The BIAL Foundation also assigns Scientific Research Scholarships for the study of neurophysiological and mental health in people, arousing the interest of researchers in the areas of Psychophysiology and Parapsychology.
To date the BIAL Foundation has supported 461 projects, more than 1000 researchers, with research groups in twenty-seven countries, resulting, until April 2013, in about 600 full papers, out of which 172 published in indexed international journals with an average impact factor of 3.6 and a substantial number of citations (1665).
Since 1996 the BIAL Foundation organizes the Symposia entitled "Behind and Beyond the Brain", a Forum that gathers well renowned neurosciences speakers and the BIAL Foundation Fellows which are spread around the world.
Classified as an institution of public utility, the BIAL Foundation includes among its patrons the Portuguese President, the Portuguese Universities Rectors' Council and the Portuguese Medical Association.

In 1994 the BIAL Foundation launched a programme of science research grants with the aim of encouraging the research into Man’s physical and mental processes, namely in fields still largely unexplored but which warrant further scientific analysis, as Psychophysiology and Parapsychology.
Since its launch, applications to the BIAL grants have been increasing. Up to now 461 projects have been supported, involving more than 1000 researchers from 27 countries.

Abstract/Results: Hippocampal dysfunctions are a core feature of schizophrenia, but conflicting evidence exists whether volumetric and morphological changes are present in early-stage psychosis and to what extent these deficits are related to clinical trajectories. In this study, we recruited individuals at clinical high risk for psychosis (CHR-P) (n=108), patients with a first episode of psychosis (FEP) (n=37), healthy controls (HC) (n=70) as well as a psychiatric control group with substance abuse and affective disorders (CHR-N: n=38). MRI-data at baseline were obtained and volumetric as well as vertex analyses of the hippocampus were carried out. Moreover, volumetric changes were examined in the amygdala, caudate, nucleus accumbens, pallidum, putamen and thalamus. In addition, we obtained follow-up functional and symptomatic assessments in CHR-P individuals to examine the question whether anatomical deficits at baseline predicted clinical trajectories. Our results show that the hippocampus is the only structure showing significant volumetric decrease in early-stage psychosis, with FEPs showing significantly smaller hippocampal volumes bilaterally alongside widespread shape changes in the vertex analysis. For the CHR-P group, volumetric decreases were confined to the left hippocampus. However, hippocampal alterations in the CHR-P group were not robustly associated with clinical outcomes, including the persistence of attenuated psychotic symptoms and functional trajectories. Accordingly, our findings highlight that dysfunctions in hippocampal anatomy are an important feature of early-stage psychosis which may, however, not be related to clinical outcomes in CHR-P participants.

In recent years, machine learning (ML) has been a promising approach in the research of treatment outcome prediction in psychosis. In this study, we reviewed ML studies using different neuroimaging, neurophysiological, genetic, and clinical features to predict antipsychotic treatment outcomes in patients at different stages of schizophrenia. Literature available on PubMed until March 2022 was reviewed. Overall, 28 studies were included, among them 23 using a single-modality approach and 5 combining data from multiple modalities. The majority of included studies considered structural and functional neuroimaging biomarkers as predictive features used in ML models. Specifically, functional magnetic resonance imaging (fMRI) features contributed to antipsychotic treatment response prediction of psychosis with good accuracies. Additionally, several studies found that ML models based on clinical features might present adequate predictive ability. Importantly, by examining the additive effects of combining features, the predictive value might be improved by applying multimodal ML approaches. However, most of the included studies presented several limitations, such as small sample sizes and a lack of replication tests. Moreover, considerable clinical and analytical heterogeneity among included studies posed a challenge in synthesizing findings and generating robust overall conclusions. Despite the complexity and heterogeneity of methodology, prognostic features, clinical presentation, and treatment approaches, studies included in this review suggest that ML tools may have the potential to predict treatment outcomes of psychosis accurately. Future studies need to focus on refining feature characterization, validating prediction models, and evaluate their translation in real-world clinical practice.

Abstract/Results: ABSTRACT:
Negative symptoms reflect a currently much-untreated loss of normal functioning and are frequently found in psychotic disorders. We present the first translation of the Brief Negative Symptom Scale (BNSS) to European Portuguese and evaluate its validity in a sample of Portuguese male patients with a psychotic spectrum disorder. The Portuguese BNSS showed excellent internal consistency, high convergent validity (i.e., strong correlation with the PANSS negative factor), and high discriminant validity (i.e., a lack of association with the PANSS positive factor). In sum, the present European Portuguese BNSS has shown to be reliable, thus extending this instrument's clinical availability worldwide.

