent habituation) both LEPs and intensity ratings were also recorded one hour after CVS ('Post'). CVS induced a significant analgesic effect, with lower pain ratings in 'CVS' compared to both 'Baseline' and 'Post' (p=0.016 and p=0.001, respectively). There were no differences in pain ratings between 'Baseline' and 'Post'. CVS reduced the amplitude of standard LEP peaks, relative to 'Pre' and 'Post', both in the early time-window of the N1 wave (all p< 0.01) and in the later time-windows of the N2 (all p< 0.005) and P2 waves (all p< 0.05). There were trends for habituation of the N2 and P2 peaks between ´Pre´ and ´Post´ conditions (p=0.06 and p=0.10, respectively). Thus, the most striking effect of the CVS was a strong reduction of cortical responses to nociceptive laser stimulation, including components localised to the primary somatosensory cortex. These results provide the first evidence that CVS modulates cortical responses elicited by nociceptive stimuli. The analgesic effect of CVS could involve either subcortical gating of the nociceptive input, or direct modulation of evoked somatosensory cortex activity, by concurrent vestibular inputs to the same area.

RESULTS:
CVS induced a significant analgesic effect, with lower pain ratings in 'CVS' compared to both 'Baseline' and 'Post' (p=0.016 and p=0.001, respectively). OBJECTIVE:
Chronic pain is believed to be related to a dysfunction of descending pain modulatory mechanisms. Functioning of descending pain modulation can be assessed by various methods, including conditioned pain modulation (CPM). CPM refers to the inhibition of one source of pain by a second noxious stimulus, termed the conditioning stimulus. This procedure can activate an endogenous pain inhibitory mechanism that inhibits early nociceptive processing. Chronic pain and anxiety disorders are more prevalent among females and it has been hypothesized that females react with more negative emotions towards unpleasant stimuli and this might be part of the explanation of greater pain sensitivity in females. The present study investigated whether expectations modulate the effect of conditioning stimulation on pain, subjective stress, and heart rate. In addition, we investigated whether the modulation of CPM by expectations differed between males and females.
METHODS:
Seventy-two subjects (including 36 women) received six noxious heat stimuli to the forearm. During three of these stimuli, a conditioning stimulus (cold-water bath) was applied to the contralateral arm in order to activate CPM. One third of the subjects were told that this would reduce pain (analgesia group), one-third that it would increase pain (hyperalgesia group), and one third received no information about its effect (no info group).
RESULTS:
Information that conditioning stimulation decreased or enhanced pain had the corresponding effect in females, but not in males. Conditioning stimulation increased stress, but not heart rate in females in the hyperalgesia group. A higher expectation of analgesia and lower stress during conditioning stimulation was associated with larger inhibitory CPM.
CONCLUSION:
These results suggest that reduced inhibitory CPM can be due to contextually induced cognitive and emotional factors and not necessarily a dysfunction of descending inhibitory pathways.

The BIAL Foundation also assigns Scientific Research Scholarships for the study of neurophysiological and mental health in people, arousing the interest of researchers in the areas of Psychophysiology and Parapsychology.
To date the BIAL Foundation has supported 461 projects, more than 1000 researchers, with research groups in twenty-seven countries, resulting, until April 2013, in about 600 full papers, out of which 172 published in indexed international journals with an average impact factor of 3.6 and a substantial number of citations (1665).
Since 1996 the BIAL Foundation organizes the Symposia entitled "Behind and Beyond the Brain", a Forum that gathers well renowned neurosciences speakers and the BIAL Foundation Fellows which are spread around the world.

Since its launch, applications to the BIAL grants have been increasing. Up to now 461 projects have been supported, involving more than 1000 researchers from 27 countries.
The approved applications have benefited from grants in amounts comprised between €5,000 and €50, 000. The amount to be granted is fixed by the Scientific board according to the needs of each project.

Abstract/Results: ABSTRACT:
The present study tested the impact of an acoustic prepulse on pain caused by an electrical shock to the arm. If pain can be inhibited by a prepulse, then prepulses might be added to diagnostic and therapeutic medical interventions in which pain is unavoidable in order to decrease patient discomfort and increase patient compliance. College students had shocks (150V, 0.5ms duration) delivered to the upper arm.On some trials this shockwas preceded by a 40ms broadband noise at 80 dB, with a lead interval of 80ms.Midway through the 16-trial session, participants were told that the prepulse would either increase or decrease the pain of the shock (N520 per instruction group). The only difference between Blocks 1 and 2 were these instructions; the actual prepulses were not changed. Participants rated the pain and unpleasantness of each shock on a 100mm visual analog scale (a separate sheet for each trial). Eyeblink EMG was measured from orbicularis oculi ipsilateral to the shocked arm. Rated pain in the shock-alone condition increased from Block 1 to Block 2 by 38%. In Block 1, the prepulse increased pain by 19%. In Block 2, the prepulse increased pain by 6% in the Increase group, and decreased pain by 5% in the Decrease group, relative to the control condition in Block 2. EMG activity elicited by the shock was not affected by the prepulse. These data show that an acoustic prepulse did not inhibit the pain of an intense shock to the arm, perhaps because the timing between the two stimuli was too long. A second study with a shorter lead interval will also be reported.