Abstract/Results: ABSTRACT:
Negative symptoms in the context of psychosis are still poorly understood and diagnosed, which impairs the treatment efficacy of current therapies and patient's integration in society. In this study, we aimed to test hypothesis-based and exploratory associations of negative symptom domains, as defined by the Brief Negative Symptom Scale (BNSS), with hormonal and hematological variables, and, complementarily, with standard psychological/cognitive and psychopathological measures. Fifty-one male patients diagnosed with a psychotic disorder underwent a structured interview and blood collection. Standard Spearmen bivariate correlations were used for data analysis. We obtained evidence of hypothesis-based associations between specific negative symptoms and oxytocin, thyroid stimulating hormone levels and neutrophil-to-lymphocyte ratio; as well as novel and hypothesis-free associations with erythrocyte and lymphocyte count, mean corpuscular volume and red cell distribution width. Complementarily, we also obtained some validation of previous associations of negative symptoms with illness resolution, cognitive symptom severity and social performance, and a novel association with anger contagion. We hope our results can generate new hypotheses in psychosis research. Our work suggests further avenues in research on erythrocytic, inflammatory, thyroid and oxytocin-related markers and abnormalities in psychosis, especially in regards to specific negative symptoms, towards more precise and comprehensive etiological, diagnostic and therapeutic models.

Background
Studies of salivary cortisol levels in psychosis have yielded inconsistent findings, which may be attributable to heterogeneity in cortisol measurement, illness stage, and approaches to dealing with sampling factors and potential confounders. To address these issues, we performed an individual participant data (IPD) meta-analysis comparing individuals at different stages of psychosis to controls using five different salivary cortisol measures and explored potential effect modifiers.
Methods
Salivary cortisol data from five London-based cohorts were used to derive the cortisol awakening response, total daytime cortisol output, basal cortisol, and diurnal slope measures (wake-to-evening and peak-to-evening). Linear regression models were first performed to obtain standardised beta coefficients (ß), representing the difference in each cortisol metric between each clinical stage group (cases) and healthy individuals (controls) after accounting for relevant sampling factors; we then used random-effects meta-analyses and meta-regression models to investigate the effect of psychosis stage and sample characteristics on effect sizes.
Results
Data were available for 352 individuals distributed across psychosis clinical stages (1a – distress disorder: N = 35; 1b – clinical high-risk for psychosis: N = 90; 2a – first-episode psychosis: N = 197; 2b – single episode remitted: N = 5; 3 – relapsing/remitting illness: N = 18; 4 – severe and persistent illness: N = 7) and 292 controls. A significant overall main effect of clinical stage on peak-to-evening diurnal slope was observed (?2=12.83, p = 0.025), with both the clinical high-risk (ß=0.21, 95 % CI: 0.06, 0.36) and first-episode psychosis (ß=0.20, 95 % CI: 0.10, 0.31) groups characterised by flatter slopes than controls. The clinical stage groups and controls did not differ on any other cortisol measure. Several sample characteristics were significantly associated with diurnal slope effect sizes, but after accounting for clinical stage, only the association between mean age in cases and wake-to-evening diurnal slope retained significance.
Conclusion
Clinical high-risk and first-episode psychosis participants differed from healthy controls in the peak-to-evening diurnal cortisol slope. This measure has not been examined in these populations before, and its potential predictive and prognostic utility for psychotic disorders merits further investigation.

Hallucination research is a fast-growing, inherently interdisciplinary field bridging psychology, neuroscience, psychiatry, and philosophy. This article maps out key conceptual and methodological issues underlying the study of hallucinations. We begin by unpacking core theoretical issues - how hallucinations differ from other perceptual alterations, whether they form a single construct or several, and how these distinctions influence study design and interpretation. Next, we review the most commonly used experimental paradigms. A clear distinction is drawn between tasks that measure enduring hallucinatory tendencies and those that capture hallucinations in real time. We also review the most widely used rating instruments - including confidence scales - and discuss the phenomenological approach, which foregrounds participants' first-person experience. The final section offers a concise, though not exhaustive, checklist of variables researchers must account for - ranging from sensory modality and context to cognitive style, affective state, and cultural background. Taken together, the article serves as an entry-level guide, posing critical questions that every researcher should answer before designing a study on hallucinations.

Since 1996 the BIAL Foundation organizes the Symposia entitled "Behind and Beyond the Brain", a Forum that gathers well renowned neurosciences speakers and the BIAL Foundation Fellows which are spread around the world.

In 1994 the BIAL Foundation launched a programme of science research grants with the aim of encouraging the research into Man’s physical and mental processes, namely in fields still largely unexplored but which warrant further scientific analysis, as Psychophysiology and Parapsychology.

Abstract/Results: ABSTRACT:
Distinct patterns in the premorbid development of verbal and performance intelligence might differentiate the propensity for later illness among individuals putatively at risk for schizophrenia. This study applied linear mixed-effects models to four verbal and performance intelligence assessments gathered from a community sample of youth (n = 114) over ages 9–20 years, comparing developmental trajectories for at-risk youth with a triad of well-replicated antecedents of schizophrenia (ASz) and at-risk youth with a family history of schizophrenia (FHx), each relative to low-risk, typically developing peers (TD). Supplementary analyses explored heterogeneity among ASz youth according to persistence of symptoms, and FHx youth according to familial loading and presence of antecedents. Both ASz and FHx youth demonstrated early deficits (age ~11 years) in verbal intelligence relative to TD youth, which remained stable into young adulthood. FHx youth additionally showed this stable developmental deficit relative to TD youth in performance intelligence. In exploratory supplementary analyses, verbal intelligence improved with age toward TD performance among ASz children whose psychopathological symptoms remitted during follow-up. Uniquely among FHx youth, those with a low familial loading (one second-degree affected relative only) and none/one of the three antecedents did not evidence the stable developmental deficits in verbal and performance intelligence. These distinct developmental trajectories of verbal and performance intelligence among youth presenting different profiles of schizophrenia risk may signify potential for targeted preventative interventions. Further research is needed to determine whether these trajectories distinguish later transition to schizophrenia from a cognitive vulnerability that normalises among at-risk individuals who do not transition.

Abstract/Results: ABSTRACT:
Objective: Longitudinal investigation of brain alterations in children putatively at-risk of developing schizophrenia may identify early markers of pathophysiology associated with vulnerability for the disorder.
Methods: Children aged 9-12 years with multiple antecedents of schizophrenia (psychoticlike experiences; speech and/or motor delays; and social, emotional, and/or behavioural difficulties; ASz, n=31), with a family history of schizophrenia/schizoaffective disorder (FHx, n=21), and typically developing children (TD, n=35) were recruited through community screening using questionnaires. T1-weighted structural MRI scans were obtained at baseline, and after approximately 2- and 4-years of follow-up, and processed using the FreeSurfer longitudinal pipeline. Across these three assessment waves, total grey matter (GM) and white matter (WM) volume, and regional GM volumes in twelve regions of interest, were compared among the groups using linear mixed models.
Results: Children from three groups were 11.21 (±1.61) years at baseline and the average lapse-of-time between each assessment was 1.76 (±0.36) years. The ASz group exhibited consistently higher total WM volume (b=29.45) compared to the TD group, with no statistically significant time-related volumetric WM or GM differences in any region. The FHx group also demonstrated higher total WM volume (b=36.49), and time-related total and regional GM volume changes were observed; however, these effects were attenuated after correction.
Conclusion: Global and regionally-specific brain abnormalities distinguished children with different risk profiles for schizophrenia relative to their typically developing peers. Follow-up is required to determine how these changes may relate to later schizophrenia.

Trajectories of gray and white matter volume in children at elevated risk for schizophrenia

Abstract/Results: ABSTRACT:
Objective: Longitudinal investigation of brain alterations in children putatively at-risk of developing schizophrenia may identify early markers of pathophysiology associated with vulnerability for the disorder.
Methods: Children aged 9-12 years with multiple antecedents of schizophrenia (psychoticlike experiences; speech and/or motor delays; and social, emotional, and/or behavioural difficulties; ASz, n=31), with a family history of schizophrenia/schizoaffective disorder (FHx, n=21), and typically developing children (TD, n=35) were recruited through community screening using questionnaires. T1-weighted structural MRI scans were obtained at baseline, and after approximately 2- and 4-years of follow-up, and processed using the FreeSurfer longitudinal pipeline. Across these three assessment waves, total grey matter (GM) and white matter (WM) volume, and regional GM volumes in twelve regions of interest, were compared among the groups using linear mixed models.
Results: Children from three groups were 11.21 (±1.61) years at baseline and the average lapse-of-time between each assessment was 1.76 (±0.36) years. The ASz group exhibited consistently higher total WM volume (b=29.45) compared to the TD group, with no statistically significant time-related volumetric WM or GM differences in any region. The FHx group also demonstrated higher total WM volume (b=36.49), and time-related total and regional GM volume changes were observed; however, these effects were attenuated after correction.
Conclusion: Global and regionally-specific brain abnormalities distinguished children with different risk profiles for schizophrenia relative to their typically developing peers. Follow-up is required to determine how these changes may relate to later schizophrenia.
